Studie van de modificaties van de immuunmicro-omgeving geïnduceerd door neoadjuvante chemotherapie bij triple-negatieve borstkanker
Het voorschrijven van neoadjuvante chemotherapie wordt een standaard bij vrouwen met HER2-positieve of triple-negatieve borstkanker en maakt een volledige histologische respons (pCR) mogelijk, wat een prognostische factor voor overleving vertegenwoordigt. . Het probleem voor patiënten die geen pCR zijn, is dat ze momenteel een niet-gepersonaliseerde adjuvante systemische behandeling krijgen.
De identificatie van biomarkers die aanwezig zijn in de resterende ziekte zou een criterium zijn om de keuze van post-neoadjuvante adjuvante systemische behandeling te begeleiden, om deze te personaliseren.
Op dit moment is er geen gepubliceerde studie die een uitgebreide beschrijving geeft van de immuun-micro-omgeving (ME) bij borstkanker, zowel voor als na chemotherapie, noch de modificatie ervan veroorzaakt door chemotherapie.
Het team stelt daarom voor om in een retrospectieve en monocentrische reeks de modificaties van de immuun-ME te bestuderen die worden geïnduceerd door een "standaard" neo-adjuvante chemotherapie bij patiënten met triple-negatieve CS, of ze nu een volledige histologische respons hebben of niet (n = tweemaal 50).
Het hoofddoel van dit project is het beschrijven van de veranderingen in de immuun-ME van triple-negatieve borstkankers geïnduceerd door neoadjuvante chemotherapie voor alle patiënten (in pCR of niet):
- Kwantificering van TIL's en subtypes van TIL's (CD4 en CD8)
- Expressie van de drie immuuncontrolepunten die PDL1, TIM3 en LAG3 zijn
- Beschrijf de organisatie van het immuunsysteem (immunokleuring op hetzelfde objectglaasje van de PDL1-, TIM3- en LAG3-immuuncontrolepunten)

Retrospectieve en monocentrische translationele studie uitgevoerd bij patiënten die in het IUCT-Oncopole werden behandeld met neoadjuvante chemotherapie (sequentiële behandeling FEC100 of EC100, daarna taxaan, grotendeels wekelijks paclitaxel) voor een triple-negatieve CS tussen 2012 en 2018.
We laten de microbiopsie van de primaire tumor bewaren in FFPE en de operatiekamer bewaren in FFPE.
We hebben alle klinische gegevens voor de diagnose en monitoring van deze patiënten al in een database ingevoerd.
De zoektocht naar een BRCA-kiemlijnmutatie is bij de meeste patiënten beschikbaar indien geïndiceerd voor een oncogenetische consultatie.

- TILS-account volgens Salgado et al. voor en na neoadjuvante chemotherapie
- IHC CK5-6 en EGFR dan RA als de eerste twee negatief zijn om drievoudige negatieve tumoren te karakteriseren in "basaalachtig" en "niet-basaalachtig"
- IHC CD3 (labelen van T-lymfocyten)
- IHC CD4, CD8 en FOXP3 voor en na neoadjuvante chemotherapie
- PDL1, TIM3 en LAG3 multiplex IHC voor en na neoadjuvante chemotherapie
