Mais de 15% dos adultos americanos com diabetes tipo 2 têm glicose no sangue mal controlada, aqui definida como um nível de hemoglobina glicada (HbA1c) de 8,0% ou superior. Esses adultos têm um risco elevado para a saúde de uma variedade de resultados, incluindo amputação e mortalidade por doenças cardiovasculares e por todas as causas. As intervenções focadas na nutrição podem ser eficazes para melhorar o controle glicêmico, reduzir os medicamentos anti-hiperglicêmicos e reduzir o peso corporal, todos resultados críticos para adultos com diabetes tipo 2. No entanto, as intervenções típicas focadas na nutrição podem ser onerosas, muitas vezes exigindo instruções complexas e uma revisão completa da dieta. Além disso, adultos com diabetes tipo 2 mal controlado têm maior probabilidade de ter baixos níveis de alfabetização, o que pode ser uma barreira para a adesão a intervenções complexas. Portanto, uma intervenção eficaz para adultos com diabetes mal controlado que podem ter níveis mais baixos de alfabetização em saúde é necessária para reduzir os níveis de HbA1c e os medicamentos anti-hiperglicêmicos. A ingestão de carboidratos tem o impacto mais forte na glicemia pós-prandial de qualquer fator dietético, e uma dieta muito pobre em carboidratos - devido à sua capacidade de melhorar o controle glicêmico - é agora recomendada pela American Diabetes Association (ADA) para o tratamento do diabetes tipo 2 diabetes. Os pesquisadores levantam a hipótese de que alguns dos benefícios de uma dieta com baixo teor de carboidratos podem estar disponíveis para indivíduos que mudam apenas o café da manhã para um baixo teor de carboidratos, em vez de modificar toda a dieta.
Assim, os investigadores conduzirão um estudo de aceitabilidade, viabilidade e eficácia preliminar de um programa de café da manhã com duração de 4 meses, on-line, pequenos passos, baixo nível de alfabetização e muito baixo teor de carboidratos em 120 adultos com diabetes tipo 2 mal controlado. Os investigadores medirão a aceitabilidade e a viabilidade, além de resultados críticos de eficácia, como alterações na HbA1c, medicamentos anti-hiperglicêmicos, variabilidade glicêmica, peso corporal, pressão arterial e lipídios.
Os investigadores também testarão se fatores como sexo, nível de alfabetização em saúde e resistência basal à insulina moderam significativamente o impacto da intervenção na mudança na HbA1c e na mudança nos medicamentos anti-hiperglicêmicos.

Critério de inclusão:
- HbA1c 8% ou superior
- 18-80 anos
- A capacidade de compreender o inglês verbal e escrito
- Vontade de seguir a dieta prescrita
- Capaz de consentir e seguir instruções
- Disposição para verificar regularmente os níveis de glicose no sangue, conforme necessário
Critério de exclusão:
- Incapacidade de fornecer consentimento informado
- Grávida, amamentando ou planejando nos próximos 6 meses ou <6 meses após o parto
- Baixo peptídeo C e possível nível subseqüente de GAD 65 que sugere diabetes tipo 1, fatores clínicos que sugerem diabetes tipo 1 (magra, falta de histórico familiar e cetoacidose diabética no passado) ou diagnóstico anterior de diabetes tipo 1 ou diabetes autoimune latente
- Câncer, insuficiência cardíaca ou insuficiência renal
- vegano
- Condição de saúde mental não tratada
- Atualmente seguindo uma dieta ou café da manhã com muito baixo teor de carboidratos
- Níveis de tireoide fora do intervalo
- Alcoolismo
- Cirurgia bariátrica anterior
- Dificuldade para mastigar ou engolir
- Dependência de outros para preparação de alimentos
- Atualmente inscrito em outro estudo investigativo que pode entrar em conflito com esta pesquisa

HbA1c is the proportion of hemoglobin in the blood that has glucose attached to it relative to total hemoglobin.
It reflects average blood glucose levels over approximately the previous 2-3 months.
To assess the effect of dietary change on HbA1c, the change in HbA1c from baseline to 4 months were evaluated.
A negative change indicates an improvement in HbA1c from baseline.

Change in anti-hyperglycemic medications

The Medication Effect Score (MES) quantifies the overall intensity of antiglycemic therapy based on potency and dosages of medications (Alexopoulos et al., 2021), with higher scores indicating greater medication intensity.
It is interpreted as the reduction in HbA1c attributable to the medication regime.
For each diabetes medication, the percentage of the maximum recommended daily dose is multiplied by an adjustment factor reflecting its expected hemoglobin A1c-lowering potency; these values are summed to yield the total MES.
Scores range from 0 upward (no fixed maximum), with lower scores reflecting lower medication intensity.
The change in MES from baseline to 4 months was evaluated, with negative values indicating a reduction in medication intensity.
