Avance1 er et fase 1/2a, monocentrisk, åbent label-studie til evaluering af sikkerheden, farmakokinetikken og farmakodynamikken af SQY51 hos 12 patienter med en genetisk bekræftet diagnose af Duchenne muskeldystrofi. Denne undersøgelse vil omfatte i) 13-ugers fase 1 multiple dosis Eskaleringsfase, og a ii) 32-ugers fase 2a.
Tolv (12) patienter ≥ 6 år, både ambulante og ikke-ambulante, vil blive sekventielt indskrevet i fase 1 og vil modtage eskalerende doser af SQY51 en gang hver anden uge. I fase 2a vil patienter blive allokeret i tre kohorter på en ikke-randomiseret måde.

INKLUSIONSKRITERIER FOR FASE 1:
- Drenge på ≥6 år og ≥16 kg kropsvægt.
- Ambulatorisk eller ikke-ambulatorisk status,
- Patienter og, hvis mindreårige, deres juridiske værger, som er villige og i stand til at overholde planlagte besøg, behandlingsplan, laboratorietests og andre undersøgelsesprocedurer.
- Diagnosticeret med Duchenne muskeldystrofi (DMD), genotypisk bekræftet med DMD-mutationer, der er modtagelige for exon-51-spring.
- Stabil lever- og nyrefunktion.
- Venstre ventrikulær ejektionsfraktion (LVEF) ved screening ≥40 %.
- Hvis det er klinisk indiceret, godkendt samtidig behandling inden for standarder for plejeretningslinjer for DMD, såsom antihypertensiva, vasodilatorer, lipidsænkende, thyreoideaudskiftning, vitaminer, mineralsubstitution, gastriske beskyttere og kosttilskud.
- Non-invasiv mekanisk ventilation er tilladt, hvis < 16 timer/dag.
- At være tilknyttet en fransk socialsikring.
- Formular til informeret samtykke underskrevet af patienten eller, hvis den er mindreårig, af den eller de juridiske værger.
INKLUSIONSKRITERIER FOR FASE 2a:
Patienterne skal have gennemført fase 1 af undersøgelsen.
UDELUKKELSESKRITERIER FOR FASE 1 OG 2a:
- Patient med enhver alvorlig medicinsk/kirurgisk eller psykiatrisk tilstand/sygdom/historie, som efter investigatorens opfattelse ville bringe patientens sikkerhed i fare eller ville forstyrre undersøgelsens vurderinger/resultater, herunder utilstrækkelig vaccination mod infektionssygdomme som anbefalet af nationale retningslinjer, sygehistorie for infektion med hepatitis B, C og HIV.
- Patient med kendte allergier over for produkter, der sandsynligvis vil blive brugt i undersøgelsen (f.eks. antiseptiske midler, anæstetika), kendt overfølsomhed over for nogen af ingredienserne eller hjælpestoffer i undersøgelseslægemidlet).
- Patient, der har deltaget i andre undersøgelsesundersøgelser inden for de sidste tre måneder, inklusive dem med undersøgelseslægemidler, der sigter mod at genoprette dystrofinekspression, såsom andre antisense-oligomerer.
- Patient, der modtog genterapi.
- Patient med et intellektuelt handicap eller adfærdsproblem, så de ikke kan overholde undersøgelsesproceduren.
- Patient med fremskreden kardiomyopati og LVEF < 40 %. Patienter med rytmeforstyrrelser og i behandling for rytmeforstyrrelser. Patienter med ubehandlet takykardi.
- Patient, for hvilken der er planlagt ortopædkirurgi i løbet af undersøgelsen.
- Trakeostomiserede patienter og afhængige af invasiv mekanisk ventilation. Non-invasiv mekanisk ventilation ≥ 16 t/dag. Forudsagt vital forceret kapacitet < 20%. Sygehistorie med mere end to respiratoriske dekompensationer, der kræver indlæggelse i løbet af det foregående år. Ingen respiratorisk dekompensation i de fire måneder forud for indskrivning.
- Patienter på medicin, der kan genoprette dystrofinekspression, tamoxifen og andre lægemidler uden indikation for DMD eller pædiatrisk population.
- Unormale laboratorieværdier i det klinisk signifikante område.
