Anwendung von mit Antibiotika beladenem Calciumsulfat als Prophylaxe für Patienten mit nicht veränderbaren Risikofaktoren für periprothetische Gelenkinfektionen
Um die prophylaktische Wirkung von mit Antibiotika beladenen Calciumsulfatkügelchen bei Patienten mit nicht veränderbaren Risikofaktoren zu demonstrieren, die sich einem Hüft- oder Kniegelenkersatz unterziehen, im Vergleich zu einer Kontrollgruppe.
Ermittlung der wirtschaftlichen Kosten, die bei der Antibiotikaprophylaxe mit Calciumsulfatkügelchen bei Patienten entstehen, die sich einem Hüft- oder Kniegelenkersatz mit nicht veränderbaren Risikofaktoren unterziehen.

Beschreibung
Einschlusskriterien:
- Patienten mit Frakturen oder Arthrose der Hüfte oder des Knies, die eine Behandlung durch Gelenkersatz benötigen.
- Patienten, die vor der Operation oder während der transoperativen Phase einen der nicht veränderbaren Risikofaktoren aufweisen.
- Patienten mit Anspruch auf das ISSEMyM (Instituto de Seguridad Social del Estado de Mexico y Municipios)
Ausschlusskriterien:
- Patienten, die ihre institutionellen Rechte verlieren und sich nicht weiterverfolgen
- Patienten, die während der Studie aus anderen Gründen sterben, die nicht mit dem orthopädischen Eingriff zusammenhängen.
- Patienten, bei denen keiner der Risikofaktoren für eine periprothetische Infektion vorliegt
- Patienten, die gegen Vancomycin oder Ceftriaxon allergisch sind.

Teilnehmer, die über den 12-wöchigen Beobachtungszeitraum eine periprothetische Gelenkinfektionsrate der unteren Hüfte oder des Knies mit lokaler versus konventioneller intravenöser Antibiotika-Prophilaxie erreichen, gemessen mit CRP, ERS im Serum und Sinovialflüssigkeits-Leukozyten.
Zeitfenster: Tag 5, Woche 4, 8 und 12

Akute periprothetische Knie- oder Hüftinfektionen wurden mithilfe von CRP- und ESR-Serumbiomarkern bestimmt, die in der Literatur zu periprothetischen Gelenkinfektionen die am häufigsten veröffentlichten Serumbiomarker sind.
Als Grenzwert für CRP wurde 93 mg/l und für BSG 44 mm/h angenommen.
Die Serum-Biomarker-Probe wurde am 5. Tag und in den Wochen 4, 8 und 12 entnommen und ausgewertet. Leukozyten in der Synovialflüssigkeit gehören zu den Kriterien zur Definition einer periprothetischen Gelenkinfektion mit einem Grenzwert über 12.800 Zellen/µL und wurden nur bei Serum-Biomarkern berücksichtigt wurden erhöht.

Die Verweildauer ist ein wichtiger Indikator für Effizienz und wirtschaftliche Belastung des Krankenhauses.
Die Reduzierung der Krankenhausaufenthaltstage führt zu einem geringeren Infektionsrisiko, weniger Nebenwirkungen von Medikamenten und einem geringeren Bedarf an Krankenhausbedarf.
Der Unterschied in den durchschnittlichen Krankenhausaufenthaltstagen zwischen beiden Gruppen spiegelt indirekt die wirtschaftlichen Kosten wider, die das Krankenhaus verursacht.
