Effectiviteit van Dry Needling bij pijn, functionele beperkingen, houdingscontrole en pijndrukdrempel bij patiënten met chronische niet-specifieke lage rugpijn: een dubbelblinde, gerandomiseerde, gecontroleerde studie
Lage rugpijn is een groot gezondheidsprobleem dat de psychosociale, economische, functionele en fysieke aspecten van het leven van de patiënt beïnvloedt. Het doel van deze studie is het onderzoeken van de effectiviteit van dry needling op pijn, functionele beperkingen, houdingscontrole en pijndrukdrempel bij patiënten met chronische aspecifieke lage-rugpijn met behulp van een gerandomiseerde gecontroleerde onderzoeksopzet.
Patiënten met chronische aspecifieke lage-rugpijn worden willekeurig verdeeld in twee groepen: de experimentele groep (dry-needling en routinematige fysiotherapie) en de controlegroep (schijn-dry-needling en routinematige fysiotherapie). Primaire uitkomsten zijn pijnintensiteit en functionele beperkingen. Houdingscontrole en pijndrukdrempel worden als secundaire uitkomstmaten beschouwd.

Lage rugpijn is een groot gezondheidsprobleem dat de psychosociale, economische, functionele en fysieke aspecten van het leven van de patiënt beïnvloedt. Aangezien patiënten met chronische lage-rugpijn een verminderde houdingsregulatie hebben en deze stoornis chronische lage-rugpijn kan veroorzaken en terugkeren, is het belangrijk om de effectiviteit van interventies op houdingsregulatie bij patiënten met lage-rugpijn te evalueren. Eerdere studies hebben de effectiviteit aangetoond van dry needling op pijn, functionele beperkingen en pijngevoeligheid bij patiënten met lage rugpijn. De effectiviteit van dry needling op houdingsregulatie is niet volledig bekend.
Het doel van deze studie is het onderzoeken van de effectiviteit van dry needling op pijn, functionele beperkingen, houdingscontrole en pijndrukdrempel bij patiënten met chronische aspecifieke lage-rugpijn.
Deze studie zal een dubbelblinde gerandomiseerde gecontroleerde studie zijn. Veertig patiënten met chronische aspecifieke lage-rugpijn worden willekeurig verdeeld in twee groepen: de experimentele groep (dry-needling en routinematige fysiotherapie) en de controlegroep (schijn-dry-needling en routinematige fysiotherapie). Primaire uitkomsten zijn pijnintensiteit en functionele beperkingen. Houdingscontrole en pijndrukdrempel worden als secundaire uitkomstmaten beschouwd. De resultaten worden voor en een week na de interventie beoordeeld.
Alle deelnemers worden geïdentificeerd en geworven door middel van posters en mond-tot-mondreclame van de universiteit en de omliggende lokale gemeenschappen. In aanmerking komende deelnemers zullen worden uitgelegd over het doel en het onderzoek dat betrokken is bij dit onderzoek, en alle in aanmerking komende deelnemers zullen een schriftelijke geïnformeerde toestemming ondertekenen voordat ze aan het onderzoek beginnen.

Beschrijving
Inclusiecriteria:
- Patiënten zijn tussen de 18 en 45 jaar oud.
- Matige pijn in rust (tussen 30 en 60 in NPRS).
- Patiënten met triggerpoints in de lumbale multifidus-spier.
- Patiënten kunnen Perzisch spreken en lezen.
Uitsluitingscriteria:
- Gebruikt momenteel antistollingsmiddelen
- Specifieke lage-rugpijn (neurogene lage-rugpijn, spinale stenose, cauda-equinasyndroom, spondylolisthesis en aanwezigheid van tekenen of symptomen van niet-musculoskeletale pathologie, b.v. kanker, infectie en fracturen in de lage rug en onderste ledematen op basis van paraklinische bevindingen)
- Eerdere operatie aan de lumbosacrale wervelkolom
- Onvermogen om liggend te worden
- Ernstige afwijkingen in de cervicale, thoracale, lumbale of bekkenregio en de onderste ledematen
- Geschiedenis van niet-gecorrigeerde visusstoornissen, vestibulaire, gehoor- of cognitieve stoornissen
- Beenlengteverschil dat het evenwicht verstoort
- Systemische ziekten, zoals diabetes, fibromyalgie, reumatoïde artritis, degeneratieve ziekten en andere reumatoïde ziekten
- Naald fobie
- Sacro-iliacale pijn geïdentificeerd met zes klinische tests: compressie, afleiding, sacrale stuwkracht, dijstoot, Gaenslen's en FABER's

Experimenteel: Experimentele groep
De experimentele groep zal bestaan uit 20 deelnemers met chronische aspecifieke lage rugpijn.
De behandeling omvat dry-needling van de lumbale multifidus-spier en routinematige fysiotherapie.
De naalden worden ingebracht om een lokale spierrespons te verkrijgen, en dit proces gaat door totdat er geen lokale spierrespons meer optreedt in elke sessie.
Daarna blijven de naalden 20 minuten zitten.
Routinematige fysiotherapie omvat lasertherapie op laag niveau en motorische controletraining.
De behandeling duurt 3 weken, 6 sessies, tweemaal per week.

