この研究は、2 種類の疼痛管理方法を比較し、腹腔鏡下結腸直腸手術の術後にどちらがより効果的かを判断するために行われています。
グループ1は、背中に髄腔内(IT)注射を受け、少量のデュラモルフとブピバカインが脊髄液に注入されます。
グループ 2 は、ブピバカインとデカドロンが背中の筋肉の下の神経の近くに注射される脊柱起立面 (ESP) ブロックを受けます。

- -インディアナ大学病院またはメソジスト病院で待機的腹腔鏡下結腸直腸手術を受けている患者
- ASA クラス 1、2、3 (米国麻酔科学会身体状態分類システム)
- 18歳~80歳(男女問わず)
- BMI < 40kg/m2
- 術後疼痛管理のために局所麻酔を望む
除外基準:
-神経軸鎮痛またはESPブロック手順の禁忌
- 神経軸鎮痛の禁忌には、頭蓋内圧の上昇(仮性脳腫瘍の場合を除く)、注射部位の感染、患者からの同意の欠如、患者の拒否、脊椎で使用される薬物に対する真のアレルギー、および未修正の血液量減少が含まれます。 .
- ESP ブロック手順の禁忌には、注射部位での感染、患者の拒否、ブロックで使用される薬物に対する真のアレルギー、および患者の同意の欠如が含まれます。
- -腹腔鏡下腹会陰切除を受けているすべての患者。
- -調査官の意見では、手術に起因する術後の痛みの定量化を混乱させる可能性のある身体的、精神的、または医学的状態。
- -研究薬に対する既知の真のアレルギー(モルヒネ、ブピバカイン、デカドロン、タイレノール、セレブレックス)
- 毎日 30 mg 以上の経口モルヒネ相当量を摂取
- -過去6か月間の薬物乱用の履歴
- 末期肝疾患、末期腎疾患
- 体重50kg未満
