Symbiotisch gebruik bij patiënten na een bariatrische operatie
Bieden symbiotica nuttige voordelen bij gastro-intestinale problemen bij patiënten die een afslankoperatie ondergaan?

Het onderwerp van interesse is het darmmicrobioom en de impact van symbiotica (prebiotica in combinatie met probiotica) op de darmgezondheid na gewichtsverliesoperaties als gevolg van chirurgische veranderingen van de maag en darmen als gevolg van de procedures. Symbiotica bieden positieve voordelen voor de darmgezondheid, dwz. verbetering van de darmfunctie en vermindering van gastro-intestinale klachten zoals een opgeblazen gevoel. Bij het uitvoeren van een literatuuronderzoek naar probiotica/prebiotica zijn de meeste onderzoeken echter van slechte kwaliteit en wordt niet de hoeveelheid van elke onderzochte stam genoemd. Tot op heden is er geen onderzoek uitgevoerd naar symbiotische toediening bij patiënten na een bariatrische operatie en hun relatie met betrekking tot verbeteringen in gastro-intestinale symptomen.
Er wordt geen standaarddosis aanbevolen voor probiotica. Elke stam moet worden toegediend in een dosis waarvan uit klinische onderzoeken is gebleken dat deze effectief is in het bieden van gezondheidsvoordelen voor de gastheer. Er zijn echter geen duidelijke criteria vastgesteld om te bepalen wat als effectief wordt geclassificeerd en welke gezondheidsvoordelen als klinisch relevant worden beschouwd. De meeste gezondheidszorgorganisaties adviseren tussen 1 en 20 x 109 kolonievormende eenheden (CFU)/dag voor volwassenen. In veel gevallen is aangetoond dat hogere doseringen effectiever zijn, maar er is nog geen duidelijke dosisafhankelijke respons vastgesteld en de ‘meer is beter’-filosofie is niet op alle soorten van toepassing. Daarom luidt één regel: "Als een product meerdere stammen bevat, moet elke stam aanwezig zijn in niveaus van 109 om de effectiviteit te garanderen."
Het primaire doel van deze studie zou zijn om de werkzaamheid van symbiotische toediening te bepalen voor het verbeteren van de gastro-intestinale gezondheid bij patiënten die een afslankoperatie ondergaan. Deze studie zou de werkzaamheid testen van een eenmaal daags symbioticum met meerdere stammen op veranderingen in de darmgezondheid bij patiënten die een afslankoperatie ondergaan, twaalf maanden of langer na de operatie, door ontlastingsmonsters te testen vóór toediening en vervolgens drie maanden na toediening van het symbioticum om te controleren eventuele veranderingen in bacteriën in de ontlastingsmonsters. Deelnemers aan de studie zullen ook een enquête invullen waarin hun stoelgang, consistentie van de ontlasting en gastro-intestinale symptomen vóór en na symbiotische toediening worden geëvalueerd.

Inclusiecriteria:
- 12 maanden of langer post-malabsorptieve gewichtsverliesprocedure
Uitsluitingscriteria:
- LapBand- of intra-maagballonpatiënten vanwege het ontbreken van een chirurgische ingreep aan de darmen
- Personen die geen goedgekeurde afslankprocedure hebben gehad
- Personen met een verminderde gastro-intestinale functie die de inname van een oraal supplement zou belemmeren
- Personen met een verminderde gastro-intestinale functie die het vermogen om ontlastingsmonsters te verzamelen zou belemmeren
- Onvermogen om beoordelingsinstrumenten te begrijpen en in te vullen
- Onvermogen om zich te houden aan de toediening van 90 dagen capsulesupplement
- Neem momenteel dagelijks een probiotisch, prebiotisch of symbiotisch supplement
- Momenteel onder behandeling met antibiotica van de arts

Experimenteel: Symbiotische groep
Een vrij verkrijgbare, eenmaal daagse symbiotische capsule, inclusief: 15 miljard CFU probiotisch mengsel en 250 mg prebioticum.

Eén dagelijkse toediening van een capsulesupplement met daarin: 15 miljard CFU probiotisch mengsel (Lactobacillus acidophilus La-14, Bifidobacterium longum BB536, Lactobacillus acidophilus NCFM, Lactobacillus rhamnosus GG, Lactobacillus gasseri Lg-36) en 250 mg cichoreiwortelinuline.

Symbiotische veranderingen in ontlastingsmonsters

Het primaire doel zal zijn om de werkzaamheid van een eenmaal daags, multi-stammen symbioticum op veranderingen in de darmgezondheid bij patiënten met gewichtsverliesoperaties te testen door ontlastingsmonsters te testen vóór toediening en vervolgens drie maanden na toediening van een symbioticum of placebo.
Er zullen chi-kwadraattesten worden uitgevoerd om verbanden aan te tonen tussen symbiotische inname en de diversiteit en concentraties van bacteriën in de ontlasting.

Wekelijkse veranderingen op de Bristol-ontlastingsschaal

|
Deelnemers vullen wekelijks een Bristol-krukschaal in die de algehele consistentie van hun ontlasting voor de week identificeert. Deelnemers vullen deze tool wekelijks in en SPSS-software wordt gebruikt om een onafhankelijke t-test van gemiddelde veranderingen uit te voeren om het verschil in ontlastingvorm te evalueren.
De totale scores kunnen variëren van 7 tot en met 49, waarbij scores die het dichtst bij 24,5 liggen, de normale vorm van de ontlasting vertegenwoordigen.
De vergelijkingen van deze tests zullen statistische relevantie kunnen opleveren voor het gebruik van symbiotische suppletie bij normale ontlasting.

Deelnemers vullen een wekelijkse enquête in die de hoeveelheid ontlasting identificeert die de week ervoor is geproduceerd.
Er zal een onafhankelijke t-test worden uitgevoerd om veranderingen in de ontlastingsfrequentie aan te tonen.

Er zal een laatste Chi-kwadraat-associatietest worden uitgevoerd met behulp van SPSS-software om te testen of er een verband bestaat tussen symbiotische inname en vermindering van GI-gerelateerde symptomen.
P-waarden van 0,05 of lager zullen worden gebruikt om de statistische significantie voor alle uitgevoerde tests aan te tonen.
