Il s'agissait d'un essai clinique multicentrique de phase II, à un seul groupe et à deux cohortes, évaluant l'efficacité et l'innocuité du vismodegib (GDC-0449) chez des patients atteints d'un carcinome basocellulaire avancé.
Tous les patients ont reçu du vismodegib jusqu'à l'apparition de signes de progression, de toxicités intolérables très probablement attribuables au vismodegib ou jusqu'à l'arrêt de l'étude.

Critère d'intégration:
- Hommes et femmes ≥ 18 ans.
- Pour les patients atteints d'un carcinome basocellulaire (CBC) métastatique, confirmation histologique d'une métastase distante du CBC (p. ex., poumon, foie, ganglions lymphatiques ou os), avec une maladie métastatique qui est Critères d'évaluation de la réponse dans les tumeurs solides (RECIST) mesurable à l'aide de la tomodensitométrie ( CT) ou imagerie par résonance magnétique (IRM).
- Pour les patients atteints d'un CBC localement avancé, maladie confirmée histologiquement considérée comme inopérable.
- Pour les patients atteints d'un CBC localement avancé, la radiothérapie doit avoir été administrée au préalable pour leur CBC localement avancé, sauf si la radiothérapie est contre-indiquée ou inappropriée. Pour les patients dont le CBC localement avancé a été irradié, la maladie doit avoir progressé après l'irradiation.
- Pour les femmes en âge de procréer, accord sur l'utilisation de deux méthodes de contraception acceptables, dont une méthode barrière, pendant l'étude et pendant 12 mois après l'arrêt du vismodegib (GDC-0449).
- Pour les hommes ayant des partenaires féminines en âge de procréer, accord pour l'utilisation d'un préservatif en latex, et de conseiller à leur partenaire féminine d'utiliser une méthode de contraception supplémentaire pendant l'étude et pendant 3 mois après l'arrêt du vismodegib.
Critère d'exclusion:
- Traitement antérieur par le vismodegib ou d'autres inhibiteurs de la voie Hedgehog.
- Grossesse ou allaitement.
- Espérance de vie < 12 semaines.
- Patients atteints d'un CBC multifocal superficiel qui peuvent être considérés comme non résécables en raison de l'ampleur de l'atteinte.
- Thérapie antitumorale concomitante non spécifiée dans le protocole (p. ex., chimiothérapie, autre thérapie ciblée, radiothérapie ou thérapie photodynamique).
- Participation récente, actuelle ou prévue à une étude expérimentale sur un médicament.
- Antécédents d'autres tumeurs malignes dans les 3 ans suivant le premier jour de traitement par le vismodegib dans cette étude (jour 1), à l'exception des tumeurs présentant un risque négligeable de métastases ou de décès, telles que le carcinome épidermoïde de la peau correctement traité, le carcinome canalaire chez situ du sein ou carcinome in situ du col de l'utérus.
- Maladies médicales non contrôlées telles qu'une infection nécessitant un traitement avec des antibiotiques intraveineux.

Expérimental: Vismodégib 150 mg
Les patients ont reçu 150 mg de vismodegib par voie orale une fois par jour jusqu'à progression de la maladie ; toxicité intolérable, très probablement attribuable au vismodegib ; ou le retrait de l'étude.

The SF-36 Health Survey (Version 2) uses patient-reported symptoms on 8 subscales to assess health-related quality of life (HRQoL).
The Physical Component Summary (PCS) score summarizes the subscales Physical Functioning, Role-Physical, Bodily Pain, and General Health.
The Mental Component Summary (MCS) score summarizes the subscales Vitality, Social Functioning, Role-Emotional, and Mental Health.
Each score was scaled from 0 to 100.
A positive change score indicates better HRQoL.

Percentage of Participants With Absence of Residual Basal Cell Carcinoma (BCC) in the Efficacy-Evaluable Locally Advanced BCC Cohort

In patients with locally advanced BCC, the histopathological effect of vismodegib was determined in tissue biopsies obtained at baseline (prior to study drug dosing) and at 24 weeks after the start of vismodegib treatment, if the patient was still on study without evidence of progression, or at the investigator's assessment of best clinical response (or best clinical/RECIST response), if occurring prior to 24 weeks.
At any time during a patient's participation in the study, an optional tumor biopsy might have been requested to clarify the response status of the patient.
Reported is the percentage of efficacy-evaluable patients with locally advanced BCC pathology that was confirmed in a baseline biopsy who had an absence of residual BCC post-baseline as assessed by an independent pathological review.
