Inductiechemotherapie gecombineerd met radiochemotherapie:
Start therapie: binnen 28 dagen na bioptische diagnose capecitabine 1000 mg/m2 tweemaal daags gedurende 14 dagen (d1-d14), oxaliplatine 130 mg/m2 en bevacizumab 7,5 mg/kg lichaamsgewicht d1; herhaling dag 22 en 43 (3 cycli)
Gecombineerde radiochemotherapie na 1 week afgesloten 3e cyclus inductiechemotherapie:
Radiotherapie: 5 x 5 dagen 1,8 Gy; cumulatieve dosis 45 Gy Chemotherapie: capecitabine 825 mg/m² tweemaal daags, op elke bestralingsdag gedurende de eerste 4 weken van RCTx
Chirurgie volgens TME-criteria (totale mesorectale excisie) met inachtneming van een onderbreking van min. 14 dagen na RCTx

Beschrijving
Inclusiecriteria:
- Leeftijd 18-80 jaar
- Histologische bevestiging van rectaal adenocarcinoom stadium cT3 (≤ 5 mm tot de mesorectale fascia)/cT4 (primaire curatieve intentie)NxM0
- Geen eerdere chemotherapie, geen eerdere radiotherapie van het bekken, geen eerdere tumorresectie van een rectumcarcinoom
- Algemene toestand WHO-graad 0-2
- Voldoende beenmergreserve (leukocyten ≥3 000/μl, trombocyten ≥100 000/μl)
- Adequate nierfunctie (creatinine ≤ 1,5 mg/dl, creatinineklaring > 50 ml/min (formule Cockcroft en Gault))
- Adequate leverfunctie (bilirubine ≤1,5x ULN, GOT en GPT ≤3,5xULN)
- Uitsluiting van zwangerschap voor vrouwen die zwanger kunnen worden (negatieve zwangerschapstest urine of serum)
- Vrouwelijke patiënten die zwanger kunnen worden en mannelijke patiënten die niet chirurgisch steriel zijn, moeten een medisch aanvaardbaar anticonceptieregime volgen tijdens de studiebehandeling tot 3 maanden na het einde van de studie (bijv. orale anticonceptiva, condoom, spiraaltje)
- Levensverwachting van minimaal 3 maanden
- INR en aPTT ≤ 1,5 x LLN
- Verstrekking van ondertekende geïnformeerde toestemmingen vóór registratie
Uitsluitingscriteria:
- Rectumcarcinoom stadium cT3 (> 5 mm van de mesorectale fascia) alle stadia <cT3, M1
- Andere kwaadaardige tumoren in de afgelopen 5 jaar behalve cervicaal carcinoom in situ en basaalcelcarcinoom van de huid
- Algemene contra-indicatie of bekende overgevoeligheid voor Bevacizumab, Capecitabine en Oxaliplatin
- Niet-maligne ziekten waarvoor behandeling met radiotherapie, resectie van het rectum en behandeling met chemotherapie (Bevacizumab, Capecitabine) gecontra-indiceerd is: ongecontroleerde hypertensie (systolisch > 150 mmHG en/of diastolisch > 100 mmHG) of klinisch significante (bijv. actief) hart- en vaatziekten: CVA (cardiovasculair accident)/ apoplectisch insult (≤ 6 maanden voor registratie), myocardinfarct (≤ 6 maanden voor registratie), instabiele angina pectoris, CHF (congestief hartfalen) met NYHA (New York Heart Association ) Graad II of hoger, hartritmestoornissen die therapie vereisen, leveraandoeningen, significante neurologische of psychiatrische stoornissen
- Bloemrijke, ernstige infectie bij registratie
- Perifere neuropathie (NCI CTCAE v 4.0 ≥ graad 1)
- Juridisch beperkte contractuele capaciteit, indicatie van neurologische of psychiatrische ziekte die de onderzoekers beperkt in hun mening over het vermogen van de patiënt om zich aan de onderzoeksroutines te houden
- Grote chirurgische ingreep binnen 28 dagen voor aanvang van de studie, open wonden
- Aanzienlijk traumatisch letsel, botbreuk, niet-genezende wonden
- Patiënten met compressie van het ruggenmerg of metastasen in het centrale zenuwstelsel
- Indicatie van bloedingsdiathese of coagulopathie
- Inname van antistollingsmiddelen of trombolytica en/of aspirine > 325 mg/d binnen 10 dagen voorafgaand aan registratie
- Huidige of recente (binnen 10 dagen voor aanvang van de behandeling) therapie met volledig gedoseerde anticoagulantia. Preventieve therapie is toegestaan.
- Eerdere trombo-embolische of hemorragische voorvallen binnen 6 maanden voorafgaand aan registratie
- Eerdere abdominale fistels, gastro-intestinale perforatie of intra-abdominale abcessen binnen 6 maanden voorafgaand aan registratie
- Behandeling met een ander onderzoeksgeneesmiddel binnen 28 dagen voorafgaand aan de registratie
- Patiënten met malabsorptiesyndroom of slikproblemen
- Indicatie van slechte therapietrouw van de patiënt
- Zwangere vrouwen of vrouwen die borstvoeding geven
- Proteïnurie: Peilstok <2+. Als de peilstok ≥2+ is, moet eiwit worden geschat in de 24 uur urine. De waarde mag niet hoger zijn dan 1g/24 uur.

Experimenteel: inductiechemotherapie + radiochemotherapie
preoperatieve inductiechemotherapie in combinatie met bevacizumab gevolgd door gecombineerde radiochemotherapie met capecitabine inductiechemotherapie: start binnen 28 dagen na bioptische diagnose.
Alle patiënten krijgen capecitabine (Xeloda®) 1000 mg/m2 bid gedurende 14 dagen (d1-d14), oxaliplatine 130 mg/m2 en bevacizumab (Avastin®) 7,5 mg/kg lichaamsgewicht op dag 1; herhalingsdag 22 en 43 (3 cycli) Gecombineerde radiochemotherapie: start op zijn vroegst één week na het beëindigen van de derde cyclus van inductiechemotherapie.
Radiotherapie vindt plaats op 5 x 5 dagen (dosis: 1,8 Gy; cumulatieve dosis: 45 Gy).
Voor chemotherapie krijgen patiënten capecitabine (Xeloda®) 825 mg/m² bid, op elke bestralingsdag gedurende de eerste 4 weken van radiochemotherapie.
