Efficacité du gel muco-bioadhésif au gingembre dans la prise en charge du lichen plan buccal : essai clinique contrôlé randomisé avec analyse immunohistochimique
- Comparer l'efficacité clinique de l'extrait topique de gingembre par rapport à l'acétonide de triamcinolone à 0,1 % pour le lichen plan buccal symptomatique (objectif principal).
- Étudier à l'aide d'une analyse immunohistochimique l'effet des deux modalités de traitement différentes sur l'expression de FasL dans les lésions de lichen plan buccal (objectif secondaire).

La description
Critère d'intégration:
- Hommes ou femmes, âgés de 40 à 60 ans.
- Formes douloureuses bulleuses/érosives (score 4) ou atrophiques (score 3) cliniquement prouvées de LPO confirmées par la présence de remaniements rouges ou érythromateux, ou d'ulcérations peu profondes avec de fines lignes de dentelle à la périphérie de la lésion accentuées par l'étirement et non éliminées par le frottement (stries de Wickham) (Lozada-Nur et Miranda, 1997)
- Formes histopathologiquement prouvées bulleuses/érosives (score 4) ou atrophiques (score 3) de PLO confirmées par la présence de critères histopathologiques acceptés pour le lichen plan ; liquéfaction des cellules basales, infiltrat lymphocytaire semblable à une bande à la jonction épithéliale-stromale avec dégénérescence de la région des cellules basales (Ellis, 1967 ; Van der Meij et Van der Waal, 2003).
Critère d'exclusion:
- Antécédents de lésions lichénoïdes d'origine médicamenteuse.
- Présence de conditions systémiques comme ; infections graves actives ou récurrentes, malignité, diabète sucré, hypertension ou maladies cardiaques, hépatiques ou rénales importantes. Évalué à l'aide d'un questionnaire médical guidé par Cornell Medical Index (Pendleton et al., 2004).
- Fumeur.
- Hypersensibilité connue ou effets indésirables graves aux médicaments de traitement ou à tout ingrédient de leur préparation tel que mentionné dans les antécédents médicaux.
- Grossesse ou allaitement.
- Antécédents de traitements antérieurs potentiellement efficaces sur l'OLP tels que les agents antipaludéens, les rétinoïdes, les corticostéroïdes ou les médicaments immunosuppresseurs depuis moins de 2 semaines pour les médicaments topiques et 4 semaines pour les médicaments systémiques avant le début de l'étude (Swift et al., 2005).
- Patients souffrant de lésions cutanées de type lichen plan.
- Perte de souplesse ou de flexibilité des tissus concernés par les lésions buccales du lichen plan.
- Signes histologiques de dysplasie épithéliale ou de lésions lichénoïdes au sein de la biopsie.
- Refus de participer à l'étude.
- Groupes vulnérables (handicapés, orphelins et prisonniers).

Groupe I Comprendra 14 patients recevant de l'extrait topique de gingembre à 1 % quatre fois par jour (après chaque repas et avant d'aller au lit) pendant huit semaines (Zakaria et al., 2020).

Comparateur actif: Groupe de stéroïdes topiques
Groupe II Comprendra 14 patients recevant un stéroïde topique (acétonide de triamcinolone à 0,1 %) quatre fois par jour pendant huit semaines (Laeijendecker et al., 2006) Au cours des quatrième et huitième semaines, un antifongique topique sera appliqué par un personnel médical tiers pour éviter la candidose 2ry pour ce groupe. |
Le gel d'acétonide de triamcinolone à 0,1 % sera appliqué localement sur les participants souffrant de lichen plan buccal
