Een arterieel anuerysma is een permanente, plaatselijke dilatatie van een slagader met een diametertoename die ≥ 50% groter is dan die van de normale slagader. Hoewel elke slagader een aneurysma kan ontwikkelen, worden ze het meest gezien in de abdominale aorta, de thoracale aorta, de arteria poplitea en de arteria iliaca communis.
Het gebruik van intravasculaire stents en endoluminale transplantaten om natuurlijke arteriële aneurysma's uit te sluiten of occlusieve vasculaire laesies te behandelen, is geëvalueerd in een aantal preklinische studies. Endovasculaire implantatie van een stentgraft maakt buikchirurgie overbodig door de perifere slagaders te gebruiken als route naar het aneurysma, en stents bieden een ander middel voor het bevestigen van een transplantaat dan hechtingen. Het doel van endoluminale transplantatie is hetzelfde als dat van conventioneel herstel en maakt het inbrengen mogelijk van een veerkrachtige leiding tussen de uiteinden van het aneurysma om het uit de bloedsomloop te houden en scheuren van het aneurysma te voorkomen.
Dit is een klinische fase 2-studie van het Bifurcated PowerLink-systeem (endoluminale transplantatie) voor de behandeling van abdominale infrarenale aorto-iliacale aneurysmale ziekte (AAA). Diagnostische beeldvormingsmethoden zoals spiraal-CT-scans, angiografie, echografie en fluoroscopie zullen worden gebruikt om de locaties voor plaatsing van het apparaat te kiezen en om een nauwkeurige plaatsing te verzekeren.
De plaatsingskatheter maakt endovasculaire plaatsing van het hulpmiddel (endoluminale graft) mogelijk via retrograde (femorale of iliacale slagaders) of antegrade (brachiale slagaders) benaderingen. De endoluminale prothese (ELG) is een zelfuitzettende stentkooi van een metaallegering, die aan de buitenkant is bedekt met dunwandig ePTPE-transplantaatmateriaal. Het transplantaatmateriaal wordt volledig ondersteund over de gehele ELG-lengte. De ELG is ook verkrijgbaar in een gevorkte configuratie. De gebifurceerde plaatsingskatheter is verkrijgbaar in verschillende diameters en werklengtes. De katheter bestaat uit een inbrenghuls met homeostatische klep en een binnenschacht. De binnenschacht is een obturator met een taps toelopende punt die door middel van een hypotube is verbonden met een achterste obturator. Het hoofdgedeelte van de ELG wordt rond de hypotube samengedrukt en vervolgens in de plaatsingskatheter geladen. De contralaterale en ipsilaterale ledematen van de ELG worden samengedrukt in hun respectievelijke ledematenafdekkingen en in de plaatsingskatheter geladen. De binnenschacht biedt plaats aan een standaard .035 inch voerdraad.
De gelijktijdige deelname van chirurgische controle- en testpatiënten aan het onderzoek omvat inschrijving, de procedure voor het onderzoeksapparaat of chirurgische reparatie en follow-upperiode. Patiëntgegevens worden verzameld tijdens: preoperatieve, operatieve en postoperatieve follow-up bij ontslag en na 1 maand. De follow-up op lange termijn voor zowel de gelijktijdige chirurgische controles als de testpatiënten zal gedurende 6 en 12 maanden worden voortgezet ter ondersteuning van een PMA. Verlengde follow-up kan nodig zijn totdat het onderzoeksapparaat is goedgekeurd onder een PMA, of tot 5 jaar follow-up onder Post Market Surveillance-vereisten.

Beschrijving
Inclusiecriteria:
- ≥ 18 jaar oud
- Geïnformeerde toestemming begrepen en ondertekend
- Voldoet aan de follow-upvereisten na de behandeling
- Kandidaat voor conventioneel open chirurgisch herstel
Criteria voor anatomische inclusie:
- Aneurysma is ≥ 4,0 cm in buitendiameter of Sacculair aneurysma ≥ 3,0 cm in buitendiameter of Aneurysma ≥ tweemaal de normale aorta buitendiameter of snel groeiend aneurysma (≥ 5 mm gedurende 6 maanden)
Uitsluitingscriteria:
- Levensverwachting < 2 jaar
- Deelname aan een ander klinisch onderzoek
- Zwangere of zogende vrouwen
- Acuut gescheurd of lekkend aneurysma, of vasculair letsel als gevolg van trauma
- Patiënt heeft andere medische of psychiatrische problemen die naar de mening van de onderzoeker hem beletten deel te nemen aan het onderzoek.
- Contrastmiddel of antistollingsmiddelen zijn gecontra-indiceerd
- Coagulopathie of bloedingsstoornis
- Actieve systemische of gelokaliseerde liesinfectie
- Inferieure mesenteriale slagader is onmisbaar
- Bindweefselziekte (bijv. syndroom van Marfan)
- Creatininegehalte > 1,7 mg/dl
- Trombus op implantatieplaatsen
