Critère d'intégration:
- Hémoculture positive avec coloration de Gram montrant GNB
- Hospitalisé au moment du résultat de la coloration de Gram
Critère d'exclusion:
- Hémoculture positive pour le GNB dans le même établissement au cours des 7 jours précédents (si connue au moment du résultat de la coloration de Gram)
- Décédé au moment du résultat de la coloration de Gram
- Bacilles à Gram positif, coques à Gram positif, coques à Gram négatif, levures, champignons ou morphologies multiples de GNB détectées par coloration de Gram d'hémoculture

Aucune intervention: Norme de soins
Lorsqu'une hémoculture en croissance GNB est randomisée dans le groupe de soins standard, les hémocultures positives seront caractérisées à l'aide de tests de sensibilité aux antimicrobiens (AST) standard de soins.

Lorsqu'une hémoculture en croissance GNB est randomisée dans le bras Reveal, l'hémoculture positive sera caractérisée à l'aide de Reveal, d'un AST rapide et de l'AST standard de soins.
|
Reveal est une méthode AST rapide, qui utilise la technologie de capteurs à petites molécules pour détecter la croissance des populations bactériennes en mesurant les métabolites volatils, et fournit des résultats AST en environ 5 heures.
Reveal™ est approuvé pour une utilisation clinique dans l'Union européenne (UE) et en Israël et l'approbation est en cours en Inde, et fournit des concentrations minimales inhibitrices (CMI) pour 28 antibiotiques et 9 espèces à Gram négatif, qui représentent ensemble environ 90 % des organismes. provoquant des infections du sang à Gram négatif (BSI).

The composite 3-category DOOR outcome will assess three deleterious events (unsuccessful discharge, lack of clinical response, and undesirable events) in addition to survival up to 30 days after Gram stain result. The primary DOOR outcome measure is defined using three ordered levels. From best to worst, they are:

New acquisition is defined as detection of MDRO/C. difficile in subjects who do not have preceding clinical or surveillance cultures with these organisms in the prior 3 months. MDRO will be identified on routine clinical or surveillance samples using local laboratory diagnostic procedures and include: Methicillin-resistant Staphylococcus aureus Vancomycin-resistant Enterococcus species 3rd generation cephalosporin-non-susceptible Enterobacterales Carbapenem-resistant Enterobacterales, as defined by the Center for Disease Control and Prevention: resistant to imipenem, meropenem, doripenem, or ertapenem OR documentation that the isolate produces a carbapenemase Pseudomonas aeruginosa resistant to carbapenems/multi-drug resistant (resistant to aminoglycosides, cephalosporins, fluoroquinolones, and carbapenems) Carbapenem-resistant Acinetobacter species Candida auris |

De-escalation: Changing to a narrower spectrum antibiotic , cessation of one or more antibiotics, or changing from an IV to PO route of appropriate drug (i.e. IV to PO ciprofloxacin or levofloxacin). |
