In de perioperatieve periode kunnen ernstige veranderingen worden waargenomen in het endocriene, immuunsysteem en zenuwstelsel. Deze veranderingen worden de chirurgische stressrespons genoemd. Klinisch kan dit worden waargenomen als een verhoogde pijnrespons en stoornissen in het circadiane ritme, geheugen, cognitieve en executieve functies, en kan leiden tot postoperatief delirium. De postoperatieve cognitieve disfunctie wordt in verband gebracht met een verhoogde mortaliteit en het risico van voortijdig verlaten van het beroep. Postoperatief delirium met fluctuerende niveaus van aandacht en bewustzijn is een ernstige aandoening die gepaard gaat met een slechte uitkomst, waaronder langere ziekenhuisopname en verhoogde vroege mortaliteit.
Algemene anesthesie kan bijdragen aan de chirurgische stressrespons. Sommige gegevens geven aan dat algehele anesthesie op zichzelf veranderingen en verstoringen in de hersenen kan veroorzaken, zoals slaap- en circadiane stoornissen. Recent bewijs suggereert dat anesthetica de neurogenese kunnen schaden en geheugenstoornissen kunnen veroorzaken. Bovendien kan inhalatie-anesthesie leiden tot meer cerebrale stoornissen in vergelijking met totale intraveneuze anesthesie (TIVA). In deze studie zullen we de effecten isoleren van de twee primaire hypnotica die worden gebruikt bij algemene anesthesie, sevofluraan en propofol, zonder de interacties en confounders van polyfarmacie en variërende ziekteniveaus bij een chirurgische populatie.
Materialen
De studie bestaat uit twee studiesessies. In elke studiesessie zullen MRI-scans (Magnetic Resonance Imaging) worden gemaakt voor en na algehele anesthesie, en bovendien na één en acht dagen.
De volgende beeldvormingsmodaliteiten zullen worden gebruikt: (i) T1-gewogen 3D-anatomie (T1w3D), (ii) Diffusion Tensor Imaging (DTI), (iii) rusttoestand functionele MRI (rsfMRI). De MRI-scans worden aangevuld met cognitieve testen, slaapevaluatie en bloedafname. De opzet voor elke vrijwilliger is dus:
Sessie één:
Dag 0: MRI 1, cognitieve testen, vragenlijsten en bloedafname. Algemene anesthesie (onderhoudsfase met sevofluraan OF propofol volgens randomisatie) en zorg na de anesthesie. MRI 2 en herhaalde cognitieve testen, vragenlijsten en bloedafname.
Dag +1: MRI 3, cognitieve testen, vragenlijsten en bloedafname.
Dag +8: MRI 4, cognitieve testen en vragenlijsten.
Sessie twee: Identiek aan sessie één, behalve dat de vrijwilliger het resterende type algemene anesthesie krijgt (sevofluraan of propofol, tegengesteld aan sessie één).
Gegevensevaluatie zal worden uitgevoerd met een beoordelaar die blind is voor het type anesthesie.

Beschrijving
Inclusiecriteria:
- Leeftijd ≥18 en ≤35.
- Gezond individu.
- BMI ≥18 kg/m2 en ≤30kg/m2.
- Normaal elektrocardiogram (ECG).
- Normaal lichamelijk onderzoek, inclusief neurologisch onderzoek, auscultatie van hart en longen en meting van bloeddruk en pols.
- American Society of Anaesthesiologists (ASA) klasse 1.
- Mallampati I-II en vereenvoudigde luchtwegrisico-index (SARI) 0-2 (d.w.z. geen indicatie van moeilijke intubatie). Zie bijlage voor details.
- Rechtshandig.
- Vrouwelijke deelnemers moeten veilige anticonceptiva gebruiken (hormonaal of mechanisch, inclusief spiraaltjes).
- Spreekt en verstaat Deens.
- Geeft mondelinge en schriftelijke geïnformeerde toestemming.
Uitsluitingscriteria:
- Contra-indicaties voor MRI.
- Linkshandigheid of ambidexteriteit.
- Voorgeschiedenis van complicaties bij algehele anesthesie, waaronder maligne hyperthermie.
- Familiegeschiedenis van kwaadaardige hyperthermie.
- Bekend incident van kwaadaardige hyperthermie of een onverklaarbare complicatie van algemene anesthesie bij naaste familieleden.
- Allergie voor elk soort medicatie of materiaal waaraan de vrijwilliger tijdens dit onderzoek zou kunnen worden blootgesteld.
- Geschiedenis van ernstige ziekte.
- Geschiedenis van kanker, immuunziekte, auto-immuunziekte, chronische pijn of neurologische / psychiatrische ziekte.
- Groot trauma of hoofdtrauma met eventuele symptomen op het moment van opname.
- Chirurgie minder dan zes weken voorafgaand aan de onderzoeksperiode.
- Infectie (met koorts) minder dan twee weken voorafgaand aan of tijdens de studiesessies.
- Dagelijks gebruik van medicatie (anticonceptiva niet meegerekend).
- Gebruikte antidepressiva gedurende de laatste 30 dagen vóór studiedagen.
- Zwakke inname van >21 (voor vrouwen >14) eenheden alcohol.
- Zware inname van cafeïne (> 5 kopjes/dag).
- Roken in de laatste 30 dagen voor studiedagen.
- Middelenmisbruik (beoordeeld door de onderzoeker).
- Zwangerschap
- Reflux of dyspepsie.
- Slechte tandheelkundige status of mondgezondheid.
- Verwachte of vermoede moeilijke luchtweg.
- Weigert informatie te ontvangen over toevallige pathologische bevindingen tijdens MRI-scans van de hersenen.
- Kan niet meewerken aan testen.
- Anders ongeschikt bevonden voor deelname door de onderzoeker.
Uitsluitingscriteria tijdens het onderzoek:
- Een van de bovengenoemde uitsluitingscriteria.
- Groot trauma of hoofdtrauma tijdens de studieperiode.
- Chirurgie tijdens de studieperiode.
- Infectie (met koorts) tijdens de studieperiode.
- Consumptie van meer dan 3 eenheden alcohol binnen 24 uur voor elke studiedag (interventiedag of MRI-scandag)
- Verbruikte analgetica binnen 3 dagen voor elke studiedag
- Gebruikte antihistaminica minder dan 48 uur voor elke studiedag
- Inname van cafeïne 12 uur voorafgaand aan elke studiedag
