Un ensayo pivotal prospectivo, multicéntrico, aleatorizado y controlado para evaluar la seguridad y la eficacia de la reparación transcatéter de la válvula mitral con el sistema de reparación transcatéter de válvula PASCAL de Edwards en comparación con Abbott MitraClip en pacientes con regurgitación mitral degenerativa (DMR, por sus siglas en inglés) que se ha determinado que tienen un nivel prohibitivo. riesgo de cirugía de la válvula mitral por parte del Heart Team, y en pacientes con insuficiencia mitral funcional (FMR) en tratamiento médico dirigido por las guías (GDMT)

Criterios de inclusión:

  • Dieciocho (18) años de edad o más
  • El paciente puede y está dispuesto a dar su consentimiento informado y seguir los procedimientos del protocolo, y cumplir con los requisitos de la visita de seguimiento.
  • El equipo cardiaco determina que el paciente tiene un riesgo prohibitivo de cirugía de la válvula mitral (cohorte CLASP IID únicamente).
  • El paciente recibe medicamentos estables para la insuficiencia cardíaca/terapia médica dirigida por las pautas (cohorte CLASP IIF solamente)
  • El equipo cardiaco determina que el paciente es candidato para la reparación transcatéter de la válvula mitral tanto para PASCAL como para MitraClip
  • Insuficiencia mitral (3+ a 4+) por eco
  • Válvula adecuada y morfología de chorro regurgitante
  • BNP corregido elevado > 400 pg/ml o NT-pro BNP corregido > 900 pg/ml u hospitalización por insuficiencia cardíaca en los últimos 12 meses (solo cohorte CLASP IIF)
  • FEVI ≥ 20 % (y ≤ 50 %; solo cohorte CLASP IIF)

Criterio de exclusión:

  • Paciente en el que está contraindicado un TEE o el TEE de cribado no tiene éxito
  • Anatomía de la válvula mitral que puede impedir el acceso, uso y/o despliegue adecuados de PASCAL o MitraClip o una reducción suficiente de la regurgitación mitral
  • Paciente con insuficiencia cardíaca refractaria que requiere intervención avanzada (es decir, dispositivo de asistencia ventricular izquierda, estado ≤5 trasplante de corazón) (insuficiencia cardíaca ACC/AHA etapa D)
  • Enfermedad arterial coronaria no tratada clínicamente significativa
  • Accidente cerebrovascular reciente
  • Otros trastornos valvulares graves que requieren intervención
  • Necesidad de cirugía emergente o urgente por cualquier motivo o cualquier cirugía cardíaca planificada dentro de los próximos 12 meses
  • Cualquier cirugía previa de válvula mitral o procedimiento transcatéter de válvula mitral (excluyendo reemplazo de cuerdas o reparación de anuloplastia quirúrgica)
  • Insuficiencia tricuspídea grave o enfermedad de la válvula tricúspide que requiere cirugía
  • Cardiopatía reumática activa o etiología reumática de la RM
  • Estenosis o regurgitación aórtica severa
  • Antecedentes conocidos de estenosis carotídea grave no tratada
  • Trombosis venosa profunda (TVP) o embolia pulmonar (EP) reciente
  • EPOC grave
  • Embarazada o planeando un embarazo dentro de los próximos 12 meses. Nota: Las pacientes en edad fértil deben tener una prueba de embarazo negativa realizada dentro de los 14 días anteriores a la intervención y cumplir con un método anticonceptivo aceptado.
  • Condición médica concurrente con una expectativa de vida de menos de 12 meses a juicio del Investigador
  • El paciente está participando actualmente en otro estudio clínico de investigación biológico, medicamento o dispositivo en el que no se alcanzó el criterio principal de valoración del estudio en el momento de la inscripción.
  • Otras condiciones médicas, sociales o psicológicas que impiden el consentimiento y el seguimiento adecuados, incluidos los pacientes bajo tutela
