Beoordeling van de werkzaamheid en veiligheid van thioctinezuur bij de orale behandeling van symptomatische diabetische neuropathie (SYDNEY 2) Gerandomiseerde, dubbelblinde, placebogecontroleerde multicentrische studie met 4 parallelle groepen
Het primaire doel van het onderzoek is het bepalen van de optimale dosis van oraal (tablet) toegediend thioctinezuur bij de behandeling van symptomen van diabetische polyneuropathie (dPNP). Verwacht wordt dat ten minste één van de drie te testen doseringen (600, 1200 of 1800 mg tabletten) van oraal toegediend thioctinezuur de symptomen van dPNP verbetert in vergelijking met placebo.
Secundaire doelstellingen zijn evaluaties van andere variabelen die relevant zijn voor dPNP, veiligheid en verdraagbaarheid.

Beschrijving
Inclusiecriteria:
- Diabetes mellitus (type I of II), zoals gedefinieerd door de American Diabetes Association, 1997, duurt 1 jaar.
- Patiënt moet een symmetrische sensomotorische perifere polyneuropathie van ten minste stadium 2 hebben die toe te schrijven is aan diabetes mellitus na een grondige evaluatie van andere oorzaken van neuropathie.
- HbA1C < 10%.
- TSS > 7,5 punten.
- NISLL > 2 punten.
- Pijnsensatie (volgens speldenprikgevoeligheidstest) afwezig of verminderd (NIS item 35 1).
- Leeftijdscategorie: 18 tot 74 jaar. Inclusiecriteria voorafgaand aan randomisatie
- De TSS moet > 5 punten zijn
- Het TSS-bereik (maximale TSS minus minimale TSS tijdens de inloopfase) moet kleiner zijn dan 3 punten om opname van patiënten met snel wisselende symptomen te voorkomen.
- Minstens 1 van de 4 symptomen van de TSS moet de afgelopen 3 maanden onafgebroken zijn voorgekomen.
- Gebrek aan therapietrouw, d.w.z. minder dan 85% of meer dan 115% (daarom zouden 6 dagelijkse doses van de 7 dagelijkse doses die zijn gepland voor fase A acceptabel zijn).
Uitsluitingscriteria:
Gebrek aan geschiktheid voor het proces:
- Proximale asymmetrische neuropathie, craniale neuropathieën, rompradiculopathie, diabetische plexopathieën of acute of actieve mononeuropathieën (waaronder craniale neuropathieën, postherpesneuralgie, enz.), met uitzondering van carpaaltunnelsyndroom (CTS) of late ulnaire neuropathie (TUN) of beide.
- Neuropathie met een andere oorzaak dan diabetes mellitus die de beoordeling van de ernst van dPNP zou kunnen verstoren.
- Andere neurologische aandoeningen die zwakte, sensorisch verlies of autonome symptomen of testafwijkingen kunnen veroorzaken.
- Myopathie van welke oorzaak dan ook.
- Perifere vaatziekte ernstig genoeg om claudicatio intermittens of ischemische ulcera of ischemie van ledematen te veroorzaken.
- Patiënten met prolifererende retinopathie die onmiddellijk therapie nodig hebben en dreigende blindheid.
- Psychiatrische, psychologische of gedragssymptomen die het vermogen van de patiënt om aan het onderzoek deel te nemen zouden kunnen belemmeren.
- Patiënten met een actieve neoplastische ziekte behalve basaalcelcarcinoom.
- Patiënten met atriumfibrilleren, tenzij gecontroleerd en gestabiliseerd door medicatie.
- Patiënten met een klinisch significante cardiale, pulmonaire, gastro-intestinale, hematologische of endocriene ziekte (anders dan diabetes) die de interpretatie van de onderzoeksresultaten kan verwarren of de patiënt ervan kan weerhouden het onderzoek af te ronden.
- Patiënten die een orgaantransplantatie hebben ondergaan, van welke aard dan ook.
- Patiënten met significante lever- of nierziekte (ASAT, ALAT of GGT >2 keer normaal, serumcreatinine >1,8 mg/dl (>159 mmol/l) voor mannen of >1,6 mg/dl (>141 mmol/l) voor vrouwen) .
- Patiënten met een recente geschiedenis (in de afgelopen 12 maanden) van drugs- of alcoholmisbruik.
- Gebruik van een onderzoeksgeneesmiddel (deelname aan een klinische proef) in de afgelopen 1 maand.
- Geschiedenis van ernstige of anafylactische reactie op medicijnen, zwavel of biologische producten.
- Recente (in de afgelopen 3 maanden) ketoacidose of hypoglykemie, waarvoor ziekenhuisopname noodzakelijk was.
- Behandeling met antioxidanten (vitamine E > 400 IE, C > 200 mg en beta-caroteen > 30 mg) of pentoxyfylline binnen de laatste 1 maand voor aanvang van de proef.
- Gebruik van thioctinezuur (> 50 mg), teunisbloemolie of een andere stof die gamma-linoleenzuur bevat in de afgelopen 3 maanden.
- Voortgezet gebruik van medicijnen vermeld in rubriek 6.2.3.
- Bilaterale biopten van de nervus suralis.
Bestaande voetulcera.
Bezorgdheid over de veiligheid:
- Zwangere vrouwen of vrouwen die borstvoeding geven: Zwangerschap zoals blijkt uit een positieve b-hCG-test tijdens het screeningsbezoek of uit tests die op verzoek op de onderzoekslocatie zijn uitgevoerd, of vrouwen in de vruchtbare leeftijd die geen adequate anticonceptie gebruiken.
Voorgeschiedenis van allergische reactie op de onderzoeksmedicatie of de hulpstoffen.
Administratieve redenen:
- Geïnformeerde toestemming is niet ondertekend of de patiënt is niet volledig bekwaam om mee te werken.
- Eventuele taalbarrières die een adequaat begrip kunnen beïnvloeden.
- Verwachte niet-beschikbaarheid voor studiebezoeken/procedures.
- Kwetsbare personen (zoals gedetineerden)
