Deoxynucleosiden Pyrimidinen als behandeling voor mitochondriaal depletiesyndroom (dC-dT-MDS)
Een monocentrische, eenarmige fase II-studie om de veiligheid en werkzaamheid van de combinatie van deoxycytidine en deoxythymidine voor mitochondriale depletiestoornissen te beoordelen
Mitochondriaal DNA (mtDNA) depletiesyndromen (MDS) zijn een genetisch en klinisch heterogene groep van autosomaal recessieve aandoeningen die worden gekenmerkt door een ernstige vermindering van het mtDNA-gehalte, wat leidt tot verminderde energieproductie in aangetaste weefsels en organen. MDS is het gevolg van defecten in het onderhoud van mtDNA, veroorzaakt door mutaties in nucleaire genen die functioneren in beide mitochondriale nucleotidesynthese. MDS is fenotypisch heterogeen en wordt gewoonlijk geclassificeerd als myopathisch, encefalomyopathisch, hepatocerebraal of neurogastro-intestinaal.
Voor geen van deze aandoeningen is een effectieve therapie beschikbaar. Getroffen personen moeten een uitgebreide evaluatie ondergaan om de mate van betrokkenheid van verschillende systemen te beoordelen. De behandeling is voornamelijk gericht op het bieden van symptomatische behandeling. Geen behandeling voor MDS.
Klinische studies en in vitro/in vivo onderzoeksstudies toonden aan dat de verbetering van de bergingsroute door de beschikbaarheid van deoxyribonucleosiden die nodig zijn voor elk specifiek genetisch defect te vergroten, mtDNA-uitputting voorkomt.
Vroege herkenning en onmiddellijke therapie om de mitochondriale functie te herstellen, zou het klinische beloop mogelijk kunnen verbeteren.
Bevestiging van het voordeel van deoxynucleosiden als een veilige en potentieel effectieve therapie, zal leiden tot de beschikbaarheid van de eerste specifieke en effectieve behandeling voor mitochondriëndepletiestoornissen.
In deze fase II-studie zal een mix van deoxynucleosiden pyrimidine (deoxycytidine dC en deoxythymidine dT) worden gebruikt als vroege behandeling van MDS.
De gebruikte dosis is al gebruikt in andere klinische onderzoeken en lijkt effectief en wordt goed verdragen. De geïncludeerde proefpersonen zijn kinderen (0-18 jaar), met een positieve MDS-diagnose en die mutaties vertonen in een van de volgende genen: POLG, C10orf2, RRM2B, MPV17, SUCLA2, SUCLG1, FBXL4. Proefpersonen met MDS die neurologische fenotypes disfunctioneren.

Dit onderzoek is opgezet als een fase II, monocenter, open-labelonderzoek bij pediatrische patiënten.
Het doel is om de veiligheid, verdraagbaarheid en werkzaamheid van deoxycytidine en deoxythymidine bij de behandeling van kinderen met mitochondriale depletiestoornissen te evalueren.
Primaire doelstellingen Het primaire doel van deze studie is het evalueren van de werkzaamheid van dC/dT100-400 bij proefpersonen met mitochondriëndepletiestoornissen.
Secundaire doelstellingen De secundaire doelstellingen van deze studie zijn het evalueren van de verdraagbaarheid en veiligheid van dC/dT100-400 bij proefpersonen met mitochondriëndepletiestoornissen.
Eerste resultaat
Werkzaamheid van dC/dT100-400:
- Neurologische verbetering door elektro-encefalografie (EEG), aanvalsdagboek, ontwikkeling en kwaliteit van leven, klinische status waargenomen tijdens de neurologische follow-up.
- Verbeterde klinische status waargenomen tijdens de genetische follow-up en de Newcastle Pediatric Mitochondrial Disease Scale (NPMDS), formulieren die door genetici worden gebruikt om evaluatie van de progressie van mitochondriale ziekte bij patiënten jonger dan 18 jaar mogelijk te maken.
- Bloedonderzoek voor verschillende beoordelingen:
leverfunctie (aspartaataminotransferase (AST), alanineaminotransferase (ALT), gamma-glutamyltransferase (GGT), bilirubine en albumine.), nierfunctie (creatinine, ureum, elektrolyten). Beoordeel op myopathie met serumcreatinekinase (CK). Evaluatie van de mitochondriale functie met capillair/veneus bloedgas, serumlactaat, plasma-aminozuren, acylcarnitineprofiel, urine-aminozuren, urine-purines en pyrimidinezuren, en groeidifferentiatiefactor 15 (GDF15; een marker voor de ernst van mitochondriale disfunctie).
Secundaire uitkomst
- Veiligheid en verdraagbaarheid worden getest door bijwerkingen (AE) vast te leggen: AE wordt tijdens het onderzoek gecontroleerd en verzameld.
- Diarree: De gerapporteerde frequentie van diarree tijdens de behandeling zal het mogelijk maken om de verdraagbaarheid van dC/dT100-400 te bepalen.
- AE die leiden tot stopzetting van het studiegeneesmiddel, behandelingsgerelateerde bijwerkingen (TEAE's), SAE's (Severe Adverse Effect) zullen worden gerapporteerd vanaf de eerste dag dat de proefpersonen beginnen met het innemen van medicatie tot de laatste ingenomen dosis.

- Pathogene variant(en) in een van de volgende genen: POLG, C10orf2, RRM2B, MPV17, SUCLA2, SUCLG1, FBXL4

Experimenteel: dC/dT100-400 arm
Kinderen en volwassenen (0-60 jaar), die het onderzoeksproduct deoxynucleosiden pyrimidine (mix van deoxycytidine en deoxythymidine) gebruiken volgens het protocol.

Het onderzoeksproduct (IP) dC/dT100-400 wordt elke dag (QD) oraal toegediend en de dosis wordt verdeeld over 3 innames/dag voor de dagelijkse dosis van 100 mg/kg van dag 1-7, 200 mg/kg van Dag 8-14, 300 mg/kg van dag 15-21 en 400 mg/kg van dag 22 tot 730.
Doses werden gekozen op basis van de veiligheids- en werkzaamheidsdoses die in de literatuur worden gebruikt.

Beschrijving IPD-plan
REDCap-software zal worden gebruikt als IPD. De gegevens worden anoniem gedeeld, de betrokkene wordt geïdentificeerd door middel van een Identifiant (ID).
REDCap wordt beheerd door teams van kwaliteitsgegevens van onderzoeksinstituut van McGill University Health Center (RI-MUHC).
Voor klinische onderzoeksrapporten, aangezien de studie is gepland in het Children Hospital of Montreal, zal de toegang tot het Clinical Study Report (CSR) worden gedaan via het Open Architecture Clinical Information System (Oacis)-tool van het ziekenhuis.
