Snel steraal sluitsysteem (TALON) (TALON)
Een gerandomiseerde, gecontroleerde fase 4-studie van sternale plating (Rapid Sternal Closure System, KLS Martin L.P.) bij hartchirurgische patiënten met een hoger risico op sternale wondcomplicaties.
Het primaire doel is om vast te stellen of het Rapid Sternal Closure System (RSCS) het vroege postoperatieve herstel verbetert, wat zich uit in verminderde pijn en verbeterde longfunctie.
Secundaire doelstellingen omvatten evaluatie van het Rapid Sternal Closure System met betrekking tot SWC's (chirurgische wondcomplicatie gedefinieerd als chirurgisch behandelde sternale wondinfectie en sternale instabiliteit/non-union) zoals beschreven in het protocol.
Voor een bepaald onderzoekseindpunt zal de nulhypothese geen verschil zijn tussen de Rapid Sternal Closure System-groep en de controlegroep. De alternatieve hypothese is een verschil tussen 2 groepen. Het statistische doel van deze studie is om de nulhypothese te verwerpen ten gunste van de alternatieve hypothese.

Beschrijving
Inclusiecriteria:
- Man of vrouw. Vrouwen die zwanger kunnen worden, moeten voorafgaand aan de operatie een negatief humaan choriongonadotrofine-assay in serum (of urine) hebben en bereid zijn om gedurende ten minste 180 dagen na de operatie effectieve anticonceptiemiddelen te blijven gebruiken. Medisch aanvaardbare anticonceptiva omvatten: (1) chirurgische sterilisatie (zoals afbinden van de eileiders of hysterectomie), (2) goedgekeurde hormonale anticonceptiva (zoals anticonceptiepillen, pleisters, implantaten of injecties), (3) barrièremethoden (zoals een condoom). of diafragma) gebruikt met een zaaddodend middel, of (4) een spiraaltje (IUD). Anticonceptiemaatregelen zoals Plan B(TM), verkocht voor gebruik in noodgevallen na onbeschermde seks, zijn geen aanvaardbare methodes voor routinematig gebruik.
- Leeftijd ≥ 18 jaar.
- Gepland om een niet-spoedeisende coronaire bypass-transplantaat (CABG) en/of klepreparatie- of -vervangingsoperatie te ondergaan via een volledige mediane sternotomie.
- Een hoger risico op SWC, gedefinieerd als de aanwezigheid van een van de volgende factoren: zwaarlijvigheid (body mass index> 30), chronisch gebruik van steroïden (> 6 maanden en momenteel gebruikt), ernstige chronische obstructieve longziekte, geplande bilaterale interne borstslagader oogst, ondergaat opnieuw mediane sternotomie en voorgeschiedenis van bestraling op de borst.
- Bereid en in staat om schriftelijke geïnformeerde toestemming te geven.
- Beschikbaar voor evaluatie vanaf baseline tot eindevaluatie 180 dagen na de operatie.
Uitsluitingscriteria:
- Noodhartchirurgie ondergaan (spoedoperatie is toegestaan als geïnformeerde toestemming is verkregen en de onderzoeksprocedures kunnen worden uitgevoerd).
- Een significante gelijktijdige chirurgische ingreep ondergaan (bijv. halsslagader-endarteriëctomie, aortawortelreparatie of -vervanging, diepe hypothermische circulatiestilstand [DHCA] of pulmonale resectie).
- Een minimaal invasieve of een thoracale chirurgische benadering ondergaan.
- Gebruik van een preoperatief mechanisch hulpmiddel of intra-aortale ballonpomp (IABP), indien ingebracht voor shock/low output-syndroom (een IABP is toegestaan als deze wordt ingebracht voor onstabiele angina of lage ejectiefractie).
- Actieve en significante systemische infectie, bijv. Actieve endocarditis of een voorgeschiedenis van significante terugkerende systemische infectie.
- Antibiotische therapie ontvangen binnen de 2 weken vóór de operatiedatum.
- Geschiedenis van maligniteit in het afgelopen jaar (behalve plaveiselcel- of basaalcelcarcinoom van de huid dat is behandeld, zonder bewijs van recidief).
- Geschiedenis van belangrijke orgaantransplantatie, inclusief beenmergtransplantatie.
- Recente geschiedenis van aanzienlijk drugs- of alcoholmisbruik.
- Huidige immunosuppressieve aandoening (bijv. Symptomatische infectie met het humaan immunodeficiëntievirus [HIV]). Gebruik van steroïden is geen uitsluitingscriterium).
- Vrouwelijke proefpersoon die zwanger is (inclusief een positieve zwangerschapstest bij screening of baseline) of borstvoeding geeft. Vrouwtjes in de vruchtbare leeftijd die geen anticonceptie toepassen met een hoge mate van betrouwbaarheid.
- Postoperatieve levensverwachting ≤ 90 dagen, volgens de mening van de onderzoeker of sponsor.
- Huidige deelname of deelname binnen 30 dagen voor de start van deze studie aan een experimenteel geneesmiddel- of hulpmiddelonderzoek of momenteel deelname aan een onderzoek waarin de toediening van experimentele geneesmiddelen binnen 90 dagen wordt verwacht.
- Weigering medisch geïndiceerde bloedproducten te accepteren.
- Matige of ernstige pectusafwijking.

Het maximale stimulerende spirometrievolume werd gemeten bij baseline (vóór de operatie) en dagelijks van postoperatieve dag 1 tot en met postoperatieve dag 7 (of ontslag indien eerder) met behulp van een Coach 2 stimulerende spirometer met eenrichtingsklep (Coach 2 model # 22-4000, Smiths Medical , Keene, NH).
