Avance1 is een fase 1/2a, monocentrische, open-label studie om de veiligheid, farmacokinetiek en farmacodynamiek van SQY51 te evalueren bij 12 patiënten met een genetisch bevestigde diagnose van Duchenne spierdystrofie. Deze studie omvat i) 13 weken durende fase 1 meervoudige dosis Escalatiefase, en a ii) 32 weken durende fase 2a.
Twaalf (12) patiënten ≥ 6 jaar, zowel ambulant als niet-ambulant, zullen achtereenvolgens worden opgenomen in fase 1 en zullen eenmaal per twee weken stijgende doses SQY51 krijgen. In fase 2a worden patiënten op niet-gerandomiseerde wijze ingedeeld in drie cohorten.

Beschrijving
INSLUITINGSCRITERIA VOOR FASE 1:
- Jongens van ≥6 jaar en ≥ 16 kg lichaamsgewicht.
- Ambulante of niet-ambulante status,
- Patiënten en, indien minderjarig, hun wettelijke voogden, die bereid en in staat zijn zich te houden aan geplande bezoeken, het behandelplan, laboratoriumtests en andere onderzoeksprocedures.
- Gediagnosticeerd met Duchenne spierdystrofie (DMD), genotypisch bevestigd met DMD-mutaties die vatbaar zijn voor exon-51 skipping.
- Stabiele lever- en nierfunctie.
- Linkerventrikelejectiefractie (LVEF) bij screening ≥40%.
- Indien klinisch geïndiceerd, goedgekeurde gelijktijdige behandeling binnen de zorgrichtlijnen voor DMD, zoals antihypertensiva, vasodilatatoren, lipidenverlagende middelen, vervanging van de schildklier, vitamines, vervanging van mineralen, maagbeschermers en voedingssupplementen.
- Niet-invasieve mechanische ventilatie is toegestaan indien < 16 h/dag.
- Aangesloten zijn bij een Franse sociale zekerheid.
- Geïnformeerd toestemmingsformulier ondertekend door de patiënt of, indien minderjarig, door de wettelijke voogd(en).
INSLUITINGSCRITERIA VOOR FASE 2a:
Patiënten moeten fase 1 van de studie hebben voltooid.
UITSLUITINGSCRITERIA VOOR FASE 1 EN 2a:
- Patiënt met een ernstige medische/chirurgische of psychiatrische aandoening/ziekte/voorgeschiedenis die naar de mening van de onderzoeker de veiligheid van de patiënt in gevaar zou brengen of de evaluaties/resultaten van het onderzoek zou verstoren, inclusief onvoldoende vaccinatie tegen besmettelijke ziekten zoals aanbevolen door nationale richtlijnen, medische voorgeschiedenis van infectie met Hepatitis B, C en HIV.
- Patiënt met bekende allergieën voor producten die waarschijnlijk in het onderzoek zullen worden gebruikt (bijv. antiseptica, anesthetica), bekende overgevoeligheid voor een van de ingrediënten of hulpstoffen van het onderzoeksgeneesmiddel).
- Patiënt die in de afgelopen drie maanden heeft deelgenomen aan een ander onderzoeksonderzoek, waaronder patiënten met onderzoeksgeneesmiddelen die gericht zijn op het herstel van de dystrofine-expressie, zoals andere antisense-oligomeren.
- Patiënt die gentherapie kreeg.
- Patiënt met een verstandelijke beperking of zodanig gedragsprobleem dat zij de onderzoeksprocedure niet kunnen volgen.
- Patiënt met gevorderde cardiomyopathie en LVEF < 40%. Patiënten met ritmestoornissen en worden behandeld voor ritmestoornissen. Patiënten met onbehandelde tachycardie.
- Patiënt voor wie orthopedische chirurgie is gepland tijdens de duur van het onderzoek.
- Patiënten met een tracheotomie en afhankelijk van invasieve mechanische beademing. Niet-invasieve mechanische ventilatie ≥ 16 u/dag. Voorspelde vitale geforceerde capaciteit < 20%. Medische voorgeschiedenis met meer dan twee respiratoire decompensaties waarvoor ziekenhuisopname nodig was in het voorgaande jaar. Geen respiratoire decompensatie in de vier maanden voorafgaand aan inschrijving.
- Patiënten die medicijnen gebruiken die de dystrofine-expressie kunnen herstellen, tamoxifen en andere medicijnen zonder indicatie voor DMD of pediatrische patiënten.
- Abnormale laboratoriumwaarden in het klinisch significante bereik.
