Hemodynamische effecten van standaard cardiopulmonale reanimatie (CPR), actieve compressie-decompressie-reanimatie met een inspiratoir impedantie-apparaat en standaard-reanimatie met een intrathoracale drukregelaar tijdens een hartstilstand buiten het ziekenhuis

Meer dan 300.000 Amerikanen ervaren jaarlijks een hartstilstand buiten het ziekenhuis, met een gemiddelde overlevingskans van minder dan 5%. Lage overlevingspercentages blijven bestaan, deels omdat handmatige borstcompressies en beademing, standaard cardiopulmonale reanimatie (S-CPR) genoemd, een inherent inefficiënt proces is, dat minder dan 25% van de normale bloedstroom naar het hart en de hersenen levert. De hemodynamica wordt vaak verder aangetast door slechte S-CPR-technieken, met name onvoldoende borstcompressie en onvolledige borstterugslag. Actieve compressie Decompressie CPR (ACD-CPR) wordt uitgevoerd met een handapparaat dat op de borst van de patiënt is bevestigd en bevat ook een handvat met een metronoom en krachtmeter om de juiste compressiesnelheid, diepte en volledige terugslag van de borstwand te bepalen. Het impedantiedrempelapparaat (ITD) is ontworpen voor snelle aansluiting op een luchtwegadjunct (bijv. gezichtsmasker of endotracheale tube) en maakt overdrukventilatie mogelijk, terwijl ook passieve inspiratoire gasuitwisseling tijdens decompressie van de borstwand wordt belemmerd. Eerdere studies hebben aangetoond dat de combinatie van ACD-CPR + ITD het opnieuw vullen van het hart na elke compressie verbetert door de negatieve intrathoracale druk tijdens de decompressiefase van CPR te verhogen, wat resulteert in een verbeterde cardiale en cerebrale perfusie. De intrathoracale drukregelaar (ITPR) is een nieuwe generatie inspiratoire impedantietherapie. De ITPR gebruikt een gereguleerde externe vacuümbron om de negatieve intrathoracale druk te verlagen en is daarom minder afhankelijk van de kwaliteit van de reanimatie (bijv. volledigheid van de borstwandterugslag). De ITPR genereert een vooraf ingestelde continue en gecontroleerde negatieve intrathoracale druk in de expiratoire fase die alleen wordt onderbroken wanneer beademing met positieve druk nodig is om de oxygenatie in stand te houden en voor gasuitwisseling te zorgen.
Het doel van de studie is om de vroege veiligheids- en hemodynamische effecten van S-CPR, ACD-CPR + ITD en S-CPR + ITPR te vergelijken bij patiënten met een hartstilstand buiten het ziekenhuis.

Beschrijving
Initiële opnamecriteria:
- Van de proefpersoon wordt in eerste instantie aangenomen of bekend dat hij 18 jaar of ouder is;
- Betrokkene presenteert zich met vermoedelijk niet-traumatische hartstilstand buiten het ziekenhuis EN komt in aanmerking voor een reanimatiepoging. [OPMERKING: de hartstilstand kan getuige OF onopgemerkt zijn];
- Proefpersoon heeft een beveiligde geavanceerde luchtweg met manchet [bijv. endotracheale tube, Combitube, King-luchtweg].
Definitieve opnamecriteria:
- Proefpersoon bij wie een femorale arteriële toegang met succes tot stand is gebracht;
- Proefpersoon bleef in hartstilstand (onderging reanimatie) op het moment van hemodynamische data-acquisitie;
- Proefpersoon bij wie ten minste 5 minuten aan continue hemodynamische gegevens konden worden verzameld, OF als ROSC vóór 5 minuten optreedt, ten minste 2 minuten aan hemodynamische gegevens konden worden verzameld.
Initiële uitsluitingscriteria:
- De proefpersoon kende reeds bestaande DNR-bevelen voorafgaand aan deze hartstilstand;
- Proefpersoon heeft tekenen van duidelijk klinisch overlijden of aandoeningen die het gebruik van reanimatie uitsluiten;
- De familie of wettelijke voogden van de proefpersoon verzoeken dat de proefpersoon niet in het onderzoek wordt opgenomen op het moment van arrestatie;
- De patiënt heeft een recente sternotomie ondergaan, waarbij de wond niet volledig lijkt te zijn genezen (als de datum van de sternotomie onbekend is) of minder dan zes maanden (als de datum van de sternotomie bekend is);
- Proefpersoon heeft voorafgaand aan arrestatie een stoma, tracheotomie of tracheotomie;
- Het is bekend of het vermoeden bestaat dat de proefpersoon zwanger is;
- Onderwerp is bekend/vermoedelijk een gevangene.
Definitieve uitsluitingscriteria:
- Proefpersoon bij wie < 2 minuten hemodynamische gegevens werden verkregen tijdens het ontvangen van reanimatie;
- Proefpersoon bij wie geen arteriële drukkatheter was geplaatst of bij wie de arteriële druk niet met succes kon worden gecontroleerd;
- Onderwerp blijkt vervolgens een traumatische arrestatie te hebben gehad;
- Proefpersoon was in asystolie op het moment van eerste arrestatie EN bleef in asystolie tijdens reanimatiepoging EN arrestatie was niet getuige of onbekend indien getuige.
