Évaluation exploratoire de l'efficacité et de l'innocuité de la chimiothérapie au paclitaxel à base de gemcitabine-albumine associée à la SBRT dans le traitement des patients atteints d'un cancer du pancréas borderline résécable et localement avancé non résécable nouvellement diagnostiqués avec sélection séquentielle des investigateurs (IC).

Description détaillée
La question de savoir comment administrer une chimiothérapie adéquate pour synchroniser la stratégie de traitement SBRT afin de maximiser les avantages du traitement néoadjuvant pour améliorer le pronostic des patients atteints d'un cancer du pancréas limite résécable (BRPC) et localement avancé (LAPC) est une question difficile et discutable.
Aucune étude n'a encore évalué l'efficacité de la SBRT en cure fractionnée comme schéma de chimioradiothérapie néoadjuvante.
Les chercheurs avaient pour objectif d'étudier si la chimiothérapie néoadjuvante plus la SBRT en cours fractionné entraînaient de meilleurs résultats chez les patients BRPC et LAPC.

La description
Critère d'intégration:
- Âge ≥ 18 ans et ≤ 70 ans.
- Adénocarcinome du pancréas confirmé histologiquement ou cytologiquement.
- Cancer du pancréas limite résécable ou localement avancé prouvé par des examens d'imagerie via des approches multidisciplinaires selon les directives du NCCN
- Aucune chimiothérapie ou radiothérapie antérieure
- Statut de performance ECOG de 0 ou 1.
- Sans métastase à distance
- Le diamètre maximal de la tumeur ne doit pas dépasser 5 cm
- Paramètres hématologiques acceptables : a. Nombre absolu de neutrophiles (ANC) ≥1500 cellules/mm3 b. Numération plaquettaire ≥100 000/mm3 c. Hémoglobine (Hb)≥9 g/dL
- Niveaux de chimie sanguine acceptables : a. AST/SGOT et ALT/SGPT≤2.5× limite supérieure de la plage normale (LSN) b. Bilirubine totale≤1.5 LSN c. Phosphatase alcaline≤2.5× LSN d. Albumine sérique > 3 g/dL e. Créatinine sérique≤1.5 LSN
- Comprendre et signer volontairement un document de consentement éclairé avant que toute évaluation / procédure liée à l'étude ne soit effectuée.
Critère d'exclusion:
- Âge < 18 ans et > 70 ans. Traitement anticancéreux antérieur pour le cancer du pancréas.
- Présence ou antécédents d'adénocarcinome pancréatique métastatique.
- Patients ayant subi une chirurgie, une chimiothérapie ou d'autres traitements avant l'inclusion.
- Toute autre tumeur maligne dans les 5 ans précédant l'inscription
- Antécédents d'allergie ou d'hypersensibilité au nab-paclitaxel ou à la gemcitabine ou à l'un de leurs excipients.
- Neuropathie sensorielle périphérique Grade > 1
- Facteurs de risque médicaux graves impliquant l'un des principaux systèmes d'organes ou troubles psychiatriques graves.
- Enceinte ou allaitante.
- Patients inscrits à d'autres essais cliniques ou non conformes au suivi régulier
- Refus ou incapacité de se conformer aux procédures d'étude.

Expérimental: SBRT avec chimiothérapie concomitante
SBRT avec chimiothérapie nab-paclitaxel plus gemcitabine (nab-P+Gem)
|
Médicament : nab-Paclitaxel 125 mg/m2 IV pendant environ 30 à 45 min les jours 1 et 15, suivi d'une perfusion de gemcitabine 1000 mg/m2 IV pendant environ 30 min les jours 1 et 15 de chaque cycle de 28 jours pendant 6 cycles.
Radiation : Au cours des 1 et 2 premiers cycles de chimiothérapie, la SBRT a été administrée en une seule irradiation de 10 Gy ⅹ 4 fois (jours 2 et 16 de chaque cycle de 28 jours).

La survie sans progression (PFS) a été définie comme le temps écoulé entre la date de la première dose et la date de progression de la maladie ou de décès (quelle qu'en soit la cause), selon la première éventualité.
Le jour d'analyse a été calculé à partir de la date d'inscription pour un participant qui n'a pas été traité.
Les participants qui n'ont pas de progression de la maladie ou qui ne sont pas décédés ont été censurés à la dernière date d'évaluation de la tumeur sans progression.
