Tidsavhengig amiodaronbehandling ved atrieflimmer
Bruk av tidsavhengig intravenøs infusjon av amiodaron for konvertering til sinusrytme hos pasienter med paroksysmal atrieflimmer
Seksti påfølgende akuttpasienter med nylig diagnostisert atrieflimmer ble randomisert i to grupper. Den første fikk intravenøse amiodaroninfusjoner kontinuerlig i 24 timer; den andre fikk Amiodaron inntil sinusrytmen ble nådd eller i opptil 72 timer.

En åpen randomisert klinisk studie ble utført som inkluderte 60 akutte voksne pasienter som var kandidater for medikamentalkonvertering etter første symptomatiske episoder av AF.
Pasientene ble randomisert i like proporsjoner og sekvensielt plassert i en av to behandlingsgrupper; ingen pasient visste hvilken gruppe han eller hun tilhørte. Den første gruppen fikk intravenøse infusjoner av amiodaron i doser på 5 mg/kg i over 30 minutter, etterfulgt av 20 mg/kg til 24 timer var nådd. Den andre gruppen fikk intravenøse amiodaroninfusjoner i doser på 5 mg/kg i over 30 minutter, etterfulgt av 20 mg/kg hver 24. time i opptil 72 timer.
Alle pasientene ble kontinuerlig overvåket ved hjelp av elektrokardiogrammer, og ekkokardiogrammer ble utført innen 24 timer etter at den randomiserte studien startet. Det nøyaktige tidspunktet for hver pasients kardioversjon ble dokumentert. I tillegg ble det tatt generelle laboratorietester, lipidprofiler, skjoldbruskkjertelprofiler og røntgen av thorax for alle pasienter.
En prøvestørrelse på 30 forsøkspersoner i hver gruppe ble beregnet. En konverteringsrate på 60 % ble antatt, ifølge litteraturen, og en estimert økning til 90 % ble antatt for at forsøksgruppen skulle finne forskjeller med en potens på 0,8 og en alfafeil på 0,05.
Det primære målet var å oppnå en sinusrytmekonverteringsrate. Det sekundære målet var å oppnå en kombinert komplikasjonsrate ved å notere ulike komplikasjoner hos pasientene, slik som flebitt, symptomatisk hypotensjon som krevde vasopressorer, bradykardi, kvalme, oppkast og eventuell organemboli.

- Første symptomatiske episoder av atrieflimmer
Ekskluderingskriterier:
- hemodynamisk ustabilitet,
- Tidligere bruk av antiarytmika
- Bruk av digoksin syv eller færre dager før studien
- Aktiv skjoldbrusk sykdom
- Kjente bivirkninger av amiodaron
- Andre ventrikulære arytmier
- Kontraindikasjoner for antikoagulasjon
- Akutt nyresvikt
- Kronisk leversykdom
- Transaminasenivåer som var to ganger de normale nivåene
- Akutt lungeødem
- Ukontrollert hypertensjon (> 180/110 mmHg)
- Ustabil angina
- Pasienter som var gravide og/eller amming
