상세 설명
미주리 주의 85% 이상이 농촌 지역이며 이러한 농촌 지역의 개인은 미주리 도시 지역에 거주하는 개인에 비해 나이가 많고 정기적인 의료 서비스에 대한 접근성이 낮습니다.
장애가 있는 사람, 특히 노인은 COVID-19에 걸릴 위험이 더 높습니다.
COVID-19 팬데믹 기간 동안 지역사회에서 거주하는 장애가 있는 노인의 성공적인 거주를 위해 장애를 줄이고 삶의 질을 개선하는 것이 절실히 필요합니다.
우리는 파트너 회사인 Foresite Healthcare와 함께 가정에서 건강 관련 행동을 모니터링하기 위한 입증된 센서 기반 기술 솔루션을 개발했습니다.
다중 사이트 무작위 통제 시험에서 우리는 간호 조정 기능이 있는 센서 시스템이 생활 보조 시설에 거주하는 노인의 기능 저하를 방지한다는 것을 입증했습니다.
이 연구의 장기 목표는 장애가 있는 노인들이 가능한 한 오랫동안 독립적인 생활을 할 수 있도록 지원하는 것입니다.
이 프로젝트의 목적은 센서 시스템을 농촌 지역에 거주하는 장애 노인의 가정에 배치하고 센서 시스템이 장애 감소 및 건강 관련 삶의 질 향상에 미치는 영향을 평가하는 것입니다.
양팔 무작위 통제 시험을 사용하여 센서 시스템을 노인 64명의 가정에 설치합니다.
연구 부문 1에 무작위로 배정된 참가자는 센서 시스템과 짝을 이루는 종합적(간호, 작업 치료 및 사회 사업) 자가 관리 중재를 받게 됩니다.
이 개입은 5A 자기 관리 접근 방식을 기반으로 하며 이전 연구에서 간호 조정을 직접 번역한 것입니다.
Study Arm 2에 무작위 배정된 참가자는 센서 시스템과 함께 표준 건강 교육을 받게 됩니다.
RE-AIM 프레임워크를 사용하여 목표 1, 2 및 3에서 수집한 개인 및 설정 수준 데이터를 사용하여 향후 다른 파트너 기관과 함께 사용할 구현 가이드를 개발할 것입니다.
이 프로젝트는 세 가지 목표로 달성됩니다.
목표 1에서는 1년 후 장애 및 건강 관련 삶의 질에 대한 표준 건강 교육 관리와 결합된 센서 시스템과 비교하여 종합적인 자기 관리 개입과 결합된 센서 시스템의 효과를 평가합니다.
목표 2에서는 센서 시스템이 2차 건강 결과(우울증, 불안, 직업 수행 및 간병인 부담), 낙상률 및 의료 이용에 미치는 영향을 평가합니다.
목표 3에서는 만족도 및 채택을 위한 개별 참가자 데이터와 조직 설정에 대한 이해 관계자 데이터를 수집합니다.
목표 1, 2 및 3의 데이터는 RE-AIM을 사용하여 분석되어 조직 설정에 따라 맥락화된 구현 지침을 생성합니다.
시골 지역에 거주하는 장애가 있는 노인의 경우 센서 시스템은 의료 및 장애 관리에 대한 접근 방식을 변경할 수 있는 잠재력이 있습니다.

- 기대 수명 1년 미만, 중증 인지 장애(최소 정신 상태 검사 점수 <17), 간병 서비스를 제공하는 시설에서의 생활, Katz ADL 점수 6점, 가정에서 물리 치료, 작업 치료 또는 간호를 받고 있음, 입원한 적이 있음 최근 12개월 동안 3회 이상, 내년 내 거주지 변경 예정

5A의 행동 변화 모드[39]는 자기 관리 개입을 위한 프레임워크입니다.
다섯 가지 "A"는 자가 관리 개입과 센서 시스템의 통합을 통해 해결될 것입니다.
참가자당 12회 방문에 대해 각 의료 전문직(OT, RN 및 SW)과 최소 4회의 개입 세션이 있습니다.

팀(OT, RN 및 SW)은 처음 2개월 동안 두 번 만나 참가자의 SMART 목표 및 관심 영역에 따라 중재자를 결정합니다.
선임 중재자는 참가자와 세 번의 추가 세션을 가지며 센서 시스템 경고 및 메시지의 담당자가 됩니다.
목표 달성 척도[83]는 SMART 목표에 대한 참가자 진행 상황을 평가하기 위해 분기별 인터뷰 중에 관리됩니다.
이 측정은 참가자와 함께 공동으로 관리되며 추가 책임을 제공하고 참가자에게 진행 상황에 대한 성찰의 기회를 제공하며 자기 관리 개입의 "성공"에 대한 구체적인 측정입니다.

이 세션의 내용은 참가자(및 적절한 경우 가족 구성원/간병인)가 자신의 건강 데이터를 이해하고 기술 문제를 지원하며 참가자에게 자신의 상태 및 요청된 리소스에 대한 교육을 제공하는 데 중점을 둘 것입니다.
연구원은 또한 외부 제공자 방문 후 조건에 변화가 있거나 새로운 진단이 있는 경우 추가 건강 교육을 제공할 것입니다.
