Leverkanker en immunotherapie: klinische relevantie van LIquid BioPSY
Het doel van deze prospectieve klinische studie is het identificeren van een voorspellende biomarker bij patiënten met gevorderde HCC (stadium B en C) met behulp van een combinatorische benadering van de vloeibare biopsie.
De belangrijkste vragen die het wil beantwoorden zijn:
- Is multi-omische vloeibare biopsiebenadering in staat om een sterk voorspellende biomarker van immunotherapie-efficiëntie te identificeren?
- Is er een correlatie tussen weefselbiopsie (PD-L1 weefselexpressieniveau) en vloeibare biopsie (detectie van CTC die PD-L1 tot expressie brengt) bij HCC-patiënten?
Het bloed van de deelnemers wordt op verschillende tijdstippen afgenomen.

Bij solide kankers maken enkele meer agressieve tumorcellen actief los van de primaire laesie en reizen vervolgens door het circulerende compartiment om verre organen te bereiken en micro-metastasen te vormen. Detectie van CTC's in het bloed is ook relevant voor het beoordelen van tumorprogressie, prognose en therapeutische follow-up. De niet-invasieve, zeer gevoelige analyse voor CTC's wordt "vloeibare biopsie" genoemd. De afgelopen jaren is een multi-anlaytes-benadering (CTC's, circulerend tumor-DNA, extracellulaire blaasjes, miRNA...) van vloeibare biopsie ontwikkeld.
Hepatocellulair carcinoom (HCC) is het overheersende pathologische type van primaire leverkanker. Het vertegenwoordigt de zesde meest voorkomende incidentie wereldwijd en de derde meest voorkomende doodsoorzaak door kanker.
Sinds 2021 is de gouden standaardbehandeling voor patiënten met gevorderde en/of inoperabele HCC de combinatie van atezolizumab (anti-PD-L1) en bevacizumab (VEGF-remmer) in gevallen waarbij chemo-embolisatie niet geïndiceerd is (patiënten met lymfeklierinvasie en/of laesies op afstand of patiënten met portale flowafwijkingen). Deze therapie biedt inderdaad een significant voordeel in de totale overleving (19,2 versus 13,4 maanden, HR 0,66, p<0,0009) en ook in de progressievrije overleving (6,9 versus 4,3 maanden, HR 0,65, p=0,0001). Tot op heden is er echter geen voorspellende biomarker voor de werkzaamheid van immuuncontrolepuntremmers (ICI)
Het doel van dit onderzoeksproject is het identificeren van een voorspellende biomarker bij patiënten met gevorderde HCC (stadium B en C) met behulp van een combinatorische benadering van de vloeibare biopsie (CTC, CTC die PD-L1 tot expressie brengt, immuuncelprofilering).

Beschrijving
Inclusiecriteria:
- Patiënten van ten minste 18 jaar oud,
- Patiënten met gevorderd hepatocellulair carcinoom of HCC met indicatie voor eerstelijns PD-1- of PD-L1-immunotherapie bij MDT, zonder voorafgaande systemische therapie,
- De diagnose HCC wordt gesteld op basis van beeldvormingscriteria (LI-RADSv2018-criteria) of na histologisch bewijs,
- Geavanceerde HCC gedefinieerd door BCLC-fasen B en C,
- Patiënten met mondelinge toestemming.
Uitsluitingscriteria:
- Toediening van een eerdere systemische antitumorbehandeling (immunotherapie of chemotherapie of gerichte therapie)
- Geen persoonlijke voorgeschiedenis van neoplasie in de afgelopen 5 jaar
- Geen persoonlijke geschiedenis van systemische ontstekingsziekten
- Geen immunosuppressieve behandeling of behandeling die de immuniteit kan wijzigen (anti-TNF...)
- Geen aansluiting bij of niet-begunstigde van een socialezekerheidsstelsel;
- Kwetsbare personen volgens artikel L1121-6 van de CSP;
- Meerderjarige personen die beschermd zijn of niet in staat zijn hun toestemming te geven volgens artikel L1121-8 van de CSP;
- Zwangere vrouwen of vrouwen die borstvoeding geven volgens artikel L1121-5 van de CSP.
- Niet-inclusie wegens opvolgingsproblemen (overplaatsing, onvoldoende motivatie, slechte therapietrouw, prioritaire pathologie in de zorg, enz.)

Voor elke deelnemer wordt 30 ml bloed afgenomen bij opname/voor aanvang van de behandeling (baseline) en tijdens de standaardzorg follow-up. Het bloed wordt, in overleg of op de polikliniek, afgenomen tijdens een bloedonderzoek voor gezondheidsdoeleinden. |
