Effect van geautomatiseerde insulinetoediening met geavanceerde closed-loop op glucoseresultaten en vroege diabetische complicaties

Nieuwe algoritmen voor de automatisering van insulinetoediening (AID) laten een groot voordeel zien bij de glucoseregulatie bij mensen met diabetes type 1. Hybride closed loop (HCL)-systemen kunnen inderdaad de HbA1c-waarden, het percentage van de tijd in het gedefinieerde glucosebereik, de tijd onder het bereik en de tijd boven het bereik verbeteren, volgens de resultaten van RCT en observationele onderzoeken. Wetenschappelijke bewijzen die potentiële voordelen aantonen voor de vermindering van diabetescomplicaties zijn echter beperkt met betrekking tot CSII of SAP met aangetoonde vermindering van cardiovasculaire mortaliteit, verbetering van albuminurie en perifere zenuwbeschadiging.
Gegevens over AID-effecten op complicaties van diabetes ontbreken. In deze studie zullen tussentijdse schademarkers worden gemeten om de mogelijke effecten van AID in vergelijking met pompen met sensor te beoordelen.

Beschrijving
Inclusiecriteria:
- Mannelijke en vrouwelijke patiënten
- T1D-patiënten ouder dan 18 jaar in CSII-behandeling gedurende ten minste 3 maanden
- HbA1c-waarden tussen 6,0% en 9,5%
- Ziekteduur ≥ 2 jaar
- Schriftelijke geïnformeerde toestemming verkregen van de patiënt
Uitsluitingscriteria:
- Zwangerschap
- Deelname aan andere klinische onderzoeken
- Een geschiedenis van alcohol- of drugsmisbruik
- Gevorderde diabetische nefropathie gedefinieerd als aanwezigheid van albuminurie ≥ 300 mg/g of eGFR < 60 ml/min/1,73m2
- Proliferatieve diabetische retinopathie of macula-oedeem
- Gevestigde atherosclerotische cardiovasculaire ziekte (ASCVD) of voorgeschiedenis van hartfalen
- Aanwezigheid van ernstige ziekten of aandoeningen waardoor de patiënt naar de mening van de onderzoeker niet in aanmerking komt voor het onderzoek
- Hypoglykemie Onbewust (Clarke-score> 4)
- Patiënten die de gesproken en geschreven Italiaanse taal niet kunnen verstaan

Endotheelafhankelijke dilatatie (EDD) wordt beschouwd als een marker van disfunctionele afwijkingen die betrokken zijn bij vroege fasen van de ontwikkeling van atherosclerose. Veranderingen in EDD gaan vooraf aan structurele veranderingen en treden op in de preklinische fase van vaatziekten. In de huidige studie wordt endotheliale afhankelijke dilatatie beoordeeld door kleur Doppler-evaluatie van stroomtoename na hyperemie. |
