소마토스타틴 수용체 발현이 높은 종양에서 펩티드 수용체 방사성 핵종 치료(PRRT)
이 연구의 목적에 대한 근거는 다음과 같습니다.
- PRRT가 소마토스타틴 수용체를 발현하는 병변을 나타내고 표준 치료가 아직 이용 가능하지 않은, 수술 또는 수술 불능 질병 진행 상태에 있는 대부분의 신경내분비종양(NET) 환자에게 가치 있는 치료법이 될 수 있다는 증거;
- 시판 허가를 받아 PRRT에 사용되는 소마토스타틴의 방사성 표지 유사체를 시장에서 획득하는 것이 현재 불가능함;
- 이전 연구보다 더 많은 사례 기록을 수집해야 할 필요성;
- 얻은 반응에 기초하여 다양한 조직형을 계층화할 필요성;
- 최대 효능과 가능한 최저 독성을 보장하는 새로운 치료 체계를 정의할 필요성 - 위험과 이익 사이의 최적의 균형을 위해 낮은 누적(및 주기당) 방사성 의약품 활동과 선량 과분할 개념에 따라 - 정의할 필요성.

설명
포함 기준:
- 1. 18세 이상, 남녀, 인종 불문
- 2. NET의 세포조직학적 및 면역조직화학적 진단;
- 3. Ki-67 및/또는 E3 ubiquitin-protein ligase(MIB-1) 연구에 의한 세포 증식 지수 평가.
- 4. 등록과 관련하여 2개월 이내에 기존 영상(조영제를 사용한 CT 또는 조영제를 사용한 MRI)에 의해 RECIST 1.1 기준에 따라 측정 가능한 질병;
- 5. 표적 병변(들)에서 68Ga-DOTATOC를 사용한 PET-CT에 의해 기록된 소마토스타틴 수용체의 상승된 발현. 68Ga-DOTATOC를 사용한 PET-CT 검사에서 반정량적 분석으로 계산된 최대 표준화 섭취 값(SUVmax) 병변/평균 표준화 섭취 값(SUVmean) 근육의 비율이 4:1 이상인 "소마토스타틴 수용체의 높은 발현"으로 정의됩니다. ;
- 6. 등록 2개월 이전이 아닌 크로모그라닌 A(및 기타 특정 마커)의 용량;
- 7. 18F-FDG를 사용한 PET-CT에 의한 표적 병변(들)에서의 글루코스 대사 평가;
- 8. 보존된 혈액학적, 간 및 신장 매개변수, 특히: 백혈구 ≥2500/μL; 혈소판 ≥ 90000/μL; 헤모글로빈 ≥ 9gr/dL; 크레아티닌 ≤ 2mg/dL; 빌리루빈 ≤ 2.5mg/dL
- 9. ECOG(Eastern Cooperative Oncology Group) 수행 상태 ≤2;
- 10. 기대 수명 ≥ 6개월;
- 11. 수술이 불가능한 수술 환자 모두에서 어느 단계에서나 안정적이거나 진행성인 질병;
- 12. 이미 문서화되고 동일한 효과가 있는 표준 치료의 부재
- 13. 적어도 30일 동안 수술, 화학 요법 및/또는 방사선 요법 치료의 부재. 한편, 라파마이신의 기계론적 표적(m-TOR) 억제제와 같은 소마토스타틴 유사체 또는 생물학적 제제로 치료 중인 환자;
- 14. 정보 메모를 읽고 완전히 이해한 후 정보 동의서에 서명하여 연구에 자발적으로 참여합니다.
제외 기준:
- 1. 위에 나열된 요구 사항이 부족합니다.
- 2. 임신 상태
- 3. 모유 수유 및 모유 수유 중단에 대한 친척 거부;
- 4. PRRT 전 4주 동안 다른 치료 실험 임상 프로토콜에 참여;
- 5. 질환의 골수 침습 > 25% 확인됨;
- 6. 이전의 광범위한 방사선 요법 치료.

모든 G3/G4 독성으로 정의되는 치료 관련 부작용 평가.
평가는 모든 치료 주기 동안 그리고 마지막 치료 주기 후 12, 18, 24, 30, 36 및 42개월 후에 수행되며 CTC-AE(Common Terminology Criteria for Adverse Events) 독성 기준 버전 4.0을 기반으로 합니다.
