Schema voor primaire infectiepreventie door COVID-19, bij werknemers: fase II gecontroleerd klinisch onderzoek, uit te voeren in Medellín-Antioquia
Inleiding: het besmettingspercentage van SARS Co-2 is hoog (Ro: 2.0-3.0). De infectie is agressief met een hoge pathogeniteit. Wereldwijde opsluiting heeft gevolgen voor alle sociale en economische gebieden van menselijke activiteit. Klinisch gedrag overtreft de mogelijkheden van de openbare gezondheidszorg. Met de eerste informatie over de pandemie kan naar schatting 20% van het gezondheidspersoneel dat in direct contact staat met patiënten de ziekte krijgen, ondanks preventieve zelfzorg. De moleculaire relatie van COVID-19 met het ACE2-eiwit van de proefpersoon moedigt het virus aan om de gastheercel binnen te dringen, en initieert replicatie en de immuunrespons, waardoor een onevenwichtigheid ontstaat die een immunologische storm van cytokines veroorzaakt, met ernstige schade aan de gastheer.
Doelstelling: Er wordt voorgesteld om een gecombineerd schema van twee verbindingen aan te bieden door middel van orofaryngeale spray die het virus vangt voordat het de doelcel binnendringt en immunomodulerende verbindingen oraal toe te dienen die de door het virus afgegeven ontstekingsreactie reguleren, om de antivirale reactie te verbeteren.
Methodologie: Er zal een gecontroleerd, parallel opgezet, drievoudig blind, fase II klinisch onderzoek worden uitgevoerd met twee onderzoeksgroepen om de actieve bestanddelen (orofaryngeale spray en emulsie) te vergelijken met placebo.
Discussie: Met de toepassing van het gecombineerde schema van twee verbindingen wordt een vermindering van 75% van de infectie verwacht voor werknemers die in direct contact staan met COVID-patiënten.

