MRT-guidad SIB-strålbehandling i neoadjuvant terapi för avancerad rektalcancer
Effekten av MRT-styrd simultan integrerad boostradioterapi i neoadjuvant terapi för avancerad rektalcancer: en randomiserad, kontrollerad klinisk fas III-studie
Kolorektal cancer har en årlig incidens som överstiger 700 000 fall globalt, och rankas som den fjärde vanligaste cancersjukdomen med den näst högsta dödligheten. Rektalcancer står för ungefär en tredjedel av nydiagnostiserade fall av kolorektal cancer. Stadier II (cT3-4/N0) och III (cT1-4/N1-3) rektalcancer klassificeras vanligtvis som lokalt avancerad rektalcancer (LARC), kännetecknad av en hög risk för lokalt återfall efter operation. Neoadjuvant kemoradioterapi (CRT) i kombination med Total Mesorectal Excision Surgery (TME) har minskat den 5-åriga lokala återfallsfrekvensen i LARC från 25 % till 5 %-10 %. För närvarande kontrollerar neoadjuvant CRT+TME+postoperativ adjuvant kemoterapi effektivt lokalt återfall i LARC. Det är dock associerat med låga patologiska fullständig regression (pCR), suboptimal sfinkterbevarande, ökad fjärrmetastasering, heterogen adherens till adjuvant kemoterapi och begränsade långsiktiga överlevnadsfördelar.
Ytterligare optimering av neoadjuvanta behandlingsstrategier lovar att främja tumörregression och förbättra prognos. Vid neoadjuvant terapi är omfattningen av rektal tumörregression starkt beroende av strålbehandling, med högre stråldoser som korrelerar med ökad patologisk regression. Denna studie syftar till att undersöka rollen av MRT-styrd strålbehandling med en samtidig integrerad boost för att förbättra tumör-pCR i neoadjuvant behandling för lokalt avancerad rektalcancer.

Beskrivning
Inklusionskriterier:
- Histopatologiskt bekräftat rektalt adenokarcinom.
- Tumör lokaliserad ≤10 cm från analkanten.
- Ålder ≥18 år.
- Eastern Cooperative Oncology Group Performance Status (ECOG PS) 0-1.
- Primärbehandlingsnaiv tumör bekräftad av endorektalt ultraljud (ERUS) eller -
- Magnetisk resonanstomografi (MRT) som cT3-4/N+ enligt den 8:e upplagan av AJCC staging.
- Förmåga att tillhandahålla vävnads- och blodprover för translationell forskning.
- Förväntad överlevnad på ≥6 månader.
- Normal funktion av huvudorganen (inom 14 dagar före inskrivning) och lämplighet för att få kemoradioterapi.
Exklusions kriterier:
- Tidigare kemoterapi, strålbehandling eller kirurgisk behandling för ändtarmscancer, inklusive transanal tumörresektion.
- Lokalt återkommande rektalcancer.
- Historia av familjär adenomatös polypos.
- Aktiv Crohns sjukdom eller ulcerös kolit.
- Allergi eller överkänslighetshistoria mot 5-fluorouracil (fluorouracil) och/eller oxaliplatin.
- Historik med svårigheter eller oförmåga att ta eller absorbera orala mediciner.
- Diagnos av annan malignitet än ändtarmscancer under de senaste 5 åren (exklusive fullständigt botad basalcellscancer, skivepitelcancer i huden och/eller in situ-karcinom behandlad med radikal resektion).
- Bekräftad fjärrmetastas, d.v.s. cM1, genom bildbehandling eller biopsi.
- Historia av bäckenstrålbehandling.
- Gravida eller ammande kvinnor.
- Förekomst av någon allvarlig eller okontrollerbar systemisk sjukdom.

Experimentell: Online ART With Sequential Boost
Participants received online adaptive radiotherapy using MR- or CBCT-guided workflows.
A sequential boost of either 9 Gy in 3 fractions or 10 Gy in 2 fractions was delivered to the primary rectal tumor (GTVp) and clinically involved lymph nodes (GTVn), followed by continued online ART to 50 Gy in 25 fractions to the pelvic clinical target volume.
Concurrent capecitabine was administered during radiotherapy, followed by 3-4 cycles of CAPEOX consolidation chemotherapy and planned total mesorectal excision.

Aktiv komparator: Conventional Non-Adaptive IMRT
Participants received conventional non-adaptive intensity-modulated radiotherapy to the pelvic clinical target volume at 50 Gy in 25 fractions without dose escalation.
Concurrent capecitabine was administered during radiotherapy, followed by 3-4 cycles of CAPEOX consolidation chemotherapy and planned total mesorectal excision.

urgical difficulty is assessed using a predefined 8-item composite score incorporating operative time, conversion to laparotomy, non-routine transanal assistance, visible pelvic fibrosis, bowel edema, intraoperative blood loss, postoperative hospital stay, and postoperative complications.
The total score ranges from 0 to 12; a score ≥3 defines difficult surgery.
