ミズーリ州の 85% 以上が農村部であり、これらの農村地域に住む人々はミズーリ州の都市部に住む人々と比べて高齢であり、定期的な医療へのアクセスが限られています。
障害のある人、特に高齢者は、新型コロナウイルス感染症に感染するリスクが高くなります。
新型コロナウイルス感染症のパンデミック下で適切な高齢化を成功させるためには、地域に住む障害のある高齢者の障害を軽減し、生活の質を向上させることが極めて重要です。
当社はパートナー企業の Foresite Healthcare と協力して、家庭内での健康関連行動を監視するための実証済みのセンサーベースのテクノロジー ソリューションを開発しました。
多施設ランダム化比較試験において、介護連携機能を備えたセンサーシステムが、介護施設で暮らす高齢者の機能低下を防ぐことを実証しました。
この研究の長期的な目標は、障害のある高齢者の自立した生活をできるだけ長くサポートすることです。
このプロジェクトの目的は、農村地域に住む障害のある高齢者の自宅にセンサー システムを導入し、障害の軽減と健康関連の生活の質の向上に対するセンサー システムの効果を評価することです。
2 アームのランダム化比較試験を使用して、センサー システムは 64 人の高齢者の自宅に設置されます。
研究アーム 1 にランダムに割り当てられた参加者は、センサー システムと組み合わせた多分野 (看護、作業療法、ソーシャルワーク) の自己管理介入を受けます。
この介入は 5As の自己管理アプローチに基づいており、私たちの先行研究における看護ケアの調整を直接翻訳したものです。
研究アーム 2 にランダムに割り当てられた参加者は、センサー システムと組み合わせた標準的な健康教育を受けます。
RE-AIM フレームワークを使用して目的 1、2、および 3 から収集された個人および設定レベルのデータを使用して、さまざまなパートナー機関と将来使用するための実装ガイドが作成されます。
このプロジェクトは 3 つの目的で達成されます。
目的 1 では、標準的な健康教育ケアと組み合わせたセンサー システムと比較した、多分野の自己管理介入と組み合わせたセンサー システムの、障害および健康関連の生活の質に対する 1 年後の効果を評価します。
目的 2 では、二次的な健康影響 (うつ病、不安、仕事のパフォーマンス、介護者の負担)、転倒率、および医療の利用に対するセンサー システムの影響を評価します。
目的 3 では、満足度と採用に関する個々の参加者のデータと、組織設定に関する利害関係者のデータを収集します。
目標 1、2、および 3 のデータは RE-AIM を使用して分析され、組織設定に応じた実施ガイダンスが作成されます。

5A の行動変容モード [39] は、自己管理介入のフレームワークです。
5 つの「A」は、自己管理介入とセンサー システムの統合を通じて対処されます。
参加者あたり 12 回の訪問で、各医療専門家 (OT、RN、SW) と少なくとも 4 回の介入セッションが行われます。

自己管理介入は 1 年間にわたって実施されます。

チーム (OT、RN、SW) は最初の 2 か月間で 2 回会合を開き、参加者の SMART 目標と関心領域に基づいて主任介入者を決定します。
主導的介入者は、参加者とさらに 3 つのセッションを行い、センサー システムのアラートとメッセージのポイントパーソンとなります。
目標達成度評価 [83] は、SMART 目標に対する参加者の進捗状況を評価するために、四半期ごとの面接中に実施されます。

すべてのセンサーパラメータは匿名化されたデータとして安全なサーバーに保存されるため、NACDA に預ける前にそれ以上の処理は必要ありません。
