Effectiviteitsonderzoek van een op dissonantie gebaseerd preventieprogramma voor obesitas (Project Health)

Doel 1 test de hypothese dat Project Health tijdens een follow-up van 2 jaar significant de toename van het percentage lichaamsvet vermindert ten opzichte van de interventie Gezond Gewicht en videocontrole; secundaire uitkomstmaten zijn BMI, depressieve symptomen en eetstoornissymptomen. De onderzoekers zullen Gezond Gewicht ook vergelijken met videocontroles.
Doel 2 test de hypothese dat deelnemers aan Project Health meer cognitieve dissonantie (een ongemakkelijke psychologische toestand) zullen ervaren met betrekking tot het eten van ongezond voedsel en sedentair gedrag, en dat veranderingen in dissonantie de preventieve effecten van het programma op gewichtstoename mediëren. De onderzoekers zullen ook testen of verminderde calorie-inname en verhoogde fysieke activiteit de effecten van Project Health op verandering in % lichaamsvet mediëren.
Doel 3 test de hypothese dat interventie-effecten worden versterkt voor deelnemers met een verhoogde ontevredenheid over het lichaam (wat de motivatieverandering zou moeten vergroten) en voorkeur voor consistentie (wat dissonantie-inductie zou moeten maximaliseren) en verzacht voor deelnemers die emotioneel eten en middelengebruik rapporteren (omdat deze factoren verhoogd risico op overeten).

ACTIVE_COMPARATOR: Gezond gewicht
Het belangrijkste doel van de Gezond Gewicht-interventie is om wekelijks kleine, duurzame veranderingen door te voeren in de input en output om een balans te bereiken tussen de calorie-inname en -output.
Alle sessies beginnen met een korte terugblik op wat er in de vorige sessie aan bod is gekomen, presentatie van educatieve hand-outs, zorgvuldige beoordeling van eerdere doelen voor gedragsverandering en de ontwikkeling van plannen voor gezonde gedragsverandering voor de volgende sessie.
Thuisoefeningen voor alle sessies bestaan uit het volgen van individuele dieet- en bewegingsdoelen, en het bijhouden van een eet- en bewegingslogboek om gebieden te bepalen voor toekomstige gezonde veranderingen.

EXPERIMENTEEL: Projectgezondheid
Project Gezondheid voegt dissonantieopwekkende activiteiten, discussies en huiswerkactiviteiten toe aan de basisinterventie Gezond Gewicht.
Elke sessie begint met een mondelinge toezegging om deel te nemen (om de vrijwillige aard van deelname te onderstrepen), omvat besprekingen van voltooide huiswerkopdrachten en schrijf-/deeloefeningen tijdens de sessie (om verantwoordelijkheid te creëren), en eindigt met huisoefeningen (om het niveau van poging).
Voltooide huisopdrachten worden in volgende sessies op video opgenomen om de verantwoordelijkheid te vergroten.

Sommige deelnemers worden gerandomiseerd om de conditie te beheersen, waarbij ze een psycho-educatieve video ("Weight of the World") krijgen om te bekijken.

We gebruiken luchtverplaatsingsplethysmografie (ADP) via de Bod Pod S/T om het percentage lichaamsvet te bepalen.
Twee maten worden gemiddeld.
De lichaamsdichtheid, berekend als lichaamsmassa gedeeld door het geschatte lichaamsvolume, wordt gebruikt om voor leeftijd en geslacht gecorrigeerde percentielscores voor lichaamsvet te berekenen, op basis van normatieve referentiegegevens (McCarthy et al., 2006).
Een pilotvaliditeitsonderzoek (N = 30) onthulde dat directe DEXA-metingen van % lichaamsvet veel sterker correleren met ADP-schattingen van lichaamsvet (r = .88)
dan bij BMI (r = .23).
Verandering in voor leeftijd en geslacht gecorrigeerd lichaamsvetpercentiel gedurende de follow-up van 2 jaar zal het primaire resultaat zijn.
Deze beoordeling duurt 3-5 minuten.

Pathologie eten

Het eetstoornis diagnostisch interview, een kort semi-gestructureerd interview, zal de symptomen van de eetstoornis beoordelen.
Het biedt diagnoses voor anorexia nervosa, boulimia nervosa en eetbuistoornis, waardoor we deelnemers met deze stoornissen kunnen uitsluiten.
Het biedt ook een continue meting van de algehele eetstoornissymptomen.

De subschaal Negative Affect van 20 items van het Positive and Negative Affect Schedule (PANAS: Watson et al., 1988) meet algemene negatieve affectiviteit en depressieve symptomen.
Met behulp van een Likert-beoordelingssysteem kiezen deelnemers hun mate van goedkeuring van items met negatieve affectiviteit, zoals 'walgen van zichzelf' en 'eenzaam' (antwoordopties: 1 = helemaal niet; 5 = extreem).
