Gedetailleerde beschrijving
Deze gerandomiseerde fase II-studie heeft tot doel de werkzaamheid en toxiciteit te onderzoeken van fractionele thoracale radiotherapie in combinatie met albuminegebonden paclitaxel en nedaplatine tweemaal per week bij de behandeling van lokaal gevorderde niet-kleincellige longkanker in vergelijking met wekelijkse chemotherapie.
Radiotherapie:
Alle patiënten krijgen eenmaal per dag, 5 dagen per week intensiteitsgemoduleerde radiotherapie (IMRT). De eerste trap: DT 3000 cGy / 6 FX; de tweede trap, DT 3500 cGy / 7 FX. Ofwel: het eerste segment: DT 4000 cGy / 10 FX; het tweede segment DT 2400 cGy / 6 FX. Het interval tussen twee stadia van radiotherapie was 4 weken.
Gelijktijdige chemotherapie:
De patiënten werden willekeurig verdeeld in twee groepen en kregen een van de volgende gelijktijdige chemotherapieregimes:
Groep A: gedurende dezelfde periode van radiotherapie werd 30 mg/m2 albuminegebonden paclitaxel tweemaal per week gedurende 0,5 uur intraveneus toegediend, en nedaplatine 10 mg/m2 tweemaal per week gedurende 0,5 uur.
- Wanneer de radiotherapiedosis 3000 cGy / 6 FX + DT 3500 cGy / 7 FX was, was de infusietijd D1, D4, D8 van de eerste fase van radiotherapie; D1, D4, D8 van de tweede fase van radiotherapie.
- Wanneer de radiotherapiedosis 4000 cGy / 10 FX + DT 2400 cGy / 6 FX was, was de infusietijd D1, D4, D8, D11 van de eerste fase radiotherapie en D1, D4, D8 van de tweede fase radiotherapie.
Groep B: gedurende dezelfde periode van radiotherapie werd eenmaal per week albuminegebonden paclitaxel (50 mg/m2) gedurende 0,5 uur intraveneus toegediend; nedaplatine (25 mg/m2) werd eenmaal per week gedurende 0,5 uur intraveneus toegediend. De infusietijd was D1, D8 van de eerste radiotherapie en D1, D8 van de tweede radiotherapie.

Beschrijving
Inclusiecriteria:
- Histologisch bevestigde niet-kleincellige longkanker.
- Er waren meetbare ziekten volgens recist1.1.
- Niet-operatieve fase III (AJCC / UICC achtste editie-fase).
- Wanneer de laagste dosis radiotherapie 60Gy was, V20 > = 35% in beide longen.
- 18 jaar of ouder, jonger dan 75 jaar, ongeacht geslacht.
- ECoG fysieke statusscore (zie bijlage 1) was 0-1.
- Eerste behandeling of chemotherapie.
- Er was geen eerdere thoraxradiotherapie, immunotherapie of biotherapie.
- Neutrofielen absolute waarde ≥ 2000 cellen / mm3, bloedplaatjes ≥ 100000 cellen / mm3
- Serumcreatinine ≤ 1,25 maal ULN of creatinineklaringssnelheid ≥ 60 ml/min.
- Serumbilirubine ≤ 1,5 keer ULN, AST (SGOT) en ALT (SGPT) ≤ 2,5 keer ULN, alkalische fosfatase ≤ 5 keer ULN.
- CB6 normaal bereik.
- De patiënt en zijn familieleden gingen akkoord en ondertekenden de geïnformeerde toestemming.
Uitsluitingscriteria:
- Er waren andere kwaadaardige tumoren in het verleden of tijdens de behandeling, behalve niet-melanoom van de huid of in situ cervicaal carcinoom.
- Elke andere ziekte of aandoening is gecontra-indiceerd voor chemotherapie (bijv. actieve infectie, binnen 6 maanden na een myocardinfarct, symptomatische hartziekte, waaronder instabiele angina pectoris, congestief hartfalen of ongecontroleerde aritmie, immunosuppressieve therapie).
- Zwangere vrouwen of vrouwen die borstvoeding geven, vrouwen die geen zwangerschapstest hebben ondergaan (binnen 14 dagen voor de eerste toediening) en zwangere vrouwen.
- Bloedingsneiging.
- Deelnemers aan andere klinische onderzoeken binnen 30 dagen vóór het experiment.
- Drugsverslaving, langdurig alcoholisme en AIDS-patiënten.
- Er zijn oncontroleerbare aanvallen of verlies van zelfbeheersing als gevolg van een psychische aandoening.
- Patiënten met een ernstige allergische voorgeschiedenis of specifieke constitutie.
- De onderzoekers vinden het niet gepast om mee te doen aan dit experiment.
