Geavanceerde cardiale beeldvorming om embolische beroerte op hersen-MRI te voorspellen: een pilotstudie

De onderzoekers stellen een cross-sectionele cohortstudie voor, waarbij personen zonder voorgeschiedenis van AF en met congestief hartfalen, hypertensie, leeftijd ≥ 75 jaar, diabetes mellitus, beroerte of voorbijgaande ischemische aanval (TIA), vaatziekte, leeftijd 65 tot 74 jaar, geslachtscategoriescore (CHA2DS2VASC) ≥3, diabetes type II, congestief hartfalen of een voorgeschiedenis van beroerte/TIA worden opgenomen. Gegevens over demografie, persoonlijke gezondheidsgewoonten, medicijnen en medische geschiedenis zullen worden verkregen door deelnemers te interviewen en de elektronische medische dossiers te bekijken. Alle deelnemers ondergaan een Cardiac Magnetic Resonance Imaging (CMR) om te beoordelen op markers van LA- en LAA-pathologie. Markers van LA- en LAA-pathologie die zullen worden bestudeerd, zijn onder meer: LA-fibroseniveau, LA-functionele parameters, LA-vormkenmerken en LAA-kenmerken (inclusief morfologie, openinggebied en stroomsnelheid). Bovendien ondergaan alle deelnemers tijdens hetzelfde bezoek een MRI (Magnetic Resonance Imaging) van de hersenen om de aanwezigheid van embolisch lijkende herseninfarcten te beoordelen, ongeacht eerdere symptomen die verband houden met een beroerte.
De onderzoekers zullen de associatie analyseren tussen elk cardiaal beeldvormend kenmerk en de prevalentie van embolieachtige beroertes op hersen-MRI om te bepalen of patiënten met hogere LA- en LAA-geremodelleerde kenmerken meer kans hebben op embolieachtige herseninfarcten op MRI. De LA- en LAA-pathologische beeldvormingsfuncties met de sterkste statistische associatie zullen worden gebruikt om een voorspellende beeldvormingsscore te ontwikkelen waarmee patiënten met het hoogste risico op een embolische beroerte kunnen worden geïdentificeerd.
Alle beeldvormingsgegevens van de hersenen en het hart zullen worden beoordeeld door ervaren operators in de faciliteiten van het Tulane Medical Center. Operators die CMR analyseren, zijn blind voor MRI-resultaten van de hersenen, en operators die MRI van de hersenen beoordelen, zijn blind voor CMR-resultaten. De studie omvat een studie in één centrum in Tulane Medical Center and Clinics, met onderzoekers van verschillende medische specialismen, en de juiste faciliteiten en apparatuur om het project nauwkeurig en veilig uit te voeren. De onderzoekers verwachten een rekrutering van 120 proefpersonen over een periode van 18 maanden van zowel cardiologie- als neurologieklinieken om het onderzoek te voltooien.

Inclusiecriteria:
- Mannelijke of vrouwelijke patiënten
- 18 jaar en ouder
- Geen voorgeschiedenis van boezemfibrilleren
- CHA2DS2VASC-score ≥3
- Geschiedenis van diabetes type II
- Voorgeschiedenis van congestief hartfalen of een voorgeschiedenis van voorbijgaande ischemische aanval (TIA)/beroerte zonder een anderszins gedefinieerde beroerte-etiologie zoals grote vaten of kleine vatenziekte
Uitsluitingscriteria:
- Geschiedenis van atriumfibrilleren
- Patiënten die in de afgelopen 30 dagen een klinisch symptomatische acute beroerte hebben gehad
- Elke gezondheidsgerelateerde contra-indicatie voor gadolinium/MRI (inclusief eerdere allergische reacties op gadolinium, pacemakers, defibrillatoren, andere apparaten/implantaten die gecontra-indiceerd zijn voor MRI)
- Geschatte cut-off van de glomerulaire filtratiesnelheid (eGFR) bij patiënten met chronische nierziekte (CKD) waarbij gadolinium niet kan worden gebruikt, is gelijk aan een eGFR <30 ml/min
- Met een gewicht van meer dan 300 lbs (aangezien de CMR-beeldkwaliteit afneemt als gevolg van een hogere body mass index)
- Huidige zwangerschap of borstvoeding
- Cognitieve stoornissen waardoor de patiënt geen geïnformeerde toestemming kan geven

De aanwezigheid van verborgen embolische herseninfarcten

De verborgen embolische cerebrale infarcten worden gedefinieerd als de aanwezigheid van een asymptomatisch niet-lacunair embolisch infarct (acuut of chronisch op basis van MRI-uiterlijk), in de cortex, en juxtacorticale gebieden, en het cerebellum.
Ze zullen worden geïdentificeerd met behulp van hersen-MRI.
