ACHTERGROND:
De pediatrische medische gemeenschap maakt zich al lang zorgen over de toegenomen incidentie van cerebrale disfunctie bij pasgeborenen met ernstige cyanotische congenitale hartziekte. Er bestaat bezorgdheid over een groot aantal potentiële predisponerende factoren, waaronder de effecten van hypoxemie en acidose, trauma tijdens hartkatheterisatie en de duur van langdurige verminderde arteriële zuurstofverzadiging. Veel baby's lopen ongetwijfeld aanzienlijke hersenbeschadiging op voorafgaand aan de operatie, maar kunnen tijdens de perioperatieve periode nog meer beledigd raken. De voorgestelde proef bestudeerde circulatiestilstand versus low-flow bypass-methoden van chirurgische ondersteuning bij een homogene groep patiënten die op identieke wijze werden behandeld in alle andere aspecten van hun behandeling. Belangrijke factoren, zoals de invloed van hartkatheterisatie en preoperatieve status, werden specifiek opgenomen in de primaire statistische analyse. Dit was belangrijk om significante predisponerende oorzaken van hersenletsel te identificeren die verder gaan dan de chirurgische technieken die worden vergeleken, en om de kracht van de vergelijking te vergroten.
ONTWERP VERHAAL:
Gerandomiseerd, gedeeltelijk geblindeerd. Baby's werden toegewezen aan diepe hypothermie en ofwel circulatiestilstand of low-flow bypass-ondersteuning tijdens chirurgisch herstel van de arteriële schakelaar van de transpositie van de grote slagaders. Randomisatie werd gestratificeerd naar diagnose (met of zonder septumdefect) en naar specifieke chirurg van drie, wat resulteerde in zes strata binnen elke behandelingsgroep. Randomisatie werd ook geblokkeerd om onevenwichtigheden in behandelingstoewijzing tussen strata te minimaliseren. De studie was gedeeltelijk geblindeerd, in die zin dat uitkomstgegevens werden verkregen door onderzoekers die niet op de hoogte waren van management, chirurgische zorg en intensive care waren gestandaardiseerd. Hemodynamische beoordeling werd vroeg in de postoperatieve periode uitgevoerd. De uitkomsten van primair belang (beeldvorming met magnetische resonantie, neurologisch onderzoek en psychometrische tests) werden beoordeeld tijdens de eenjarige evaluatie, om de relevantie voor de langetermijnprognose te maximaliseren. Preoperatief en zeven tot tien dagen postoperatief neurologisch onderzoek werd ook uitgevoerd. De ontwikkelingsstatus na één jaar werd gemeten met de Bayley-ontwikkelingsschalen en de Fagan-test voor intelligentie bij baby's. De hersenfunctie werd na één jaar beoordeeld door neurologisch onderzoek. De aanwervingsperiode liep van april 1988 tot februari 1992.
De proef werd verlengd tot en met december 1997 om vier jaar follow-up mogelijk te maken. De follow-up maakte de beoordeling mogelijk van een primaire uitkomstvariabele in het neurologische domein en een primaire uitkomstvariabele in het ontwikkelingsdomein. De primaire uitkomstvariabele van het neurologisch onderzoek resulteerde in een geordende classificatie van drie categorieën van normale, kleine en grote disfunctie, zoals apraxie of hersenverlamming. Ontwikkeling werd beoordeeld met behulp van gestandaardiseerde metingen van cognitie, gedrag, taal, aandacht en motorische vaardigheden. De primaire uitkomstvariabele voor de ontwikkelingsevaluatie was het intelligentiequotiënt op de Wechsler Preschool and Primary Scale of Intelligence (WPPSI-R). Secundaire uitkomstvariabelen omvatten metingen van gedrag en prestaties in specifieke domeinen van spraak en taal, visuele motorische integratie, aandacht, motorische functie en temperament.
Beginnend in januari 1998, werd de studie met nog eens vier jaar verlengd tot en met december 2001 om de effecten van de twee intraoperatieve methoden met betrekking tot de ontwikkelings- en neurologische status op achtjarige leeftijd te vergelijken. De ontwikkelingsstatus werd beoordeeld door de Wechsler Individual Achievement Test (primaire uitkomst) en door de aanwezigheid van leerstoornissen en prestaties in specifieke neuropsychologische domeinen (secundaire uitkomst). De neurologische status werd bepaald door neurologisch onderzoek (primaire uitkomst) en door specifieke soorten disfunctie zoals motorische problemen. De studie vergeleek ook de academische en intellectuele prestaties van het cohort op achtjarige leeftijd met populatienormen en gebruikte de gegevens om correlaties te identificeren van de resultaten van acht jaar van de kinderen, evenals hun ontwikkeling tussen de leeftijd van één en acht jaar.
