Allogene microbiota-reconstitutie (AMR) voor de behandeling van patiënten met door diarree overheersend prikkelbare darmsyndroom - de AMIRA-studie
De onderzoekers zullen een multicenter, 2:1 gerandomiseerde, dubbelblinde, placebo-gecontroleerde studie van AMR uitvoeren bij patiënten met overheersende diarree-PDS (PDS-D), gediagnosticeerd volgens Rome III-criteria en de IBS-QOL-vragenlijst. Centrale aanvoer en kwaliteitscontrole van donormateriaal zal worden gebruikt om bias te beheersen.
Primair eindpunt is verbetering van IBS-SSS (Severity Score System) ten opzichte van baseline. Secundaire eindpunten zijn veranderingen in IBS-QOL, veiligheid op korte termijn en een jaar follow-up om effecten op lange termijn te beheersen, veiligheid en veranderingen in en acceptatie van het donormicrobioom na AMR met behulp van 16S rDNA-sequencing en kwantitatieve diversiteitsanalyse.

Deze studie beoordeelt allogene microbiota-reconstitutie (AMR) als nieuwe behandeling om de symptomen en kwaliteit van leven van patiënten met diarree-overheersend prikkelbare darmsyndroom (PDS-D) te verbeteren.
De onderzoekers zullen een prospectieve multicenter, 2:1 gerandomiseerde, dubbelblinde, placebo-gecontroleerde studie van AMR uitvoeren bij patiënten met IBS-D gediagnosticeerd volgens de Rome III-criteria en de IBS-QOL-vragenlijst.
De experimentele interventie is een infusie van donorfaeces via gastroscopie. De placebo-interventie is een infusie van steriele zoutoplossing via gastroscopie.
Gepland aantal patiënten dat deelneemt aan de studie: 42 patiënten Geplande groep per protocol: 33 patiënten

Beschrijving
Inclusiecriteria:
- schriftelijke geïnformeerde toestemming
- prikkelbaredarmsyndroom van het type met overheersende diarree volgens de ROME III-criteria
- Symptomen gedurende > 1 jaar vóór opname in het onderzoek
- aanhoudende symptomen > 1 jaar vóór opname in de studie
- relevante symptomen met verminderde kwaliteit van leven (PDS-QOL < 60 punten)
- geen specifieke bevindingen bij gastroscopie en colonoscopie met biopten in de afgelopen 2 jaar
Uitsluitingscriteria:
- chronische ontstekingsziekten
- gastro-intestinale infectieziekten
- microscopische colitis
- coeliakie
- diarree veroorzaakt door fructose- of lactose-intolerantie
- gastro-intestinale maligniteiten of darmpoliepen
- prikkelbaredarmsyndroom van een ander type dan PDS-D
- gal zure diarree
- constipatie
- symptomen veroorzaakt door andere ziekten dan PDS-D
- Dementie
- buikoperatie in de afgelopen maanden
- antibioticatherapie in de afgelopen 3 maanden
- zwangerschap
- taalbarrière voor geïnformeerde toestemming
