前瞻性、随机、双盲、双模拟、主动控制的 3 期临床试验比较鼻内右美托咪定与口服咪达唑仑作为接受磁共振成像的儿科患者异丙酚镇静术前用药的安全性和有效性
一次磁共振成像 (MRI) 检查通常需要 30 到 45 分钟,并且要求患者在整个采集过程中保持完全静止以确保质量。 6 岁以下的儿童不太合作,这个年龄段的儿童需要镇静剂。
目前,没有针对儿科 MRI 检查镇静的具体建议。 2018 年,对 Hôpital Universitaire des Enfants Reine Fabiola (H.U.D.E.R.F.) 应用的镇静方案进行了回顾性研究。 在该方案中,使用口服咪达唑仑进行术前用药,并使用异丙酚反复推注进行镇静。 这项研究的结论是,现有的方案是有效的,但发现在 25% 的病例中,手术过程中的图像采集被中断,这主要是由于儿童的不自主运动。
术前压力可能会对孩子造成情绪创伤,甚至可能会延续到围手术期之后,因此术前用药很重要。 对于最焦虑的儿童,非药物的术前用药手段往往是不够的。 此外,文献表明,药物术前给药有助于减少父母分离焦虑和促进麻醉诱导。

描述
纳入标准:
- 6个月至6岁的男女儿童,
- ASA 评分 I 至 IV,
- 由于临床情况需要标准磁共振成像,无论基础病理如何,
- 麻醉师进行镇静,
- 进入试验前根据 ICH-GCP 和当地立法签署知情同意书。
排除标准:
- MRI 的禁忌症(心脏起搏器、神经刺激器、铁磁植入物),
- 由非麻醉师进行镇静,
- 紧急核磁共振,
- 存在头部外伤,
- 镇静当天存在鼻塞或上呼吸道感染,
- 同一镇静期间的多个程序(手术室、诱发电位等),
- 患有需要气道安全的病症的儿童,
- 任何已知的对右美托咪定的过敏或超敏反应,
- 任何已知的对苯二氮卓类药物的过敏或超敏反应,
- 同时使用负性变时剂,如地高辛,
- 已知患有慢性呼吸衰竭或肌无力的患者,
- 已知患有气道解剖异常、肺部疾病或睡眠呼吸暂停综合征的患者
- 患有已知心律异常或心血管疾病的患者,
- 患有已知肝病或慢性肾病的患者,
- 检查当天出现低血压或心动过缓的患者,
- BMI > 第 97 个百分位数的患者(对应于超重,包括肥胖)。
