この研究は、観察的で、同質的(単一の施設で実施)、非介入的であり、2つの部分で構成されています.1つはレトロスペクティブ、もう1つはプロスペクティブ(臨床試験の登録簿への登録を要求します)です。
2009 年 1 月から「San Raffaele Scientific Institute」(IRCCSSRaffaele) の放射線治療部門で補助的少分割放射線療法で治療された乳癌患者が特定され、その臨床および線量測定データが取得および分析されます。
この研究のレトロスペクティブ アームの目的は、5 年間の追跡調査による、急性毒性および晩期毒性、および局所および地域の制御、全生存、無病生存、がん特異的生存などの臨床転帰の分析です。 二次的な目的は、毒性と疾患進行の予後因子を特定することです。
研究への参加に同意した 500 人の連続した患者のコホートに対する研究の前向きアームの目的は、少分割短期コース (3 週間) 放射線療法で治療された患者の生活の質を評価することです。 欧州がん研究治療機構 (EORTC) の生活の質に関するアンケート (QLQ): QLQ-C30 (一般) および QLQ-BR23 (乳房専用) は、放射線治療の前に、次の場所で配布されます。患者の登録時、治療終了時およびフォローアップ訪問中、最初のフォローアップから 5 年間、最初は 6 か月に、その後は毎年スケジュールされます。
研究の非介入的で観察的な性質を考えると、標準的な部門プロトコルに従って治療された乳癌患者のフォローアッププログラムは変更されません。 患者は、放射線治療アーカイブから検索された医療記録からの患者データベースと、写真アーカイブからの局所進化を監視する画像から識別されます。 データは医療記録から抽出されます。 さらに、治療の線量測定データは物理的線量測定アーカイブから復元され、外挿されたデータは臨床および毒性結果と相関します。
前向き観察群の患者については、線量測定データと生活の質に関するアンケート、およびフォローアップ訪問の結果が前向きに収集され、データベースに登録され、その後分析されます。

*Sars-Cov2 パンデミック中の患者の参加拒否、患者の病院訪問を最小限に抑えるためのより短いスケジュールのプロトコルの採用、患者への同時統合ブーストの採用により、QOL 研究への登録は 2021 年 11 月に中断されました。局所再発のリスクが高く(TNBC、HER2+)、使用する技術の変更が必要。

除外基準:
- 男性乳がん患者
- 保存的手術と部分的な乳房照射で治療された乳癌患者、再発、保存的保存手術後のアジュバント全乳房放射線療法で治療された乳癌患者
- 乳房切除術を受けた乳がん患者
- 全乳房放射線療法および鎖骨上窩 +/- 腋窩放射線療法で治療された TNM 病期 N1a の患者
