Anvendelse av antibiotikafylt kalsiumsulfat som profylakse for pasienter med ikke-modifiserbare risikofaktorer for periprotetiske leddinfeksjoner
For å demonstrere den profylaktiske effekten av kalsiumsulfatkuler fylt med antibiotika hos pasienter med ikke-modifiserbare risikofaktorer som vil gjennomgå en hofte- eller kneleddserstatning, sammenlignet med en kontrollgruppe.
Å vite de økonomiske kostnadene som genereres ved antibiotikaprofylakse med kalsiumsulfatperler hos pasienter som gjennomgår hofte- eller kneleddserstatning med ikke-modifiserbare risikofaktorer.

Inklusjonskriterier:
- pasienter med brudd eller artrose i hofte eller kne som krever behandling med leddprotese.
- pasienter som har noen av de ikke-modifiserbare risikofaktorene før kirurgi eller under transoperativ periode.
- Pasienter med rett til ISSEMyM (Instituto de Seguridad Social del Estado de Mexico y Municipios)
Ekskluderingskriterier:
- Pasienter som mister gyldigheten av institusjonelle rettigheter og ikke følger opp
- Pasienter som dør under studien på grunn av andre årsaker som ikke er relatert til den ortopediske prosedyren.
- Pasienter som ikke har noen av risikofaktorene for periprostetisk infeksjon
- Pasienter som er allergiske mot vankomycin eller ceftriakson.

Deltakere som oppnår en infeksjonsrate for nedre hofte- eller kne-periprotetiske ledd med lokal versus konvensjonell intravenøs antibiotikaprofilakser, målt med CRP, ERS i serum- og sinovialvæskeleukocytter, i løpet av den 12-ukers observasjonsperioden.

Akutt periprostetisk kne- eller hofteinfeksjon ble bestemt ved bruk av CRP- og ESR-serumbiomarkører, som er de mest publiserte serumbiomarkørene i litteraturen om periprotetiske leddinfeksjoner.
Avskjæringspunktet for CRP ble ansett som 93 mg/l og 44 mm/time for ESR.
Serumbiomarkørprøven ble tatt og evaluert på dag 5 og uke 4, 8 og 12. Leukocytter i leddvæske er blant kriteriene for definisjon av periprostetisk leddinfeksjon med et avskjæringspunkt over 12 800 celler/µL og ble kun inkludert hvis serumbiomarkører var forhøyet.

Reduksjonen i antall innleggelsesdager resulterer i lavere risiko for infeksjon og mindre medisinbivirkninger og redusert behov for sykehusmateriell.
Forskjellen i gjennomsnittlig innleggelsesdag mellom begge grupper representerer indirekte den økonomiske kostnaden sykehuset bruker.
