結腸直腸がん (CRC) のスクリーニング、早期発見、治療の改善により、過去数十年にわたって生存率が上昇しています。 この改善により、残存病変のない長期の CRC 生存者の数が増加しています。 ほとんどのサバイバーは、治療終了後に「新しい正常」を確立することができますが、一部のサバイバーは、かなりの心理的負担により、正常な機能が困難になり、生活の質 (QoL) が低下します。 不安や抑うつなど。診断後 1 ~ 5 年の CRC 生存者の 34% で報告されています。 がんサバイバーの間で最も一般的な懸念の 1 つは、「がんが再発または進行する可能性に関する恐怖、心配、懸念」と定義されるがん再発 (FCR) への恐怖です。 自己申告による FCR の重症度は、がんの種類によって大きく異なるようには見えず、FCR は非常に長期の生存者の間でも持続する可能性があります。 より高い FCR は、(健康) 不安、うつ病、病気のより大きな不確実性、再発リスクの認識、心配に対する否定的な信念など、複数の心理的要因と関連しています。 臨床 FCR の決定的な特徴に関する専門家のコンセンサスは、次の 4 つの特徴が臨床 FCR の重要な特徴であることを示唆しています。 b) 高いレベルの心配; c) 持続的である。 d) 身体症状に対する過度の警戒。
ほとんどの CRC 生存者は、ある程度の FCR を報告しています。 「臨床的に重要な FCR」という用語は、FCR の菌株が臨床的に重要になり、生存者の生命に悪影響を及ぼす場合を表すために導入されました。 Fear of Cancer Recurrence Inventory-Short Form (FCRI-SF) などの検証済みのスクリーニング質問票は、臨床的に重要な FCR の可能性が高い症例を特定するために使用されています。 最近の 2 つの研究では、FCRI-SF に基づいて、CRC 生存者の 13.7% と 10.1% に臨床的に重要な FCR が見られると報告されています (未発表データ、筆頭著者との個人的なやり取り)。 しかし、CRC 生存者における臨床的 FCR の有病率は、推定が小規模な研究 [n=51-91]、または単純化されたまたは検証されていない FCR 測定値を使用した研究に基づいているため、まだいくらか不確実です。
2 つの大規模なコホート研究は、(結腸直腸) がん後の患者の健康関連 QoL に焦点を当てています。 英国の研究には、「がんとともに生き、がんを乗り越えて生きている」人々が含まれており、残存疾患のない生存者、がんとともに生きている人、または再発歴のある人を区別していません。 また、FCRは1項目で評価します。 PROFILES レジストリに基づくオランダの研究では、臨床 FCR の提案されたカットオフ スコアが含まれていない、がんの影響スケール (健康不安サブスケール) 尺度が使用されました。 現在の研究は、CRC生存者における臨床的FCRの有病率のより決定的な推定値を提供します。
この研究では、CRC 生存者の FCR に関連する心理的要因も調査します。 がんの診断は人生を変えるものであり、患者や家族に大きなストレスを与えます。 多くの場合、がん治療後の多数の身体的症状や社会的変化とともに、ストレッサーの合計ががんサバイバーの適応能力を超える可能性があります。 この過負荷は、身体的症状の経験として現れる可能性があり、場合によっては、身体的苦痛症候群や健康不安などの機能障害/身体症状障害に発展する可能性があります. 身体的苦痛症候群は、患者が、通常は複数の身体的症状を、検証可能な慣習的に定義された疾患に起因しない特徴的なパターンで患っている状態として定義されます。
健康不安は、病気の客観的兆候のない重篤で生命を脅かす病気にかかることへの恐怖にとらわれることを特徴とし、医学的な安心にもかかわらず持続します。 健康不安と FCR は、不快な考えや反芻を含み、日常生活に支障をきたし、さらに不必要な調査や治療につながる可能性があるため、多少重複しています。 ある研究では、乳がん生存者の心気症を調査し、臨床レベルの FCR を有する患者の 43% が診断基準を満たしていることがわかりました。 1 つの CRC コホートに関する 2 つの研究では、身体化が測定されています(つまり、 身体症状としての生理的苦痛の発現)、FCRとの関連はありません。 私たちの知る限り、CRC生存者の機能障害、FCR、不安、うつ病の関係を調査した以前の研究はありません。