De patiënten worden in buikligging geplaatst met een kussen onder hun buik om lumbale lordose op te vangen.
In dit onderzoek zullen twee lengtes van steriele, wegwerpbare, 0,30 mm x 75 mm en 0,30 mm x 50 mm vaste filamentnaald (Tony, China) worden gebruikt.
De lengte van de naald voor elke deelnemer wordt geselecteerd op basis van de grootte van de deelnemer.
De naalden worden ingebracht in de lumbale multifidusspier, 1,5 - 2 cm lateraal van het processus spinosus van twee of drie pijnlijke segmenten, loodrecht op de lamina (na het doorprikken van de huid worden de naalden inferomediaal gericht).
De naalden worden ingebracht om een lokale spierrespons te verkrijgen, en dit proces gaat door totdat er geen lokale spierrespons meer optreedt in elke sessie.
Daarna blijven de naalden 20 minuten zitten.

Placebo-vergelijker: Controlegroep
De controlegroep zal bestaan uit 20 deelnemers met chronische aspecifieke lage rugpijn.
De behandeling omvat "sham" dry-needling van de lumbale multifidus-spier en routinematige fysiotherapie.
De sham dry needling-methode bestaat uit het subcutaan inbrengen van de naalden en er wordt geen lokale spiertrekkingsreactie verkregen.
De naalden blijven 20 minuten op hun plaats.
Routinematige fysiotherapie omvat lasertherapie op laag niveau en motorische controletraining.

De 0-100 Numeric Pain Rating Scale (NPRS) wordt gebruikt om de verandering in pijnniveau vanaf de basislijn tot een week na de interventie te evalueren.
Nul geeft geen pijn aan en 100 geeft maximale pijn aan die de patiënt ervaart.
De NPRS heeft de responsiviteit om het pijnniveau te meten bij mensen met lage rugpijn.

De Perzische versie van de Oswestry Disability Index zal worden gebruikt om veranderingen in functionele beperkingen vanaf de basislijn tot een week na de interventie te evalueren.
Deze vragenlijst bevat 10 activiteiten van het dagelijks leven en een totaalscore van 100.
Een score onder de 25 geeft het laagste niveau van invaliditeit aan, 25 tot 50 matige invaliditeit, 50 tot 75 ernstige invaliditeit en 75 tot 100 acute invaliditeit.
De validiteit en de betrouwbaarheid van de Perzische versie van deze vragenlijst is in eerdere studies aangetoond.

De drukalgometer wordt gebruikt om de pijndrukdrempel (PPT) te evalueren.
De patiënten worden in buikligging geplaatst met een kussen onder hun buik om lumbale lordose op te vangen en de algometer wordt loodrecht op de spierbuik van de lumbale multifidus aangebracht, ongeveer 1,5 cm lateraal van het processus spinosus van de pijnlijke segmenten.
De patiënten zal worden gevraagd om verbaal te signaleren na perceptie van krachtverandering van druk naar pijn.
De PPT op elke locatie wordt drie keer genomen en het gemiddelde van drie herhalingen wordt gebruikt voor statistische analyse.
De validiteit en betrouwbaarheid van de drukalgometer is in eerdere onderzoeken aangetoond.

Houdingsregulatie

Het krachtenplatform (Duitsland, Kistler, 9260AA6) zal worden gebruikt om de houdingscontrole te evalueren vanaf de basislijn tot één week na de interventie.
De houdingscontrole zal worden geëvalueerd in vier verschillende onderzoeken in stand met twee benen: 1-Ogen open op een stevige ondergrond.
2-Ogen gesloten op een stevige ondergrond.
3-ogen open op een schuimoppervlak.
4-ogen gesloten op een schuimoppervlak.
De duur van elke poging is 90 seconden, driemaal herhaald, met een rustinterval van één minuut tussen elke poging.
De posturale controlevariabelen in dit onderzoek zijn COP-excursie in anteroposterieure en mediolaterale richting, COP-amplitudestandaardafwijking (spreiding) in anteroposterieure en mediolaterale richting, COP-snelheidsstandaardafwijking in anteroposterieure en mediolaterale richting, gemiddelde snelheid van COP, COP-totaal excursiegebied, COP-vlakfase in anteroposterieure en mediolaterale richting, en COP-totale vlakfase.
