Patient Reported Outcomes Measures (PROM) bij carpaletunneltherapieën bij patiënten met erfelijke neuropathieën

Er zal een online-enquête worden gebruikt om de resultaten te beoordelen van conservatieve en chirurgische therapieën die worden gebruikt voor carpaaltunnelsyndroom (CTS) bij patiënten met een diagnose van erfelijke neuropathie zoals CMT of HNPP (ziekte van Charcot-Marie-Tooth en erfelijke neuropathie met aansprakelijkheid voor drukverlamming). respectievelijk). HNPP is een erfelijke neuropathie die allelisch is voor CMT1A. Deze door de patiënt gerapporteerde uitkomstmeting zal ook de ernst van de symptomen en de functionele status met betrekking tot het carpaletunnelsyndroom (CTS) beoordelen bij verschillende soorten patiënten met erfelijke neuropathie.
Het onderzoek bevat twee gevalideerde vragenlijsten voor de evaluatie van de resultaten met betrekking tot carpaal tunnel syndroom. De Boston Carpal Tunnel Questionnaire (BCTQ) is zeer specifiek voor carpaal tunnel syndroom (CTS). De DASH-vragenlijst (Disability of Arm, Shoulder and Hand) is minder specifiek voor CTS, maar er is aangetoond dat deze even valide is bij de beoordeling van uitkomsten bij CTS. De relatieve niet-specificiteit van DASH voor CTS kan nuttig zijn bij het beoordelen van de mogelijke bijdrage van CMT aan de algehele ernst van de symptomen van de bovenste extremiteit en het functionele resultaat. Gegevens die zijn verzameld van deelnemers met een diagnose of therapieën voor CTS kunnen worden vergeleken met gegevens die zijn verzameld van deelnemers zonder diagnose of therapieën voor CTS.
Bovendien zal de studie componenten van de CMTNS bevatten om de omvang van de CMT-ziekte bij de deelnemers te beoordelen.
Er wordt voorgesteld een retrospectieve analyse uit te voeren van patiënten met CMT die symptomen in de bovenste ledematen hebben die wijzen op carpaaltunnelsyndroom. Twee gevalideerde vragenlijsten die veranderingen in symptomen en functionele status onderzoeken (BCTQ en DASH) kunnen worden gebruikt bij het evalueren van de resultaten van patiënten met gerapporteerde symptomen of de diagnose carpaal tunnelsyndroom.
De Boston Carpal Tunnel Questionnaire (BCTQ) is een schaal met 19 items die speciaal is ontwikkeld voor het carpaletunnelsyndroom en bestaat uit 11 vragen om de ernst van de symptomen te beoordelen en 8 vragen om de functionele status te beoordelen. BCTQ heeft test-hertestbetrouwbaarheid vastgesteld met een goede Pearson's correlatie van scores bij hertest (0,91 voor de component ernst van de symptomen en 0,93 voor de component functionele status van de test). Er is ook een hoge interne consistentie gerapporteerd voor de BCTQ (0,88-9,92). Convergente validiteit is aangetoond met een hoge correlatie tussen pre- en postoperatieve BCTQ-scores en fysieke metingen zoals grip, key pinch en three-jaw pinch. Responsiviteit van de BCTQ na carpaletunnelchirurgie is aangetoond in gerandomiseerde klinische trials. Er is ook gemeld dat BCTQ 2-4 keer beter reageert op klinische verbetering in vergelijking met metingen van neuromusculaire stoornissen. Hoge responspercentages zijn gemeld met BCTQ met 80-93% na 6-12 weken follow-up en tussen 67-93% na 6-12 maanden follow-up na het loslaten van de carpale tunnel. De responspercentages bleven hoog met 85% na 2 jaar follow-up. Gemaild follow-up responspercentage van 69% is gemeld met een gemiddelde follow-up van 4,8 jaar. De gemiddelde tijd om BCTQ te voltooien is naar verluidt 5,6 minuten. Er zijn geen gegevens om de precisie van BCTQ in relatie tot carpaaltunnelsyndroom aan te geven.
De DASH-vragenlijst (Disability of Arm, Shoulder and Hand) is een schaal met 30 items die de symptomen en het fysieke functioneren beoordeelt. Deze vragenlijst is niet specifiek voor de carpale tunnel en kan worden gebruikt voor verschillende aandoeningen van de bovenste ledematen. DASH is bedoeld om handicaps te meten door de nadruk te leggen op activiteit van de bovenste ledematen. De test-hertestbetrouwbaarheid van DASH is aangetoond met een Pearson's 8-correlatie van 0,9 in een groep patiënten met carpaaltunnelsyndroom.
De validiteit van DASH werd aangetoond door correlatie met BCTQ. Pre- en postoperatieve DASH-scores correleren ook met fysieke metingen zoals grip, key pinch en three-jaw pinch. Responsiviteit door verbeterde DASH-scores is aangetoond na drie en zes maanden na de operatie. BCTQ bleek beter te reageren op DASH voor carpaal tunnel syndroom. De responspercentages per post zijn 80-93% na 6-12 weken en 66-98% na 6 maanden na het loslaten van de carpale tunnel. De gemiddelde tijd om DASH te voltooien is naar verluidt 6,8 minuten. Er zijn geen gegevens die de nauwkeurigheid van DASH met betrekking tot het carpaletunnelsyndroom aangeven.
De onderzoeksgegevens worden opgeslagen door het Data Management and Coordinating Center (DMCC) van het Rare Diseases Clinical Research Network aan de Universiteit van Zuid-Florida. Namen of andere persoonlijke gezondheidsinformatie worden niet verzameld. Na afloop van de onderzoeksperiode zullen de gegevens worden verzonden naar Dr. Michael Shy, hoofdonderzoeker van het Inherited Neuropathies Consortium. Geanonimiseerde gegevens worden naar de database van Genotypes en Fenotypes (dbGaP) gestuurd.

Inclusiecriteria:
- Diagnose van CMT of HNPP
- Volwassenen van 18 jaar en ouder
- Zelfregistratie bij het contactregister van het Inherited Neuropathies Consortium (INC).
Uitsluitingscriteria:
- Onvermogen om geïnformeerde toestemming te geven en enquête in te vullen
- Onvermogen om Engels te lezen of te spreken

Dit onderzoeksprotocol bestaat uit één anonieme enquête.
Elke deelnemer kan slechts één enquête invullen.
Het invullen van de enquête duurt ongeveer 25 minuten.
Het onderzoek is ontworpen om items te identificeren die de ernst van de symptomen en de functionele status van patiënten met CTS en CMT het beste weergeven.
