MRI-geleide SIB-radiotherapie bij neoadjuvante therapie voor gevorderde rectumkanker
Werkzaamheid van MRI-geleide gelijktijdige geïntegreerde boost-radiotherapie bij neoadjuvante therapie voor gevorderde rectumkanker: een gerandomiseerde, gecontroleerde fase III klinische studie
Colorectale kanker heeft een jaarlijkse incidentie van meer dan 700.000 gevallen wereldwijd, en staat daarmee op de vierde plaats van de meest voorkomende kanker met het op een na hoogste sterftecijfer. Rectale kanker is verantwoordelijk voor ongeveer een derde van de nieuw gediagnosticeerde gevallen van colorectale kanker. Stadium II (cT3-4/N0) en III (cT1-4/N1-3) rectumkanker worden gewoonlijk geclassificeerd als lokaal gevorderd rectumcarcinoom (LARC), gekenmerkt door een hoog risico op lokaal recidief na de operatie. Neoadjuvante chemoradiotherapie (CRT) in combinatie met totale mesorectale excisiechirurgie (TME) heeft het 5-jaars lokaal recidiefpercentage bij LARC verlaagd van 25% naar 5% -10%. Momenteel controleert neoadjuvante CRT+TME+postoperatieve adjuvante chemotherapie lokaal recidief bij LARC effectief. Het wordt echter in verband gebracht met lage percentages Pathologische Complete Regressie (pCR), suboptimaal behoud van de sluitspier, toegenomen metastasen op afstand, heterogene therapietrouw aan adjuvante chemotherapie en beperkte overlevingsvoordelen op de lange termijn.
Verdere optimalisatie van neoadjuvante behandelstrategieën is veelbelovend voor het bevorderen van tumorregressie en het verbeteren van de prognose. Bij neoadjuvante therapie is de mate van regressie van de rectumtumor sterk afhankelijk van radiotherapie, waarbij hogere stralingsdoses correleren met hogere percentages pathologische regressie. Deze studie heeft tot doel de rol van MRI-geleide radiotherapie met een gelijktijdige geïntegreerde boost in het verbeteren van tumor-pCR bij neoadjuvante behandeling van lokaal gevorderde rectumkanker te onderzoeken.

Inclusiecriteria:
- Histopathologisch bevestigd rectaal adenocarcinoom.
- Tumor op ≤10 cm van de anale rand.
- Leeftijd ≥18 jaar.
- Prestatiestatus van de Eastern Cooperative Oncology Group (ECOG PS) 0-1.
- Primaire behandelingsnaïeve tumor bevestigd door endorectale echografie (ERUS) of -
- Magnetische resonantiebeeldvorming (MRI) als cT3-4/N+ volgens de 8e editie van AJCC-stadiëring.
- Mogelijkheid om weefsel- en bloedmonsters te verstrekken voor translationeel onderzoek.
- Verwachte overleving van ≥6 maanden.
- Normale functie van belangrijke organen (binnen 14 dagen voorafgaand aan inschrijving) en geschiktheid voor chemoradiotherapie.
Uitsluitingscriteria:
- Geschiedenis van eerdere chemotherapie, radiotherapie of chirurgische behandeling voor rectumkanker, inclusief transanale tumorresectie.
- Lokaal terugkerende rectumkanker.
- Geschiedenis van familiale adenomateuze polyposis.
- Actieve ziekte van Crohn of colitis ulcerosa.
- Allergie of overgevoeligheidsgeschiedenis voor 5-fluorouracil (fluorouracil) en/of oxaliplatine.
- Geschiedenis van problemen of onvermogen om orale medicatie in te nemen of te absorberen.
- Diagnose van een andere maligniteit dan rectumkanker in de afgelopen 5 jaar (met uitzondering van volledig genezen basaalcelcarcinoom, plaveiselcelcarcinoom van de huid en/of in situ carcinoom behandeld met radicale resectie).
- Bevestigde metastase op afstand, d.w.z. cM1, door middel van beeldvorming of biopsie.
- Geschiedenis van bekkenradiotherapie.
- Zwangere vrouwen of vrouwen die borstvoeding geven.
- Aanwezigheid van een ernstige of oncontroleerbare systemische ziekte.

Experimenteel: Online ART With Sequential Boost
Participants received online adaptive radiotherapy using MR- or CBCT-guided workflows.
A sequential boost of either 9 Gy in 3 fractions or 10 Gy in 2 fractions was delivered to the primary rectal tumor (GTVp) and clinically involved lymph nodes (GTVn), followed by continued online ART to 50 Gy in 25 fractions to the pelvic clinical target volume.
Concurrent capecitabine was administered during radiotherapy, followed by 3-4 cycles of CAPEOX consolidation chemotherapy and planned total mesorectal excision.

Actieve vergelijker: Conventional Non-Adaptive IMRT
Participants received conventional non-adaptive intensity-modulated radiotherapy to the pelvic clinical target volume at 50 Gy in 25 fractions without dose escalation.

urgical difficulty is assessed using a predefined 8-item composite score incorporating operative time, conversion to laparotomy, non-routine transanal assistance, visible pelvic fibrosis, bowel edema, intraoperative blood loss, postoperative hospital stay, and postoperative complications.
The total score ranges from 0 to 12; a score ≥3 defines difficult surgery.
