Uno studio randomizzato, in doppio cieco, di fase 3 su tucatinib o placebo in combinazione con trastuzumab e pertuzumab come terapia di mantenimento per il carcinoma mammario metastatico HER2+ (HER2CLIMB-05)

Questo studio è stato condotto per vedere se tucatinib funziona meglio del placebo quando somministrato con altri farmaci per trattare i partecipanti con carcinoma mammario HER2-positivo. Un placebo è una pillola che ha lo stesso aspetto di tucatinib ma non contiene medicine. Questo studio verificherà anche quali effetti collaterali si verificano quando i partecipanti assumono questa combinazione di farmaci. Un effetto collaterale è tutto ciò che un farmaco fa al corpo oltre a curare la tua malattia.

I partecipanti avranno un cancro che si è diffuso nel corpo vicino a dove è iniziato (localmente avanzato) e non può essere rimosso (non resecabile) o si è diffuso attraverso il corpo (metastatico).

In questo studio, tutti i partecipanti riceveranno tucatinib o placebo. I partecipanti verranno assegnati in modo casuale a un gruppo. Questo è uno studio in cieco, quindi i pazienti e i loro medici non sapranno a quale gruppo appartiene un partecipante.

Tutti i partecipanti riceveranno anche trastuzumab e pertuzumab. Questi sono 2 farmaci usati per trattare questo tipo di cancro.

Braccio di controllo: placebo somministrato per via orale due volte al giorno più trastuzumab e pertuzumab ogni 21 giorni

Braccio sperimentale: Tucatinib 300 mg somministrato per via orale due volte al giorno più trastuzumab e pertuzumab ogni 21 giorni

Trastuzumab e pertuzumab saranno somministrati come segue:

• Trastuzumab verrà somministrato per via endovenosa (IV) alla dose di 6 mg/kg o per via sottocutanea (SC) alla dose fissa di 600 mg, una volta ogni 21 giorni.

E

  • Pertuzumab verrà somministrato IV a 420 mg ogni 21 giorni. O
  • La combinazione a dose fissa di 600 mg di pertuzumab, 600 mg di trastuzumab e 20.000 unità di ialuronidasi verrà somministrata SC, una volta ogni 21 giorni, al posto di trastuzumab e pertuzumab individualmente.

Descrizione

Criterio di inclusione:

  • Carcinoma mammario HER2+ confermato a livello centrale secondo le linee guida del 2018 dell'American Society of Clinical Oncologists (ASCO) College of American Pathologists (CAP).

  • Avere una malattia localmente avanzata o metastatica non resecabile.

    • In caso di recidiva (dopo terapia [neo]adiuvante), il trattamento deve essere libero da almeno 6 mesi da qualsiasi trastuzumab o pertuzumab ricevuto per la malattia HER2+ avanzata.
  • Hanno ricevuto 4-8 cicli (cicli di 21 giorni) di precedente trattamento con trastuzumab, pertuzumab e taxano come terapia di prima linea per carcinoma mammario HER2+ avanzato senza evidenza di progressione della malattia
  • Stato noto del recettore ormonale (secondo le linee guida locali; può essere positivo per il recettore ormonale [HR+] o negativo [HR-])
  • Performance Status dell'Eastern Cooperative Oncology Group (ECOG) pari a 0 o 1

  • Inclusione del sistema nervoso centrale - Sulla base dello screening con risonanza magnetica cerebrale (MRI) con contrasto di contrasto, i partecipanti possono avere uno dei seguenti:

    • Nessuna evidenza di metastasi cerebrali
    • Metastasi cerebrali non trattate che sono asintomatiche e, se identificate su precedente imaging cerebrale, senza evidenza di progressione dall'inizio della terapia di induzione di prima linea con trastuzumab, pertuzumab e taxano

    • Metastasi cerebrali precedentemente trattate che sono asintomatiche

      • Le metastasi cerebrali precedentemente trattate con terapia locale non devono essere progredite dopo il trattamento

Criteri di esclusione:

  • Trattamento precedente con qualsiasi inibitore della tirosin-chinasi anti-HER2 e/o anti-recettore del fattore di crescita epidermico (EGFR) inclusi pirotinib, lapatinib, tucatinib, neratinib e afatinib (eccetto neratinib se somministrato in un contesto adiuvante prolungato e sono trascorsi ≥ 12 mesi dall'ultima dose di neratinib prima dell'inizio del farmaco in studio)
  • Impossibile sottoporsi a contrasto MRI del cervello

  • Esclusione del sistema nervoso centrale - Basato sullo screening della risonanza magnetica cerebrale e sulla valutazione clinica

    • Metastasi cerebrali sintomatiche
    • Progressione delle metastasi cerebrali dall'inizio della prima linea trastuzumab, pertuzumab e taxano
    • Uso continuativo di corticosteroidi sistemici a una dose totale giornaliera di >2 mg di desametasone (o equivalente)
    • Qualsiasi lesione cerebrale non trattata in un sito anatomico che può rappresentare un rischio per il partecipante
    • Malattia leptomeningea nota o sospetta (LMD)
    • Convulsioni scarsamente controllate (>1/settimana) o altri sintomi neurologici persistenti
