Bij overlevenden van kanker is de incidentie van slaapstoornissen en slapeloosheid hoger dan bij de algemene bevolking, met aanzienlijke potentiële implicaties voor de gezondheidsresultaten, ook op het gebied van overleving en sterftecijfers. De beschikbare farmacologische opties voor de behandeling van slaapstoornissen zijn beperkt wat betreft werkzaamheid en bijwerkingen. Onderzoekers stellen een dubbelblinde, placebogecontroleerde studie voor waarin het gebruik van Suvorexant bij overlevenden van borstkanker op selectieve oestrogeenreceptormodulatoren of aromataseremmers met slaapstoornissen wordt geëvalueerd om de impact op de symptomen van slapeloosheid en de kwaliteit van leven te beoordelen. Bij de evaluatie van overlevenden van borstkanker die endocriene therapie krijgen, wordt gebruik gemaakt van een onderzoekspopulatie met een hoog vijfjaarsoverlevingspercentage, terwijl de capaciteit voor rekrutering behouden blijft en een mogelijkheid wordt gecreëerd om potentiële effecten op vasomotorische symptomen te onderzoeken. Suvorexant heeft het potentieel om bij overlevenden van kanker een aanzienlijke impact te hebben, niet alleen op de slaapgerelateerde uitkomsten, maar ook op de overlevingsresultaten van kanker.
Het doel van het onderzoek is om het vermogen van Suvorexant, een medicijn dat door de Food and Drug Administration (FDA) is goedgekeurd voor slapeloosheid, te evalueren om slaapstoornissen bij overlevenden van borstkanker te behandelen met endocriene therapie.

- Volwassen
- Oudere volwassene

Beschrijving
Inclusiecriteria:
- Volwassen overlevenden van borstkanker na de menopauze (vrouw, > 18 jaar oud)
- huidig selectief gebruik van oestrogeenmodulatoren of aromataseremmers
- ten minste 6 weken na voltooiing van de definitieve behandeling voor borstkanker
- minder dan 5 jaar na diagnose.
Uitsluitingscriteria:
- levensverwachting van minder dan 6 maanden
- huidig gebruik van steroïden
- ernstige depressie of angst
- ernstige leverfunctiestoornis
- gelijktijdig gebruik van matige of sterke CYP3A-remmers
- huidige ontvangst van hospicezorg
- ernstige psychische aandoening
- huidig gebruik van meer dan 40 milligram morfine-equivalenten per dag
- diagnose van obstructieve slaapapneu, narcolepsie of een andere slaapgerelateerde ziekte anders dan slapeloosheid
- zwangerschap
- behandeling met alternatieve farmacotherapie voor slapeloosheid op het moment dat het onderzoek wordt gestart
- menstruatiecyclus in het afgelopen jaar

Experimenteel: Suvorexant met informatie over slaaphygiëne
Deelnemers die naar deze arm zijn gerandomiseerd, krijgen dagelijks 10 mg suvorexant, met een dosisverhoging naar 20 mg na dag 7. De medicatie wordt oraal toegediend.
Totale duur van 28 dagen.

Participants randomized to this arm will receive placebo control daily with a switch to an alternate placebo control vial after 7 days, consistent with intervention arms.
Medication will be administered as an oral medication.

The outcome reported was the number of participants who were adherent based on this evaluation.