病気の不確実性は、FCR および健康不安と関連しています。 スクリーニングによって無症候性と診断された場合、病気の不確実性が高まる可能性があります。 したがって、スクリーニングによる診断は、がんおよび FCR に対処する際の問題の増加につながる可能性があります。 包括的なデンマークの臨床がん登録には、診断方法に関するデータが含まれています。つまり、CRC 生存者が、症状の結果として診断されたのではなく、デンマークの全国的な結腸直腸がんスクリーニング プログラムを通じて診断されたかどうかです。 これにより、スクリーニングで検出されたがんの心理社会的影響という未踏の領域での研究が可能になります。
CRC 生存者の約 4 分の 1 (26.5%) と CRC と共に生きる人々の 20 ~ 56% が、FCR に対処するための心理社会的支援が満たされていない重要なニーズであると報告しています。 FCR を減らすための介入をテストするランダム化比較試験は、主に乳がんまたは混合がんの生存者集団で実施されています。 ほとんどの介入は、認知行動療法 (CBT) のバリエーションに基づいています。 現代の CBT は、思考の内容 (例えば 死の考え)はより効果的でした(g = 0.42 vs 0.24)。 以前または現在評価されている介入の提供形式は、グループ、対面、混合、電話、または Web ベースのプラットフォームでした。
「ConquerFear」は、大多数(89%)が乳がんの女性である混合がん患者のリラクゼーショントレーニング注意対照群と比較して、FCRを減らす効果が実証された個別の対面セラピストによる介入です。 ConquerFear の使用は、研究終了後も多くの研究療法士によって維持されてきましたが、高度な訓練を受けた心理学者がいる主要な大都市がんセンターの近くにいる人々が主に利用できるリソースと時間のかかるアプローチです。 その結果、ConquerFear の Web ベースの自己管理バージョン (iConquerFear) が作成されました。カリキュラムの内容は似ていますが、提供方法が異なります。 iConquerFear の使いやすさの質的評価は次のことを示しました。柔軟なアクセスが重要でした。配信モードは魅力的でした。テーラリングは非常に重要でした。追加のリソースへのリンクが評価されました。
ウェブベースの介入は、限られた利用可能な精神保健サービスを増強することにより、質の高いがんケアの重要なギャップを埋める可能性を秘めています。 ただし、完全に自己誘導型のウェブベースの FCR 介入は有効性が限られている可能性があるといういくつかの証拠があり、セラピストのインプットが有効性を高める可能性があることが示唆されています。 ウェブベースのセラピスト主導の認知療法は、患者と提供者の両方に利点があり、がん患者の苦痛を治療する際の従来の対面療法に匹敵する効果があるようです。 ガイド付きの Web ベースの介入は、ガイドなしの介入よりも優れていることを示す証拠があります。
目的 この RCT の主な目的は、セラピストが指導するバージョンの iConquerFear (TG-iConquerFear) が FCR を軽減し、通常の拡張治療 (aTAU) よりも CRC 生存者の QoL を改善できるかどうかをテストすることです。
二次的な目的は、i) 診断後 5 年までの集団ベースの CRC コホートにおける FCR の有病率を、臨床的カットオフを伴う検証済みの FCR 測定値を使用して概説することです。 この包括的なスクリーニングは、TG-iConquerFear の RCT へのリクルートにも使用されます。
ii) 集団ベースのCRCコホートにおける、診断後5年までの不安、うつ病、身体的苦痛症候群および健康不安の有病率の概要。
iii) デンマークの全国的な結腸直腸癌スクリーニング プログラムの結果として診断されると、身体症状に基づいて診断される場合と比較して FCR が増加するかどうか、およびこの関係が病気の不確実性の増加によって媒介されるかどうかを調査します。
iv) FCR が CRC 生存者の不安、うつ病、身体的苦痛症候群、および健康不安と関連しているかどうかを調査するとともに、病気の不確実性、心配に関する否定的な信念、および癌再発のリスクの認識がこれらの関係の調整者または仲介者として機能するかどうかを調査します。

セラピストは、モチベーションを高め、質問に答え、文書や演習についてフィードバックを提供します。

IConquerFear の理論的フレームは、病気の常識モデル、自己調整実行機能モデル、および関係フレーム理論に基づいています。
介入には、注意トレーニング、メタ認知意識の向上、受容とマインドフルネス、適切なスクリーニング行動の促進、価値に基づく目標設定の要素が含まれます。
電子プラットフォームは、教育テキスト、インタラクティブな演習、医師、セラピスト、患者の視点をフィーチャーした短いビデオを含む 5 つのモジュールで構成されています。
