Anakinra is een volledig menselijke IL-1ra (interleukine-1-receptoragonist) die in 2001 door de FDA is goedgekeurd voor de behandeling van reumatoïde artritis. Het bindt competitief aan de IL-1-receptor en blokkeert zo de IL-1-signalering. Het is een kortwerkend middel dat dagelijkse subcutane toediening vereist van 1-2 mg/kg, maximaal 100 mg/dosis. Het is effectief geweest in het verlagen van HbA1c (geglyceerd hemoglobine) bij T2D (diabetes type 2) en een gerandomiseerde studie van anakinra bij recent ontstane T1D (diabetes type 1) is aan de gang in Europa. Over het algemeen wordt anakinra al meer dan 10 jaar gebruikt bij volwassenen en kinderen met een goede staat van dienst op het gebied van veiligheid. Zelden voorkomende bijwerkingen zijn infecties, neutropenie, misselijkheid, diarree, hartstilstand, griepachtige symptomen en productie van anti-anakinra-antistoffen.
Onderzoeksopzet: een dubbelblinde, placebogecontroleerde RCT (gerandomiseerde gecontroleerde studie) met 2:1 toewijzing (14 actieve behandelingen vs. 7 placebo). De behandeling met Anakinra zal worden gegeven als een bolus van 2 mg/kg via intraveneuze infusie gedurende 30 minuten, gevolgd door een infusie van 2 mg/kg/uur gedurende 4 uur onmiddellijk na bevestiging van de diagnose DKA (diabetische ketoacidose) en wanneer de laboratoriumveiligheidsparameters beschikbaar zijn (CBC (volledig bloedbeeld) en zwangerschapstest) en nadat toestemming is verkregen. Primaire uitkomsten: veiligheid en verdraagbaarheid van anti-IL-1-behandeling (anakinra) gedurende de eerste 24 uur van DKA-behandeling. Secundaire uitkomsten: diameter van de oogzenuwschede (afsnijding om hersenoedeem te definiëren: 4,5 mm); Veranderingen in cytokineniveaus tijdens de behandeling met anakinra.

Beschrijving
Inclusiecriteria:
- Leeftijd 8-18 jaar bij diagnose diabetes type 1
Diabetische ketoacidose met:
- plasmaglucoseconcentratie >300 mg/dl,
- veneuze pH <7,30 of
- serumbicarbonaatconcentratie <15 mmol/L, en
- ketonen in urine of serum
Hematologie:
- WBC >3000 x 109/L;
- bloedplaatjes >100.000 x 109/L;
- hemoglobine >10,0 g/dl
- Negatieve bloedzwangerschapstest bij vrouwen.
Uitsluitingscriteria:
Kinderen met onderliggende aandoeningen, waaronder:
- actieve auto-immuunziekte of immuundeficiëntie anders dan diabetes type 1,
- maligniteit,
- orgaan transplantatie,
- elke aandoening die chronisch gebruik van corticosteroïden vereist
- Eerdere immunotherapie om diabetes type 1 te voorkomen
- Huidige of eerdere infectie met HIV, hepatitis B of hepatitis C beoordeeld op basis van de geschiedenis
- Patiënten die zich presenteren met DKA samen met alcohol- of drugsgebruik,
- Hoofd trauma,
- Meningitis of andere aandoeningen die de neurologische functie kunnen beïnvloeden
- Nierfalen
- Elke aandoening, al dan niet medisch, die, naar de mening van de onderzoeker, volledige deelname aan het onderzoek zou verhinderen, of die een aanzienlijk gevaar zou vormen voor de deelname van de proefpersoon
Patiënten met een voorgeschiedenis van bekende overgevoeligheid voor:
- E coli-afgeleide eiwitten,
- anakinra, of
- alle componenten van het onderzoeksgeneesmiddel
