Dit is een prospectieve, gerandomiseerde, gecontroleerde klinische studie van standaard versus verbeterde hydratatie bij patiënten met de diagnose acute pancreatitis binnen het eerste uur na diagnose.
Patiënten die op de Spoedeisende Hulp worden opgenomen met de klinische verdenking van acute pancreatitis worden behandeld via anamnese, lichamelijk onderzoek en laboratoriumuitslagen. De diagnose acute pancreatitis wordt gesteld door de aanwezigheid van 2 van de 3 volgende criteria: typische buikpijn, serumamylase en/of lipase groter dan driemaal de bovengrens van normaal, en kenmerkende bevindingen van abdominale beeldvorming. Patiënten worden geïnformeerd over het onderzoeksprotocol en de mogelijke risico's en bijwerkingen, waarna hen wordt gevraagd deelname aan het onderzoek te accepteren. Aanvaarding van deelname wordt bevestigd door ondertekende geïnformeerde toestemming.
Na acceptatie worden patiënten gerandomiseerd door computergegenereerde reeksen van willekeurige getallen in de twee groepen. Groep één (1) krijgt binnen een uur na de diagnose een vloeistofbolus van 20 ml/kg Ringer-oplossing voor infusie en groep twee (2 ) krijgt binnen een uur na de diagnose een vloeistofbolus van 40 ml/kg Ringer-oplossing voor infusie. Alle patiënten die de diagnose acute pancreatitis hebben, maar niet akkoord gaan met deelname aan onderzoek of andere uitsluitingscriteria hebben, zullen worden behandeld volgens de normen van goede klinische praktijken en de aanbevelingen van de huidige klinische richtlijnen voor de behandeling van acute pancreatitis. Uitsluitingscriteria naast weigering om geïnformeerde toestemming te ondertekenen zijn: leeftijd jonger dan 18 jaar, zwangerschap, patiënten met lichamelijke tekenen van hartfalen, eerder bekend congestief hartfalen (NYHA III - IV) en ernstige nierziekte meer dan een maand voor de diagnose acuut pancreatitis (gedefinieerd als een ernstige vermindering van de eGFR van minder dan 30 ml/min/1,73 m²). Patiënten die acuut nierfalen ontwikkelen als gevolg van acute pancreatitis worden bij het onderzoek betrokken.
Gezien het belang van vroege hydratatie bij patiënten met acute pancreatitis en het ontbreken van een duidelijk gedefinieerde dosis toegediend vocht in de huidige literatuur, was het doel van deze studie om de invloed te bepalen van de grootte van de dosisbolus van intravasculair vocht na de diagnose acute pancreatitis in het verdere verloop en de afloop van de ziekte.
De studie zal worden uitgevoerd op de afdeling spoedeisende hulp, Clinical Hospital Centre, Rijeka.

Beschrijving
Inclusiecriteria:
Alle patiënten met de diagnose acute pancreatitis.
Uitsluitingscriteria:
- Weigering om geïnformeerde toestemming te ondertekenen
- Leeftijd jonger dan 18 jaar
- Zwangerschap
- Patiënten met lichamelijke tekenen van hartfalen,
- Eerder bekend congestief hartfalen (NYHA III-IV)
- Ernstige nierziekte meer dan een maand voor de diagnose van acute pancreatitis (gedefinieerd als een ernstige vermindering van de eGFR van minder dan 30 ml/min/1,73 m2) . Patiënten die acuut nierfalen ontwikkelen als gevolg van acute pancreatitis worden bij het onderzoek betrokken.
