Questa prova includerà 2 parti (fase 1 e fase 2).
La prima parte sarà uno studio di Fase I, in aperto, di incremento della dose per stabilire la dose massima tollerata (MTD) di XB2001 misurata mediante tossicità limitante la dose (DLT), in combinazione con il regime chemioterapico ONIVYDE + LV + 5-FU in pazienti con carcinoma pancreatico avanzato e determinare la dose raccomandata per il successivo studio di Fase 2.
La porzione di fase 2 sarà implementata con la massima dose tollerata stabilita (MTD) di XB2001. L'arruolamento target nella fase 2 è di 60 pazienti che saranno randomizzati su base 1:1 a XB2001 più ONIVYDE + LV + 5-FU (Braccio 1) o placebo più ONIVYDE + LV + 5-FU (Braccio 2).

Dimensione del campione: circa 69 pazienti saranno arruolati negli Stati Uniti (almeno 9 pazienti nella porzione di fase 1 in aperto e 60 pazienti nella porzione di fase 2 randomizzata)

Questa prova comprenderà 2 fasi. La prima parte sarà uno studio di Fase I, in aperto, di incremento della dose che valuterà la sicurezza, la tollerabilità e stabilirà la dose massima tollerata (MTD) di XB2001 in almeno nove pazienti con adenocarcinoma pancreatico metastatico che ricevono ONIVYDE + Leucovorin l + d racemico + Trattamento chemioterapico con 5-fluorouracile. La durata per ciascun paziente nella fase I sarà di 14 giorni (1 ciclo di trattamento) durante i quali verrà somministrata una dose endovenosa di XB2001 prima di ricevere il trattamento chemioterapico con ONIVYDE + Leucovorin l + d racemico + 5-Fluorouracile e valutata per la dose Tossicità limitate (DLT). La parte di Fase II sarà attuata a seguito del completamento della parte di Fase I e della dichiarazione del MTD. La durata della partecipazione del soggetto alla fase II randomizzata, in doppio cieco e controllata con placebo dello studio è di circa 28 settimane: compreso un periodo di screening fino a 30 giorni e un periodo di trattamento di 24 settimane. Tutti i soggetti dello studio possono continuare il trattamento con XB2001 in un'estensione in aperto, a condizione che siano giudicati clinicamente benefici e non abbiano avuto tossicità inaccettabili.

Criterio di inclusione:
- Adenocarcinoma pancreatico del pancreas esocrino confermato istologicamente o citologicamente che è metastatico, non resecabile o ricorrente
- Almeno una lesione misurabile secondo i criteri di valutazione della risposta nel tumore solido V1.1
- Progressione della malattia documentata dopo una precedente terapia a base di gemcitabina OPPURE una terapia combinata con FOLFIRINOX e gemcitabina
- Performance dell'Eastern Cooperative Oncology Group (ECOG) pari a 0 o 1 o Karnofsky performance status (KPS) ≥ 70
- Adeguata funzionalità epatica, renale e del midollo osseo
Criteri di esclusione:
- Diminuzione clinicamente significativa del performance status (cartella clinica) entro 2 settimane dalla somministrazione della prima dose prevista
- Disturbi gastrointestinali clinicamente significativi
- Eventi tromboembolici arteriosi gravi meno di 6 mesi prima dell'inclusione
- Precedente radioterapia cerebrale intera (WBRT)
- Evidenza di metastasi cerebrali
- Insufficienza cardiaca congestizia di classe NYHA III o IV, aritmie ventricolari o pressione arteriosa incontrollata (definita come ≥ 160/100 mm Hg)
- Uso di forti induttori o inibitori del CYP3A4 e/o inibitori dell'UGT1A1 entro 14 giorni prima della Visita 1/Visita al basale.

Sperimentale: Phase I: Dose Escalation Phase
In Phase I (Dose escalation phase), 3 sequential dose levels were administered by participants; 250 mg (dose cohort 1), 500 mg (dose cohort 2), and 1000 mg (dose cohort 3), and traditional 3:3 design was used. Dose-escalation will continue if none of three subjects experience a dose-limiting toxicity (DLT). After enrollment participants received two treatment cycles (a total of 28 days) in which they were given one intravenous (IV) dose of XB2001 prior to receiving ONIVYDE 70 mg/m2 IV over 90 minutes, followed by leucovorin 400 mg/m2 IV over 30 minutes, followed by 5-Fluorouracil 2400 mg/m2 intravenously over 46 hours at each cycle. |

The primary objective of the Phase I portion is to identify the MTD of XB2001 when administered in combination with ONIVYDE + LV + 5-FU. The MTD is defined as the highest dose level at which no more than one out of six subjects experience a DLT. If two or more subjects in a cohort of six experience a DLT, the MTD is considered exceeded, and the previous lower dose level will be identified as the MTD. If no DLTs are observed at the highest dose level studied, that dose will be used for Phase II. A DLT is defined as any Grade 3-4 adverse event (per NCI CTCAE v5.0) occurring within the first 28 days that is deemed possibly related to the combination regimen, unless the toxicity is clearly attributable to a single non-XB2001 component. Safety and Tolerability of XB2001 in Combination With ONIVYDE + LV + 5-FU

This endpoint evaluates the cumulative safety profile and patient tolerance of XB2001 when administered at the Maximum Tolerated Dose (MTD) or the maximum dose studied in Phase I, in combination with the ONIVYDE + LV + 5-FU regimen. Safety is characterized by the total number of participants in each cohort who experienced one or more adverse events. For the purpose of this count, each participant is counted only once, regardless of the total number of individual adverse events experienced. |

Measurement of the concentration of Natrunix in plasma samples to evaluate the pharmacokinetic profile. Due to reporting limitations on this portal, results from only one time point are reported here. For Phase 1, samples were collected at V1 pre-dose, V1 post-dose, V2 pre-dose, V3 follow up. For phase 2, samples were collected at V1 pre-dose, V1 post-dose, V1 Day 4, V1 Day 7, V2 pre-dose, V3 pre-dose, V4 pre-dose, V6 pre-dose, V8 pre-dose, and V13 follow up. Placebo patients were tested at V1 post-dose only for verification. |
