Beschrijving
Inclusiecriteria:
- HIV-1-infectie, zoals gedocumenteerd door een goedgekeurde snelle testkit en bevestigd door Western blot of ELISA-testkit op elk moment voorafgaand aan de inschrijving voor het onderzoek.
HIV-1-RNA in plasma is aanvaardbaar als alternatieve bevestigende test.
- Mannen en vrouwen leeftijd > 18 jaar.
- U hebt geen antiretrovirale behandeling gekregen voordat u aan het onderzoek begon.
- Patiënten die een enkelvoudige dosis nevirapine of AZT voor PMTC van enige duur kregen, worden niet als ARV-naïef beschouwd.
- Aantal CD4+-cellen van </=100 cellen/mm3 verkregen binnen 90 dagen voorafgaand aan het begin van de studie.
- HIV RNA-niveau > 1.000 kopieën/ml verkregen binnen 90 dagen voorafgaand aan deelname aan het onderzoek.
- Patiënten met een opportunistische of HIV-gerelateerde infectie kunnen worden opgenomen volgens het klinische oordeel van de hoofdonderzoeker in elk centrum wanneer de patiënt klaar en in staat is om ARV-therapie te starten.
Laboratoriumwaarden verkregen binnen 30 dagen voorafgaand aan het begin van het onderzoek:
- Absoluut aantal neutrofielen (ANC) > 500/mm3.
- Hemoglobine > 8,0 g/dl.
- Aantal bloedplaatjes > 50.000/mm3.
- AST (SGOT), ALT (SGPT) en alkalische fosfatase ondergeschikt aan 5 keer ULN.
- Totaal bilirubine klein van 2,5 keer ULN.
- Kleinere creatinineklaring van 50* ml/min zoals geschat door de Cockcroft-Gault-vergelijking of creatinineklaring > 50 ml/min zoals berekend door een formele meting van de creatinineklaring
- Alle vruchtbare vrouwen (die de menopauze niet hebben bereikt of hysterectomie, ovariëctomie of afbinden van de eileiders hebben ondergaan) moeten een negatieve serum- of urine-b-HCG-zwangerschapstest ondergaan die binnen 7 dagen vóór deelname aan het onderzoek wordt uitgevoerd.
- Vrouwelijke proefpersonen die niet in staat zijn om zich voort te planten (de menopauze hebben bereikt of een hysterectomie, ovariëctomie of afbinden van de eileiders hebben ondergaan) of van wie de mannelijke partner een succesvolle vasectomie heeft ondergaan met als resultaat azoöspermie of azoöspermie heeft om een andere reden, komen in aanmerking zonder het gebruik van anticonceptie. Documentatie van de menopauze, sterilisatie (hysterectomie, ovariëctomie, afbinden van de eileiders of vasectomie) en azoöspermie door middel van een door de patiënt gerapporteerde geschiedenis is acceptabel.
- Alle proefpersonen moeten ermee instemmen niet deel te nemen aan een conceptieproces (d.w.z. actieve poging om zwanger te worden of zwanger te worden, spermadonatie, in-vitrofertilisatie), en als ze deelnemen aan seksuele activiteit die tot zwangerschap zou kunnen leiden, moet de vrouwelijke onderzoeksvrijwilliger/mannelijke partner moet akkoord gaan met het gebruik van een vorm van anticonceptie zoals gespecificeerd in de onderstaande opmerking tijdens het gebruik van in het protocol gespecificeerde medicatie(s) en gedurende één maand na het stoppen van de medicatie(s).
- Vermogen en bereidheid van de proefpersoon of wettelijke voogd/vertegenwoordiger om schriftelijke geïnformeerde toestemming te geven.
Uitsluitingscriteria:
- Zwangerschap en borstvoeding.
- Actieve neoplasie of voorgeschiedenis van neoplasie. (Behalve gelokaliseerd niet-visceraal Kaposi-sarcoom; gelokaliseerd plaveiselcel- of basaalcelcarcinoom van de huid, of intra-epitheliale cervicale neoplasie graad III of lager).
- Gebruik van de volgende geneesmiddelen binnen 180 dagen voorafgaand aan deelname aan het onderzoek: systemische kankerchemotherapie, systemische onderzoeksmiddelen en immunomodulatoren (groeifactoren, immunoglobuline, interleukinen, interferonen).
- Gebruik van systemische corticosteroïden in de laatste 2 weken voorafgaand aan randomisatie.
- Gedecompenseerde leverziekte (gedefinieerd als stadium C van de Child-Pugh-classificatie) aan het begin van het onderzoek.
- Een veranderde mentale toestand die naar de mening van de onderzoeker de naleving van het protocol in gevaar brengt.
- Allergie/gevoeligheid voor onderzoeksgeneesmiddel(en) of hun formuleringen die niet kunnen worden vervangen door een ander middel zoals beschreven in rubriek 5.1
- Actief drugs- of alcoholgebruik of afhankelijkheid die, naar de mening van de onderzoeker, het naleven van de studievereisten zou verstoren.
- Ernstige ziekte waardoor een proefpersoon niet in staat is om het antiretrovirale studieregime te nemen.
- Ernstige medische aandoening die naar de mening van de onderzoeker de naleving en/of opvolging van het protocol in het gedrang brengt.
