Beschreibung
Einschlusskriterien:
- Schriftliche Einverständniserklärung vor der Immatrikulation
- Alter ≥18 Jahre alt
- Histologisch/zytologisch bestätigtes rezidivierendes oder metastasiertes Adenokarzinom des Magens oder des Ösophagus
- Leistungsstatus der Eastern Cooperative Oncology Group (ECOG) 0-2
- Patienten, die Nahrung und Medikamente schlucken können
- Mindestens eine messbare oder auswertbare Läsion gemäß RECIST-Kriterien Version 1.1
Angemessene Knochen-, Leber- und Nierenfunktion gemäß den folgenden Laboranforderungen, die innerhalb von 7 Tagen vor der ersten Verabreichung der Studienmedikamente durchgeführt werden müssen
- Absolute Neutrophilenzahl (ANC) ≥ 1.500/µl, Thrombozyten ≥ 100.000/µl, Hämoglobin (Hb) ≥ 9,0 g/dl,
- Serum-Kreatinin ≤ 1,5 mg/dl (Wenn das Serum-Kreatinin größer als 1,5 mg/dl ist, sollte die Kreatinin-Clearance [Ccr] 60 ml/min oder mehr betragen. Ccr wird nach Cockcroft-Gault-Formel oder 24-Stunden-Urinsammlung berechnet)
- Gesamtbilirubin ≤ 1,5 x Obergrenze des Normalwerts (ULN), AST/ALT-Spiegel ≤ 3,0 x ULN (AST/ALT-Spiegel ≤ 5,0 x ULN für Patienten mit Leberbeteiligung an ihrem Krebs)
- Bei Patienten, die eine adjuvante oder neoadjuvante Chemotherapie erhalten haben, Abschluss der systemischen Chemotherapie 6 Monate vor Studieneinschluss und keine vorherige Verabreichung von Platinderivaten
- Geschätzte Lebenserwartung von mehr als 3 Monaten
Ausschlusskriterien:
- Andere histologische Typen als Adenokarzinom
- Rezidiv innerhalb von 24 Wochen nach Abschluss der adjuvanten Chemotherapie
- R1 Gastrektomie (d. h. mikroskopische Resterkrankung)
- Anamnese einer anderen Malignität innerhalb der letzten fünf Jahre ab dem Tag der schriftlichen Einverständniserklärung, außer geheiltem Basalzellkarzinom der Haut und geheiltem Carcinoma in situ des Gebärmutterhalses
- Strahlentherapie innerhalb von 4 Wochen nach Randomisierung
- Vorgeschichte signifikanter neurologischer oder psychiatrischer Störungen und Vorhandensein oder Vorgeschichte von ZNS-Metastasen
- Größerer chirurgischer Eingriff innerhalb von 4 Wochen vor Studieneintritt oder unzureichende Erholung von einem größeren chirurgischen Eingriff (mit Ausnahme der Patienten, die nur eine offene und eine Verschluss- oder Biopsie erhalten haben)
Andere schwere Krankheiten oder medizinische Bedingungen wie folgt;
- Alle folgenden Zustände traten innerhalb von 6 Monaten vor Studieneintritt auf: Myokardinfarkt, schwere/instabile Angina pectoris, Bypassoperation für Koronararterie/periphere Arterie, dekompensierte Herzinsuffizienz (NYHA-Klasse III oder IV), Hirninfarkt oder transitorische ischämische Attacke
- Überleitungsanomalien wie AV-Block 2. Grades oder höher oder schwere Arrhythmie, die eine medizinische Behandlung erfordert (Rechtsschenkelblock (RBBB) ist förderfähig, Linksschenkelblock (LBBB) jedoch nicht.)
- Unkontrollierter Bluthochdruck
- Leberzirrhose (Child-Pugh-Klasse B oder höher)
- Interstitielle Pneumonie, Lungenfibrose
- Aktive virale Hepatitis B
- Unkontrollierter Diabetes mellitus
- Unkontrollierter Aszites oder Pleuraerguss
- Unkontrollierte aktive Infektion oder Sepsis
Verabreichung von Medikamenten, die potenziell pharmakokinetische Wechselwirkungen mit S-1, Cisplatin und Oxaliplatin haben können
- Flucytosin, ein fluoriertes Pyrimidin-Antimykotikum
- Antivirale Mittel wie Sorivudin und Brivudin oder chemisch ähnliche Arzneimittel
- Warfarin (außer niedrig dosiertes Warfarin zum Zweck der Prophylaxe), Phenprocoumon
- Phenytoin
- Allopurinol
- Teilnahme an anderen klinischen Studien oder Verabreichung anderer Prüfpräparate innerhalb von 30 Tagen vor der Randomisierung
- Schwangere oder stillende Frauen
- Frauen oder Männer im gebärfähigen Alter, die während der Studienbehandlungen oder bis 3 Monate nach Ende der Studienbehandlungen keine angemessene Empfängnisverhütung anwenden
- Nach Ermessen der Prüfärzte für die Studie nicht geeignet
