Op chemotherapie gebaseerde gesplitste stereotactische lichaamsbestralingstherapie voor resectabele borderline en lokaal gevorderde pancreaskanker: onderzoeksprotocol van een prospectieve, eenarmige fase II-studie
Verkennende beoordeling van de werkzaamheid en veiligheid van op gemcitabine-albumine gebaseerde paclitaxel-chemotherapie in combinatie met SBRT bij de behandeling van nieuw gediagnosticeerde borderline reseceerbare en lokaal gevorderde inoperabele alvleesklierkankerpatiënten met sequentiële onderzoekersselectie (IC).

De vraag hoe adequate chemotherapie moet worden toegediend om de SBRT-behandelingsstrategie te synchroniseren om de voordelen van neoadjuvante therapie voor een betere prognose van patiënten met borderline resectabele (BRPC) en lokaal gevorderde (LAPC) alvleesklierkanker te maximaliseren, is een uitdagend en discutabel probleem.
Er zijn nog geen studies die de werkzaamheid van split-course SBRT als het neoadjuvante chemoradiotherapieregime hebben geëvalueerd.
De onderzoekers wilden onderzoeken of neoadjuvante chemotherapie plus split-course SBRT leidt tot betere resultaten bij BRPC- en LAPC-patiënten.

Beschrijving
Inclusiecriteria:
- Leeftijd ≥ 18 jaar en ≤ 70 jaar.
- Histologisch of cytologisch bevestigd adenocarcinoom van de pancreas.
- Borderline resecabele of lokaal gevorderde alvleesklierkanker bewezen door beeldvormingsonderzoeken via multidisciplinaire benaderingen volgens de NCCN-richtlijnen
- Geen voorafgaande chemotherapie of radiotherapie
- ECOG-prestatiestatus van 0 of 1.
- Zonder uitzaaiingen op afstand
- De maximale diameter van de tumor mag niet groter zijn dan 5 cm
- Aanvaardbare hematologische parameters: Absoluut aantal neutrofielen (ANC) ≥1500 cellen/mm3 b. Aantal bloedplaatjes ≥100.000/mm3 c. Hemoglobine (Hgb) ≥9 g/dL
- Aanvaardbare bloedchemieniveaus: AST/SGOT en ALT/SGPT≤2,5× bovengrens van normaal bereik (ULN) b. Totaal bilirubine≤1,5 ULN c. Alkalische fosfatase≤2,5× ULN d. Serum albumine>3 g/dL e. Serumcreatinine≤1,5 ULN
- Een geïnformeerd toestemmingsdocument begrijpen en vrijwillig ondertekenen voordat er studiegerelateerde beoordelingen/procedures worden uitgevoerd.
Uitsluitingscriteria:
- Leeftijd < 18 jaar en > 70 jaar. Eerdere antikankertherapie voor pancreascarcinoom.
- Aanwezigheid van of geschiedenis van gemetastaseerd pancreasadenocarcinoom.
- Patiënten die vóór opname operaties, chemotherapie of andere behandelingen hebben ondergaan.
- Elke andere maligniteit binnen 5 jaar voorafgaand aan inschrijving
- Geschiedenis van allergie of overgevoeligheid voor nab-paclitaxel of gemcitabine of een van hun hulpstoffen.
- Perifere sensorische neuropathie Graad > 1
- Ernstige medische risicofactoren waarbij een van de belangrijkste orgaansystemen betrokken is, of ernstige psychiatrische stoornissen.
- Zwanger of borstvoeding.
- Patiënten die deelnamen aan andere klinische onderzoeken of patiënten die zich niet aan de reguliere follow-up houden
- Onwil of onvermogen om studieprocedures na te leven.

Experimenteel: SBRT met gelijktijdige chemotherapie
SBRT met nab-Paclitaxel plus Gemcitabine (nab-P+Gem) chemotherapie
|
Geneesmiddel: nab-Paclitaxel 125 mg/m2 IV gedurende ongeveer 30-45 min op Dag 1 en 15, gevolgd door gemcitabine 1000 mg/m2 IV infusie gedurende ongeveer 30 min op Dag 1 en 15 van elke cyclus van 28 dagen gedurende 6 cycli.
Bestraling: tijdens de eerste 1 en 2 cycli van chemotherapie werd SBRT gegeven als een enkele bestraling van 10 Gy ⅹ 4 keer (dag 2 en 16 van elke cyclus van 28 dagen).

Progressievrije overleving (PFS) werd gedefinieerd als de tijd vanaf de datum van de eerste dosis tot de datum van progressie van de ziekte of overlijden (door welke oorzaak dan ook), afhankelijk van wat zich het eerst voordoet.
De analysedag werd berekend vanaf de inschrijvingsdatum voor één deelnemer die niet werd behandeld.
Deelnemers die geen ziekteprogressie hebben of niet zijn overleden, werden gecensureerd tot de laatste tumorbeoordelingsdatum met progressievrij.