ONDERZOEKSVRAAG: Kan de orofaryngeale spray van een virusblokkerende stof in combinatie met het verstrekken van een immunostimulerende emulsie het infectiepercentage met 2/3 verminderen
WERKINGSPRINCIPE Interventie 1. BlockACE2®-spray, antimicrobiële en antivirale vloeibare oplossing voor mondspray, samengesteld uit 3 natuurlijke bioactieve stoffen waarvan bewezen is dat ze bij mensen worden gebruikt en die moleculaire structuren vertonen die homoloog zijn aan de natuurlijke receptor van het virus. Het bevat bindende peptiden die lijken op de ACE2-receptor, die zich binden aan virale fracties, en fungeert als oplosbare wegvangers van het virus bij de ingangspoort, de bovenste luchtwegen of het orofaryngeale gebied, waardoor het de doelcel niet binnendringt en de hoeveelheid virus afneemt dat het lichaam binnenkomt; bijgevolg verminderen ze hun vermogen om schade te veroorzaken.
BlockACE2® Defense, voedsel voor orale toediening in de vorm van een emulsie met 8 bioactieve componenten die modulatie van de ontstekingsreactie hebben aangetoond. Bevat natuurlijke oliën, Vitamine B6, Vitamine C, Vitamine E, Vitamine D3 100 SD/S, en mineralen; belangrijke micronutriënten die normaal niet via een traditioneel dieet worden verkregen. Deze voedingsstoffen zijn immunostimulantia met bewezen capaciteit om de cellulaire respons tegen virussen, ontstekingsremmende, antioxiderende en chemoprotectieve activiteit te versterken, waardoor de productie van interferon I wordt gestimuleerd; en als geheel voorkomen ze het genereren van een overmatig activeringsproces van het immuunsysteem, bekend als een "immuunstorm", gemedieerd door pro-inflammatoire cytokines.
Interventie 2, Placebo:
Preparaten zonder actieve componenten in de farmaceutische vorm van het bovenstaande (Interventie 1 en Interventie 2). Ze zullen worden gepresenteerd met dezelfde fysieke kenmerken zoals grootte, vorm, kleur, smaak, geur, consistentie, gedrukte tekens, gewicht, oppervlakteafwerking, intern en extern; om het maskeringsprincipe te waarborgen.
ONDERZOEKS HYPOTHESE:
In een populatie met een gemeenschapsrisico op COVID-19-infectie, vermindert het gecombineerde gebruik van de onderzoeksinterventies (de orale antimicrobiële oplossing in Spray BlockACE2® en het voedsel in BlockACE2®-emulsie, vergeleken met het gebruik van Placebo, het risico op COVID-19 infectie uitgedrukt in een negatief PCR-resultaat bij degenen die seroconverteren naar IgM, en bij degenen die seroconverteren, zal het grootste deel de milde tot matige klinische vorm hebben.
PRIMAIRE DOELSTELLING Vaststellen van de werkzaamheid van het gecombineerde gebruik van BlockACE2® orofaryngeale spray en BlockACE2® emulsie, in vergelijking met placebo, op de vermindering van het risico op infectie door COVID-19, uitgedrukt in het negatieve resultaat van immunoglobulinen voor COVID-19 ( 30 dagen vanaf toelating tot het project).
Stel de veiligheid vast van de onderzochte producten door de verwachte en onverwachte bijwerkingen te evalueren die worden geregistreerd tijdens de gebruiksfase van het interventieschema (30 dagen vanaf de start van het project).
SECUNDAIRE DOELSTELLING Vaststellen van de werkzaamheid van het gecombineerde gebruik van BlockACE2®-spray, een oplosbare virusvangerverbinding plus inname van BlockACE2®-emulsie, een verbinding die de afweer van het immuunsysteem verbetert, in vergelijking met placebo, bij het verminderen van het risico op infectie door COVID -19 uitgedrukt in de klinische respons van de ziekte die wordt gedefinieerd in drie variabelen, mild tot matig: begin van koorts, myalgie en hoesten zonder kortademigheid; ernstig: koorts, myalgie, hoest en kortademigheid waarvoor ziekenhuiszorg nodig is; en kritiek: al het bovenstaande plus beademingshulp op de IC.
VERKENNENDE DOELSTELLINGEN De werkzaamheid onderzoeken van het gecombineerde gebruik van BlockACE2®-spray, een oplosbare virusvanger, plus de inname van BlockACE2®, een emulsievoeding met belangrijke micronutriënten die de antivirale cellulaire immuunrespons stimuleren bij het verminderen van het risico op COVID-19-infectie uitgedrukt in het negatieve resultaat van IgG, IgM; en de vermindering van het risico op symptomen zoals koorts, hoesten, kortademigheid, myalgie of beademingshulp op de IC na 15 dagen na beëindiging van het schema.
STUDIEGEBIED: Werknemers van elke afdeling van Conconcreto Company in de stad Medellín en Girardota
INTERVENTIES:
Interventie 1 BLOCKACE2: Twee orofaryngeale sprays van de verbinding, elke 4 uur, gedurende de dag. Voor de BlockACE2 emulsie wordt de eerste 5 dagen 20 ml gebruikt en daarna slechts 10 ml elke dag gedurende 30 dagen. Dosering van de stoffen in interventie 1 volgt de beschreven farmacologische kenmerken, die de permanentie ervan in het mondslijmvlies garanderen om de toegangsweg van het virus naar het organisme (Orofarynx) te beschermen. De dosering van de orale verbindingen werd bepaald volgens aanbevelingen van de WHO voor de modulatie van de immuunrespons (emulsie).
Interventie 2 PLACEBO: spray en emulsie met vergelijkbare kleur, geur en textuur maar zonder de natuurlijke actieve stoffen.
In het randomisatiebezoek V1 krijgt elke deelnemer een behandelschema toegewezen, bestaande uit interventie 1 en interventie 2, in voldoende hoeveelheid voor de duur van het onderzoek. De farmaceutische dienst zal de code van het schema verstrekken, volgens een volgorde, aangegeven in een toewijzingslijst van de schema's ter plaatse. Elk behandelingsschema is gelabeld met een unieke code die het identificeert, en het bevat één (1) apparaat voor orofaryngeale spray, met een capaciteit voor 200 toepassingen. Eén (1) fles met 360 ml (oraal). Bij de daaropvolgende bezoeken zal de naleving van de verstrekte schema's worden geverifieerd. De gebruiksfrequentie wordt gecontroleerd en het overslaan van doseringen wordt per frequentie geregistreerd (1 dosis, 1 keer per week, meer dan 1 keer per week).

Inclusiecriteria:
- Leeftijd tussen 18 en 60 jaar.
- Gezondheidswerker in de eerstelijnszorg van vermoedelijke of bevestigde gevallen van COVID-19.
- Negatieve IgM-antilichamen.
Uitsluitingscriteria:
- Deelnemen aan een andere studie met een ander medicijn.
- Temperatuur boven de 38ºC.
- De afgelopen 15 dagen samengewoond hebben met een persoon met de diagnose CoV-19.
- Zwangerschap.
- Actieve o verleden roker.
- Bekende overgevoeligheid voor medicijnen of voedsel.
- Geschiedenis van luchtwegaandoeningen.
- Onderliggende ziekten (hypertensie, kanker, diabetes, hart- en vaatziekten, leukopenie).
- Tekenen of symptomen van CoV-19-infectie: hoest, kortademigheid, spierpijn.

Resultaat van positieve IgM-antilichamen: Er zal een eerder gepland sluitingsbezoek worden gehouden, het proefproduct zal worden onderbroken als een geval wordt geclassificeerd als ernstig-kritiek, en de klinische follow-up zal worden voortgezet zoals gepland.
Zoals de protocollen van het ministerie van Volksgezondheid voorschrijven, zullen gevallen worden bevestigd met een PCR-RT-test voor de diagnose van COVID-19.
Het resultaat van de test zal worden opgevraagd om de uitkomst te documenteren.
