Eine Studie zu Lebrikizumab bei jugendlichen Teilnehmern mit unkontrolliertem Asthma, die inhalative Kortikosteroide und ein zweites Kontrollmedikament erhalten

A Phase III, Randomized, Double-Blind, Placebo-Controlled Study to Assess the Efficacy, Safety, and Tolerability of Lebrikizumab in Adolescent Patients With Uncontrolled Asthma Who Are On Inhaled Corticosteroids and a Second Controller Medication.
In dieser randomisierten, multizentrischen, doppelblinden, placebokontrollierten Parallelgruppenstudie wird die Wirksamkeit, Sicherheit und Verträglichkeit von Lebrikizumab bei jugendlichen Teilnehmern mit Asthma bewertet, deren Krankheit trotz täglicher Behandlung mit inhalativen Kortikosteroiden (ICS) nicht unter Kontrolle bleibt ein zweites Kontrollmedikament.
Die Teilnehmer werden im Verhältnis 1:1:1 randomisiert und erhalten 52 Wochen lang eine doppelblinde Behandlung entweder mit Lebrikizumab („Hoch“ oder „Niedrig“) oder Placebo, das alle 4 Wochen (Q4W) subkutan (SC) verabreicht wird Ergänzung zur Standardtherapie.
Darauf folgt eine optionale 52-wöchige doppelblinde aktive Behandlungsverlängerung.
Die voraussichtliche Dauer der Studienbehandlung beträgt bis zu 104 Wochen.
Teilnehmer, die die Studie bis Woche 104 abschließen, vorzeitig abbrechen oder sich entscheiden, nicht an der optionalen Verlängerung der aktiven Behandlung teilzunehmen, werden in die 20-wöchige Sicherheitsnachbeobachtungsphase übergehen.

Einschlusskriterien:
- Asthmadiagnose für mindestens (>/=) 12 Monate vor Besuch 1
- Reaktion des Bronchodilatators während des Screenings
- Präbronchodilatator FEV1 von 40 Prozent (%) – 90 % bei beiden Besuchen 2 und 3 vorhergesagt
- Unter hochdosierter ICS-Therapie für >/= 6 Monate vor Besuch 1
- 6 Monate lang vor Besuch 1 ein geeignetes zweites Kontrollmedikament einnehmen
- Unkontrolliertes Asthma gemäß der Definition im Protokoll sowohl während des Screenings als auch zum Zeitpunkt der Randomisierung
- Nachgewiesene Einhaltung der Kontrollmedikation während des Screening-Zeitraums
Ausschlusskriterien:
- Schwere allergische Reaktion oder anaphylaktische Reaktion auf ein biologisches Mittel in der Vorgeschichte oder bekannte Überempfindlichkeit gegen einen Bestandteil der Lebrikizumab-Injektion
- Erhaltungstherapie mit oralen Kortikosteroiden innerhalb von 3 Monaten vor Besuch 1
- Behandlung mit systemischen (oralen, intravenösen [IV] oder intramuskulären [IM]) Kortikosteroiden innerhalb von 4 Wochen vor Besuch 1 oder während des Screening-Zeitraums
- Behandlung mit intraartikulären Kortikosteroiden innerhalb von 4 Wochen vor Besuch 1 oder während des Screening-Zeitraums oder voraussichtlicher Bedarf an intraartikulären Kortikosteroiden im Verlauf der Studie
- Infektion, die die folgenden Kriterien erfüllt: Jede Infektion, die eine Krankenhauseinweisung oder eine Behandlung mit intravenösen oder intramuskulären Antibiotika innerhalb von 4 Wochen vor Besuch 1 oder während des Screenings erfordert; jede aktive Infektion, die eine Behandlung mit oralen Antibiotika innerhalb von 2 Wochen vor Besuch 1 oder während des Screenings erforderte; Infektion der oberen oder unteren Atemwege innerhalb von 4 Wochen vor Besuch 1 oder während des Screenings; aktive parasitäre Infektion oder Listeria monocytogenes-Infektion innerhalb von 6 Monaten vor Besuch 1 oder während des Screenings
- Vorgeschichte einer aktiven Tuberkulose, die einer Behandlung bedarf
- Bekannte Immunschwäche, einschließlich, aber nicht beschränkt auf eine Infektion mit dem humanen Immundefizienzvirus (HIV).
- Hinweise auf eine akute oder chronische Hepatitis oder eine bekannte Leberzirrhose

The Standardized AQLQ+12 was used to assess the participants' asthma-specific health-related quality of life.
The AQLQ+12 had a recall specification of 2 weeks.
The AQLQ+12 was a 32-item questionnaire with 4 domains: activity limitations, symptoms, emotional function, and environmental stimuli.
Each of the 32 questions were scored on a scale 1-7.
The overall AQLQ+12 score is the mean of the responses to each of the 32 questions, and ranges from 1 to 7. A score 7 indicated no impairments due to asthma, and a score of 1 indicated severe impairment.

Baseline asthma rescue medication use was defined as the average number of puffs per day over the 7 days prior to and on the day of randomization.
Participants must have recorded their asthma rescue medication use for at least 4 days to have a baseline score calculated.
The post-baseline asthma medication use for any timepoint was defined as the average number of puffs per day over the last 28 days on or prior to the timepoint.
Participants must have recorded their asthma rescue medication use for at least 14 days during a 28-day interval to have a score calculated for the respective timepoint.
For nebulizer use, one treatment (inhalation) was considered equivalent to 4 puffs.

Participants who received at least one dose of lebrikizumab and had at least one post-baseline evaluable sample were included in the analysis.
Results of post-dose samples which were less than reportable were set to 0.045 micrograms per milliliter (mcg/mL) that is half of minimum quantifiable concentration value (0.09 mcg/mL).
